Primary Duties and Responsibilities Consults with and/or makes recommendations to physicians and other allied health professionals regarding patients' nutritional statuses nutrient needs or specific diet plans. Formulates nutrition assessment evaluation and follow-up plans for patients at nutrition risk as ordered by providers or according to departmental policy. Recommends and documents patient's nutrition treatment findings and plan in the patients' medical record. Participates in clinical rounds and appropriate department/service meetings and communicates appropriate information to administrative personnel in the service area. Consults in/outpatients in diet rationale for self-management. Must be flexible to cross-cover and train in all areas including Neonatal ICU and Pediatrics as needed. Provides staff relief and cross-coverage as delegated for in/outpatient services. Reviews at least one annual review of literature pertinent to area(s) of responsibility and present to peers in Journal Club. Performs at least one formal professional presentation in an area of specialty to allied health professionals or the community. Attends pertinent meetings to enhance clinical practice to growth records and documents in CDR Professional Development Plan. Maintains current CDR credentials and DC Licensure and completes yearly competencies as outlined by MGUH. Hold current membership to professional organization pertinent to the role. Participates in licensing surveys as needed. Assumes other duties and responsibilities that are appropriate to the position and area. As part of Unity's responsibilities, they are required to provide evaluation and assessment of medical diets provided by the correctional food vendor. They also have a small number of patients with specific conditions who need nutrition assessment and counseling. A part-time registered dietician nutritionist is needed to fulfill these functions for approximately 2 hours/week or 8 hours/month. MAJOR DUTIES Summary of Responsibilities 1. Diet/menu evaluation § Evaluate all diets served at DC correctional facilities for nutritional adequacy, at least annually or whenever the menu is changed substantially. § Provide a report of the evaluation findings. § Monitor the effectiveness and appropriateness of therapeutic diets served at DC correctional facilities 2. Nutritional Instructions & Education: § Provide tailored nutrition education to patients receiving dialysis treatment § Educate patients about nutrition 3. Monitoring & Documentation: § Document nutrition education and counseling services in patients' medical records. § Review and interpret lab results to identify nutritional deficiencies or markers of chronic disease. 6. Research & Development: § Stay updated with current research and evidence-based practices in nutrition. § Participate in programmatic data collection and report preparation. 7. PRINCIPAL RESPONSIBILITIES AND DUTIES: * Assesses patient's knowledge and provides education regarding the relationship of diet and kidney disease and the impact on the treatment process. * Provides education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process. * Completes nutritional assessment and works with the interdisciplinary team to develop an individualized comprehensive plan of care for patients. * Maintains complete and accurate medical records including progress notes. * Provides ongoing assessment and counseling necessary to assist patient in achieving and sustaining an effective nutritional status. * Identifies malnourished or at-risk patients and works collaboratively with the interdisciplinary team to identify appropriate interventions, resources, or solutions. * Monitors, adjusts, and documents response to nutrition therapy and addresses any issues impacting the patient's ability to achieve the goals. * Collaborates with the interdisciplinary team on Urea Kinetic Modeling recommendations to achieve the physician's prescription of Kt/V. * Reports on nutrition QAI results and participates in the interdisciplinary QAI program. * Reviews nutrition related lab results: * Counsels patient and/or caregiver and formulates appropriate action. * Recommends treatment changes to the interdisciplinary team as appropriate. * Identifies candidates to refer to physician for nutritional supplements. * Communicates with physician and/or facility staff as appropriate. * Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable. EDUCATION: * Registered Dietitian as per Commission on Dietetic Registration. * Board Certified Specialist in Renal Nutrition encouraged. * Current state licensure if applicable. * EXPERIENCE AND SKILLS: * 1+ year experience in clinical nutrition as a Registered Dietitian. * Previous renal experience preferred. May be exposed to infectious and contagious diseases/materials. The rate of pay for this position will depend on the successful candidate's work location and qualifications, including relevant education, work experience, skills, and competencies.
