Essential Work Functions:
Evaluate patients' physical conditions using standardized tests, clinical examinations, and patient history to identify functional limitations and treatment need
Develop personalized rehabilitation programs based on assessment findings and patient goals
Implement physical therapy treatment plans using various techniques to improve patient mobility and reduce pain
Operate specialized physical therapy equipment and modalities safely and effectively, ensuring proper maintenance and calibration of devices
Assess patient progress, adjust treatment plans as necessary, and accurately document evaluations, interventions, and outcomes in the medical record
Work closely with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated care and optimize treatment outcomes
Educate patients and caregivers on home exercise programs, proper body mechanics, injury prevention strategies, and self-management techniques to promote long-term functional recovery
Maintain a safe treatment environment by adhering to infection control protocols, safety guidelines, and regulatory standards while addressing patients' comfort needs during therapy sessions
Engage in quality assurance initiatives and professional development activities to remain current with best practices in physical therapy
Collaborate in discharge planning to ensure smooth transitions and continued patient progress in community or home-based settings
Perform other duties as assigned within scope of practice

Principal Accountabilities

$$77,400 - $171,000 annual total cash target pay
$$44.65 - $98.65 per visit point

Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of “direct” and “indirect” patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable. Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association
Currently licensed (licensed physical therapist) in the state of Texas.
Two years experience preferred.
Ability to exercise initiative and independent judgment.
Spanish speaking a plus
Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)
Responsibilities:
Makes the initial assessment visit and generates OASIS documentation the same day of the visit
Understands and adheres to establish policies and procedures.
Provides physician prescribed physical therapy.
Performs evaluation and treatment visits.
Improves or minimize the residual physical disabilities of the patient.
Returns the individual to optimal and productive level within the patients capabilities.
Participates with all other healthcare personnel in patient care planning.
Direct and supervise their personnel as required.
Performs all skilled procedures as ordered by physician.
Consults with physicians regarding change in treatment ,reports to physicians regarding patients progress, instructs patients and families/significant others in home programs and activities of daily living.
Participates in in-service programs and presents in service programs as assigned.
Participates in performance improvement activities as assigned.
Attends all patient care conferences as scheduled.
Prepares clinical and progress notes daily.
Assists physicians with evaluating level of function.
Helps develop the plan of care and revise as necessary.
Consults with family and agency personnel.

This is a contract position.
Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons
Diagnose movement dysfunction by listening to patients and observing them as they move about
Develop an individualized treatment plan
Coach patients and teach them therapeutic exercises and stretches to improve their condition
Use hands-on therapy such as massage to ease patients pain and provide stimulation to promote healing
Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
Advise patients and their families on in-home treatment options and what to expect from the recovery process
