This 13-week assignment supports a high-performing ambulatory care network where the LVN will assist in vascular procedures, provide direct patient care, and float to the Urology Clinic within the same facility as needed. The role is ideal for a clinically strong LVN who excels in a fast-paced, team-oriented environment and demonstrates proficiency in IV therapy, phlebotomy, and Epic charting. Responsibilities: The LVN will deliver safe and efficient nursing care in an outpatient setting, assist physicians with procedures, and perform sterile field setups for vascular interventions. Primary duties include administering medications (oral, IM, SQ), performing IV therapy and phlebotomy, conducting vital signs and pain assessments, and managing wound care and dressing changes. The clinician will also complete patient admissions, reconcile medications, interpret laboratory results, and ensure accurate documentation in Epic. Additional responsibilities include supporting telephonic triage, performing Foley catheter insertions, administering vaccines, and maintaining aseptic techniques. Collaboration with the interdisciplinary care team is key to ensuring optimal clinical outcomes and patient satisfaction. The LVN Program Manager is responsible for implementing program plans and services for participants, and managing and supervising Direct Support Professionals (DSPs). The LVN collaborates closely with participants, their support team members, program management staff, and direct care staff to foster a safe, inclusive, and positive environment. RESPONSIBILITIES Supervisor: Ensures compliance with all applicable laws and regulations regarding services for adults with developmental disabilities including reporting suspected abuse or neglect. Maintains a safe and clean environment for the program. Substitutes on the floor as a DSP when needed. Participates in staff recruitment. Interviews and evaluates employee candidates. Makes hiring recommendations to Program Director. Orients, trains, and develops staff. Supervises DSP's. Visit our websites to learn more: ******************** | ******************** See below for more job information: Essential Duties and Responsibilities: The Adult Skills Center (TASC) provides innovative and proactive services that empower individuals with diverse intellectual, developmental, and mental health needs to achieve their highest level of Independence and realize their greatest potential. The Licensed Vocational Nurse and Licensed Psychiatric Tech perform general nursing procedures, behavior management, direct care and training as dictated by the individuals' health and behavioral care needs. Primary Duties: Assists with administration of medication policy to document and observe patient medication regimen. Participates as a member of the interdisciplinary team and contributes to the development of the Individual Support Plan. The LVN/LPT may function as lead staff on the shift and may oversee DSPs and/or CNAs. Responsible for providing active programming at all times. Assists with crisis management procedures as needed. Provide support during client's vocational training. You may be required to work alongside client, engaged in the same job duties and adhering to same policies and procedures of the workplace (may include washing dishes, janitorial services, etc.). Responsible for adherence to standards, procedures and schedules established by their supervisor. Observes patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment and taking any necessary action. Evaluates medical concerns and directs patients to emergency medical care when required. Works as part of a health care team to assess patient needs, plan and modify care and implement interventions. Assists the RN in overseeing patients Facilitates educational groups on health and medication management. Provides monitoring and supervision of patients to support non-clinical staff. Transport clients in company vehicle/personal vehicle to and from program as well as to any scheduled activities throughout the day. All other duties as assigned. Qualifications: Possess and maintain a valid, active license from the State of California to practice as a Licensed Vocational Nurse or Licensed Psychiatric technician. Must have current California Driver's license/Insurance and clean DMV driving record. Successful completion of pre-employment physical and drug screen, including current TB Successful completion of DOJ and FBI background check Successful completion of all trainings as required by applicable regulations, including, but not limited to PCM, DSP1 and DSP2 The individual must be willing to work with a wide variety of symptoms related to mental health and developmental disabilities. One year of experience working with persons with developmental disabilities required. Maintain all certifications up to date Requires initiative, creativity and the ability to work independently with minimal supervision. Physical Requirements: Ability to drive company and/or personal vehicle for work, including transporting clients to and from work and/or appointments as needed, maintain valid driver's license and car insurance, if required to drive for position. Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards. Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards While performing the duties of this job, the employee is frequently required to stand, walk, and sit. The employee must occasionally lift and/or move up to 100 pounds. The employee must provide physical assistance and/or push clients up to 250lbs in a wheelchair. Must be able to assist adult clients transitioning in and out of vehicle and in the restroom.
