The Recovery Support Specialists will act as an integral member of the ReVIDA Recovery

by building and strengthening linkages with community resources and family

among all the participants concerned with a patient's care to achieve safer and
more effective care. The Recovery Support Specialists will assist individuals in opioid use
disorder treatment in developing a service plan, referrals, and linkage, ensuring
the patient has access/continuity of care throughout the mental health and primary
care system, ensuring each patient has resources to acquire medication, has
transportation for appointments, and attends appointments. The Recovery Support Specialists
will ensure individuals have access to psychosocial rehabilitation, support,

community/natural supports to assist in managing substance use disorders.

Responsibilities
Round at assigned nursing homes and provide psychotherapy to their clients.
Work closely and collaboratively with nursing home staff, residents, and families.
Proactively follow up with patients and assess efficacy of recommended interventions.
Provide education on best practices.
Work with the psychiatric provider to integrate pharmacological and behavioral
interventions, emphasizing non-pharmacological treatments whenever possible.
Requirements
Master's or Doctoral degree in Psychology, Social Work, Counseling, or related field
Valid state licensure as a Psychologist (PhD or PsyD), Licensed Independent Social Worker (LISW)
Experience working in a nursing home setting

How you Will make a Difference
Deliver exceptional nursing services to our patients and residents maintaining the highest degree of quality and compassionate care.
Supervision of day-to-day activities performed by assigned nursing assistants
Operate in accordance with Federal, State, and Local Standards, guidelines and regulations and as required by Director of Nursing.
What you Need to make a Difference
Licensed Practical/Vocational Nurse with current state license.
Must have a current/active CPR certification.
Prior long term care experience preferred.
