POSITION SUMMARY: The Hospice Admissions Visit Nurse is responsible for evaluating and admitting patients to Hospice; for documenting observations and interventions related to the patient's environment, physical, mental, emotional and social status; and for the coordinating care with identified primary Case Manager and designated Clinical Manager as appropriate. AREAS OF RESPONSIBILITY: * Admits patients to service. Assesses patient's environment; physical, mental and social status and eligibility for hospice services in accordance with hospice admission requirements. * Assures completion of all documentation including the plan of care * Assures that all applicable services are ordered, explained, and offered. Obtains physician orders for services, equipment, medication as needed. * Assesses need for and coordinates services of all other IDG member, including Physician, Social Worker, Home Health Aide, Dietician, Volunteer, and Spiritual Care. * Documents observations, clinical findings, problems, skilled interventions, goals and plans in a timely manner (including documenting appropriately on Point of Care/laptop device). * Makes skilled nursing revisits to established patients as directed by Patient Care Manager. * Provides and documents skilled nursing care (includes skilled observation of the patient's condition, skilled care and procedures and teaching of the patient and/or family), on all intermittent visits and/or shifts. * Provides supervisory visits for the Home Health Aides as directed and as they spontaneously occur. * Demonstrates all skills of a registered nurse, including those skills required by the agency through competency testing. * Maintains and updates own level of knowledge and skills required to provide quality hospice nursing care. * Participates in QA / PI activities and in-service education related to areas of practice as appropriate. * Assists in development of hospice protocols, procedures and policies as requested. * Performs other duties as assigned consistent with skills and training and the mission and goals of Pathways. Develops an individualized plan of care based on the assessment. ● Obtains informed consent from patients and families and assists in completing forms required for Hospice and Home Health admission per regulations. ● Provides patients and their families with guidance regarding health care decision-making and options for making advance directives in coordination with the social worker. ● Assesses all aspects of the patient's pain and other symptoms and develops an individualized symptom management plan. ● Anticipates, prevents and treats undesirable symptoms or secondary symptoms including use of non-pharmacologic measures. ● Supports, instructs and educates the patient, family, caregivers, and facility staff. ● Documents nursing assessments, problems, appropriate goals, care interventions, patient and family outcomes, and evaluation of these in the electronic record. ● Accesses electronic device during patient visit and documents during or immediately after the visit or per regulation. ● Reports regularly to Shared Services After Hours Triage RN, as appropriate, and RN Case Manager or designee regarding care provided, and when patient condition is found to be unstable and there is a need for changes in current plan of care. ● Supports, instructs and educates the patient, family, caregivers, and facility staff. ● Attends educational in-service trainings, and other agency meetings as requested by manager. ● Maintains the dignity of the dying patient and supports the patient's and family's unique spiritual and cultural beliefs. ● Provides holistic, family-centered care across the treatment settings to improve the quality of life. ● Knowledgeably discusses and initiates non-pharmacologic interventions as appropriate to promote comfort, improve quality of life, and help manage emotional, spiritual, and physical distress. ● Participates in the hospice program's quality assessment performance improvement program and in-service education related to areas of practice as appropriate. ● Meets productivity standards as established by the manager. ● When in a facility, adheres to “Pathways Standards of Care and Practice in Facilities” or equivalent document. ● Prints out and files visit documentation in facility (RCFE or SNF) binder or chart per facility preference at the time of visit. ● Maintains timely communication with patient, family, and facility staff regarding all aspects of patient care as appropriate. ● Demonstrates awareness of and adherence to the safety, infection control and other policies of the facility. ● Maintains and updates level of knowledge and skills required to provide quality nursing care. ● Creates, documents, and maintains the medication list.
