DESCRIPTION OF WORK
As a Social Worker 2, you will develop, implement, and coordinate social work and case management services to inmates. You will provide support, guidance, crisis management, and counseling services to inmates and their families; motivate inmates to improve their social, emotional, physical, and mental health circumstances; and help inmates explore methods for adjusting, modifying, and improving their personal behavior. You will also participate in parole planning and work with Community Corrections Center placement to assist with reentry services. This role requires the ability to provide effective counselling services that will support inmates, as well as a genuine desire prepare inmates for successful reintegration. In addition, you will provide follow-up services to community placed inmates to validate continuity of care, investigate complaints or allegations of human rights violations, and ensure compliance with state and federal social work legislation, regulations, policies, and procedures.

Work Schedule and Additional Information:
* Full-time employment, 37.5 hours per week
* Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.

Minimum Experience and Training Requirements:
* Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (Commonwealth titles), (must have been employed in a Commonwealth social worker job title on September 8, 2008, with continuous Commonwealth employment); or
* Two years of experience as a Social Worker 1 or a Forensic Social Worker 1 (Commonwealth job titles or equivalent Federal Government job titles, as determined by the Office of Administration) and a bachelor's degree in social work or social welfare; or
* Two years of professional experience providing direct public or private social work services, and a bachelor's degree in social work or social welfare; or
* A master's degree in social work or social welfare.
* Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.
Special Requirements:
* All employees in the Department of Corrections must obtain Criminal Justice Information Services (CJIS) certification within two months of hire and maintain it for the duration of employment in the job.

JOB SUMMARY
Provides care management support services for consumers undergoing protective service investigations.
Assists senior caseworkers by conducting reassessment visits, sets up in-home services, performs service-related errands and completes necessary case documentation for all service activities.
REPORTING RELATIONSHIPS

Follows direction of senior caseworkers in establishing service for protective service consumers requiring in-home services and supports. Manages consumer cases. Completes comprehensive Individualized Service Plans (based on medical model and assessed need), documents according to requirements, interprets program policies and monitors consumer situations and provision of services on regular basis via home visits and phone contacts with consumers and caregivers to insure that services meet the needs of the consumer. Ensures a smooth and sucessful transition to other care mangagement service (OPTIONS/CSP/Community Health Choices). (40%).
Performs support service on behalf of the senior caseworkers including reassessment visits with consumer prior to case closure or transfer, conducts consumer errands and other related tasks at the direction of senior caseworker or supervisor. (25%).
Completes required paperwork and case documentation. Utilizes quality assurance reports to ensure all case documentation is complete and accurate (35%).
In the course of performing job duties, employee will be required on a routine basis to provide own transportation to consumers homes, meetings and other job-related appointments and access those locations without special accommodations.
OTHER SPECIFIC TASKS OR DUTIES
Maintains accurate knowledge of all aging services and thorough working knowledge of community services.
Attends all mandated training sponsored by Department of Aging relative to community services and programs (including enrichment sessions).

THE OPPORTUNITY
DOROT's Synagogue Partnerships Program currently supports nine synagogues in Manhattan, representing the full range of denominations within the Jewish community. The SPP team enhances synagogues' relationships with their older congregants by offering unique expertise on the evolving needs of older adults and providing social work support to clergy, staff, lay leaders, older adults, and caregivers.
POSITION DESCRIPTION
The Social Worker, Synagogue Partnerships serves as a vital link between the Jewish community and DOROT, ensuring that synagogues and older adult congregants have access to and benefit from DOROT's expertise, social work model, and programming. They will have the exciting opportunity to help DOROT deepen existing relationships and foster partnerships with new synagogues. The ideal candidate is a resourceful, responsive, and creative team member who has strong individual and group clinical skills; the ability to gain the trust and cooperation of synagogue staff, lay leaders, and members of each congregation; and confidence to take the initiative in community building efforts.
PRIMARY RESPONSIBILITIES
Serve as a liaison to 4-7 synagogue partners, working in collaboration with the Manager of Synagogue Partnerships Program on program opportunities, strategic growth, and individual case interventions.

THE OPPORTUNITY
DOROTs Synagogue Partnerships Program currently supports nine synagogues in Manhattan, representing the full range of denominations within the Jewish community. The SPP team enhances synagogues relationships with their older congregants by offering unique expertise on the evolving needs of older adults and providing social work support to clergy, staff, lay leaders, older adults, and caregivers.
POSITION DESCRIPTION
The Social Worker,Synagogue Partnerships serves as a vital link between the Jewish community and DOROT, ensuring that synagogues and older adult congregants have access to and benefit from DOROTs expertise, social work model, and programming. They will have the exciting opportunity to help DOROT deepen existing relationships and foster partnerships with new synagogues. The ideal candidate is a resourceful, responsive, and creative team member who has strong individual and group clinical skills; the ability to gain the trust and cooperation of synagogue staff, lay leaders, and members of each congregation; and confidence to take the initiative in community building efforts.
PRIMARY RESPONSIBILITIES
Serve as a liaison to 4-7 synagogue partners, working in collaboration with the Manager of Synagogue Partnerships Program on program opportunities, strategic growth, and individual case interventions.
Work with rabbis, synagogue staff, and lay leaders on issues related to older adults and caregivers.
Conduct outreach and home visits to older adults and caregivers and provide support to members of each synagogue based on their unique ends. This includes but is not limited to: completing comprehensive psycho-social assessments and providing case assistance, supportive counseling, and information and referrals to DOROT programs and other community services.
Lead or co-lead support groups on issues of social isolation, bereavement, caregiving, and other relevant topics.
