Salary Range: $113,506 - $141,887 annually, based on relevant years of experience

Role Overview:
Responsible providing individual, family, and group therapy services in acute care settings for youth and adolescent clients experiencing a range of emotional and behavioral challenges. The Clinical Psychologist will complete psychological screening and assessments across programs. Services will include providing evaluation, consultation, and monitoring of individuals receiving involuntary care. Position will work in a multidisciplinary team of providers and is critical to supporting the mental health and developmental needs of young people in our community.
Essential Duties:
Conduct age-appropriate clinical interviews, screenings, psychological assessments, and testing in acute care programs.
Diagnose emotional, behavioral, and developmental disorders specific to youth populations.
Develop and implement individualized, developmentally appropriate treatment plans in collaboration with clients, families, and care teams.
Deliver evidence-based therapy (individual, group, and family) focused on supporting youth mental health.
Consult with school personnel, pediatricians, case managers, and other providers to coordinate care.
Required:
Doctoral degree (PhD or PsyD) in Clinical Psychology from an accredited institution required.
Minimum of 2 years of experience working with children and adolescents in clinical settings (school-based, outpatient, or community behavioral health strongly preferred).
Current and valid DORA license to practice as a Clinical Psychologist in the state of CO.
Highly Desired:
Proven experience in child and adolescent development, trauma-informed care, and family systems approaches.
Bilingual/bi-cultural preferred
Additional Requirement - Longview Campus

Role Overview:
We are hiring for a Therapist for our Assertive Community Treatment (ACT) and Specialized Treatment Services (STS) Teams.
The ACT and STS Teams are a multi-disciplinary team of: case managers, therapists, providers, medical assistants, and nursing staff. ACT and STS clients are often not successful in typical Outpatient Services and need dynamic approaches and interventions to gain and maintain stability.

The team follows the SAMHSA's ACT Evidence Based Practice as a fidelity program with the State of Colorado.
Essential Duties:
Provide action oriented brief therapy to clients starting services
Provide Evidence Based treatment for a range of mental health and substance use disorders
Provide on-going assessments, crisis intervention and case management to clients and their families
Develop and monitor service plans, treatment progress, and maintains clinical documentation and other reporting data
Coordinate services in a treatment team model with internal as well as external services
Have a strong understanding of how to adapt service delivery to meet the client's needs.
Required:
Master's degree in related field from accredited college or university
1 year of experience providing individual and group therapy
Proficiency in using computers and telehealth systems • Certified Addiction Specialist (CAS), Licensed Addictions Counselor (LAC), or willingness to pursue a CAS or an LAC

Role Overview:
Responsible for providing assessment, crisis intervention, individual, family, and group therapy as well as case management. Develop and monitor service plans, treatment progress, document interactions and maintain clinical records and other reporting data for clients with mild to severe mental illness, disorders, and/or disabilities and substance use disorders. Coordinate with internal as well as external supportive services to deliver services to client. Direct client care includes care coordination, behavioral health related screenings and assessments, individual and group therapy, brief therapy, case management, and related clinical services.
Serves as a clinical resource person to building site(s), clinical staff, case managers, medical providers and collaterals.
Knowledge of professional policies and procedures.
Collaborate with other staff in providing, planning, developing, implementing, and evaluating clinical services, including collaborating with Clinical Managers and Program Supervisors.
Performs behavioral health screenings and assessments within the medical clinic setting, to include review of treatment options and referral information.
Provides on-going care coordination within SummitStone, community agencies and resources.
Provides crisis intervention to include crisis assessment and de-escalation.
Provides on-site collaboration with medical providers to provide behavioral health assessments for those clients identified with behavioral health needs.

Required:

Role Overview:
As a Behavioral Health Therapist on the Mobile Crisis team, you will serve as an integral member of the service delivery team, providing comprehensive support to individuals experiencing behavioral health crises.
Services are delivered to individuals whose behavioral health needs cannot be safely managed in the community or in less restrictive settings, including those who may require longer-term respite care. This position involves cross-training and coverage across multiple programs and sites, including Garcia House, Behavioral Health Urgent Care, Mobile Response Services, and the Crisis Stabilization Unit.
Essential Duties:
Providing on-going crisis assessments, crisis intervention techniques, conduct de-escalation, safety planning, and maintaining clinical records and other reporting data for clients with mild to severe mental health and/or substance use disorders.
Observe clients in a variety of situations; assess mental status of client and formulate treatment plans and goals. Perform case management services as needed to assist clients to meet their goals as stated in their service/treatment plans. Refer clients to supportive services such as vocational, medical, social security, housing authority, etc.
Conduct crisis intervention via telephone and/or face-to-face. Intervene and de-escalate crisis situations with clients. Assess the risk of harm to self or others, create comprehensive safety plans, and arrange needed resources.
Assist clients in improving their well-being and quality of life by training in behavioral skills; educating clients to make informed choices; developing clients coping skills, helping clients to problem solve, and negotiating tasks of daily living.
Document treatment plans, progress reports, group notes, individual notes, case.
management notes, letters, faxes and phone calls. Follow the proper protocol for documentation as set forth by Quality Assurance and to third party payor's requirements that have contracts with SummitStone.
Maintain close and cooperative relationships with community agencies and other public agencies in carrying out treatment/recovery/service plans.
Required:
Bachelor's degree in the Human Services field from an accredited College or University required

* Attend team conferences and report on an individual's progress and status in therapy as well as collaboratively assist the team in identifying realistic goals.
* Communicate effectively and work collaboratively with internal case manager.
Qualifications:
* Master's Degree in Social Work from an accredited school of social work.
* Membership in NASW and related clinical experience in a rehabilitation setting with a focus on traumatic brain injury and spinal cord injury.

An Average Day:
(Includes, but not limited to)
* Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group
* Provide an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
* Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plan intervention based on evaluation findings
* Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties; provide crisis intervention when necessary
* Assess for, and educate interdisciplinary group, on any special needs related to the culture of the patient and family, including communication, role of family, space, and any special traditions or taboos
* Maintain clinical records on all patients referred to social work
* Educate patients and families on, and assist in, preparation of advanced directives
* Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs
* Provide information to patients and families/caregivers and community agencies
* Serve as liaison between patients and families/caregivers and community agencies
* Maintain collaborative relationships with organization personnel to support patient care
* Maintain and develop contacts with public and private agencies as resources for patient and personnel
* Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings, assist the team in recognizing the effects of the psychosocial stresses on symptoms of the terminal illness
* Assist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues
* Actively participate in quality assessment performance improvement teams and activities
* Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities
* Perform other duties as delegated by the Clinical Supervisor
Requirements:
* Must have a master's degree in social work from an accredited university.
* If you hold an undergraduate B.A, Degree in Social Work, you are eligible to apply. Please know the hirable rate of pay will be based on degree type.
* Must have minimum of two (2) years of documented supervised experience in health care, hospice experience preferred
* Must understand hospice philosophy, and issues of death/dying.
