SUMMARY: Provide school-based or mental health counseling services and linkage to community services to students and families based on referrals from contract schools.

Verify TANF eligibility for students referred to the program.
Complete mental health assessments, providing accurate mental health diagnosis of assigned clients.
Complete treatment plans (ISPs) in collaboration with client, formulating goals, objectives and interventions relevant to client's diagnosis; and reviewing, at least quarterly, on-going medical necessity of those goals (and/or diagnoses) in conjunction with client's progress in treatment.
Provide timely and well-written documentation on all clients to assist therapist in developing the minimum of mental health assessments, ISPs, discharge summaries, progress notes and recommendations when necessary. Progress notes conducted in collaboration with the client and/or family member, as possible should be submitted no less than weekly for billing purposes.
Provide individual and/or family mental health counseling and appropriate and effective evidence-based therapeutic interventions according to client's treatment plan.
Provide supportive outreach to parents of students referred to the program; this may occur in the form of phone calls or home visits.
Link students and families with additional services as needed.
Assist families to prepare students to have structured activities planned and organized for them to engage in during the summer months; this will include linking students with summer school, summer camps, sports activities, arts camps, summer jobs, etc and securing funding through various sources to assist students in attending these activities.
Coordinate and/or facilitate wrap-around meetings and family team meetings with school personnel
Maintain consistent contact with community team; which could include juvenile court, children's services, family first council, schools and other applicable social service agencies.
Participate in Quality Improvement activities, including, but not limited to, peer review and case record reviews.
Discharge clients according to progress, completing necessary paperwork and making appropriate referrals for after care.
Attend community and agency meetings as needed, including, but not limited to staff meetings, clinical and/or small group supervision as required to increase & maintain competencies in mental health field.
Engage in on-going training and maintenance of licensure and/or mental health credentials.
Engage in supervision to evaluate counseling techniques and interventions.
Submit accurate timekeeping records by payroll deadlines.

Completes appropriate documentation and paperwork including, but not limited to, progress notes after each therapy session, step-down/extension/discharge recommendations, Behavior Intervention Plans, Mental Health Assessments, Discharge/Transfer summaries and ITP's. Coordinates services with psychiatric and psychological services. Develops and maintains appropriate records on clients.
Participates in various assigned committees.
Participates in referral team, giving input on which clients would fit program well. Participates in referral interviews as needed. Facilitates assessment for new intakes and works to engage client in programming.
Available 24/7, as appropriate, to meet the needs of residents on-site or face-to-face via interactive videoconferencing. Provides lethality assessments/recommendations for care.
Facilitates case management by designing, writing and coordinating individualized treatment plans (ITP) for each client.
