    The Pharmacy Technician I will perform many pharmacy-related functions under the direct supervision of a licensed pharmacist and perform administrative/operational functions of the department to including but not limited to medication preparation, distribution, and inventory maintenance. Primary Duties and Responsibilities: Medication Reconciliation Pharmacy Technician Duties: * Conduct in-person or phone meetings with patients to update medication lists prior to their provider appointments, ensuring accuracy and completeness of the medication list. * Practices as a pharmacy technician in accordance with all federal, state, DFCI and Joint Commission policies, regulations and standards. * Create and update lists of current medications, document changes in dose, frequency, or route, and flag medications for removal to prevent harmful drug interactions. * Communicate drug-drug interactions or other medication changes with pharmacists and providers to maintain patient safety. * Manage medication lists for medications listed from external entities, ensuring comprehensive and accurate records. * Utilizes multiple systems such as EHR (Electronic Health Record, Inventory Management Systems and other systems to complete and document duties. Troubleshoot as necessary. * Trains other employees as assigned. * Communicates and escalates issues or problems. * May be asked to provide cross coverage in the hospital outpatient/specialty pharmacy as needed to maintain seamless patient care. * Performs other duties as assigned.
