Position Overview Under the supervision of the Site Medical Director, Nurse Practitioners will deliver comprehensive, compassionate, and thoughtful care to patients with complex chronic diseases within the New York City jail system. This role involves collaboration with physician leadership to provide general primary care, which includes conducting detailed histories and physical exams, diagnosing and managing both acute and chronic illnesses, and assessing the necessity for consultative services. In addition, the Nurse Practitioner will engage as part of an interdisciplinary team, focusing on patients with chronic medical conditions or physical disabilities who do not require acute inpatient admission. This approach ensures a holistic view of patient care and leverages the expertise of diverse healthcare professionals to optimize treatment outcomes. Responsibilities include: (Detailed Task) Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated. Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable. Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits. Implement plans for patient care utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care. Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate. Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented. Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed. Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags). Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care. Ensure that all progress notes and orders are signed before the end of the shift. Respond to emergencies in a timely and professional manner. Notify the staff MD and Urgicare about 3 hour runs and EMS activation. Complete special housing rounds when assigned. Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards. Maintain clinical competency by participating in all CME and CHS training and in-service requirements. Maintain your schedule as directed with particular attention to punctuality and timely notification of absences. Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed. Complete tasks as delegated by a Site Medical Director or other supervising clinical team member. Maintain and provide documentation of all required credentials. Maintain professional attitude and appearance. Adhere to Occupational Health Services requirements. Minimum Qualifications 1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and 2. Current certificate as a Nurse Practitioner issued by the NYSED; and 3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). The Cardiology Inpatient Consult Team Nurse Practitioner will provide cardiology medical care and treatment to patients in the hospital, under direction of physician by ordering, interpreting, and evaluating diagnostic tests to identify and assess patient's clinical problems and health care needs. Correctional Health Services The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Nurse Practitioner will be matched with the population that most closely meets their clinical expertise, desired work location and schedule, and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information. Evaluate patients for the need for a higher level of care, including hospitalization, and escalate care when clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care. Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner. Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication. Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed. Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics. Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist. Provide clinical oversight for psychiatric nurse practitioner trainees, including appropriate direct supervision and documentation of evaluation and feedback. Other duties as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director, or Chief of Service for Psychiatry, or their respective designees. Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients. Minimum Qualifications: 1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and 2. Current certification to practice as a Psychiatric Nurse Practitioner issued by the NYSED; and 3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Department Preferences Strongly prefer Psychiatric Mental Health Nurse Practitioner (NP) certification by the American Nurses Credentialing Center (ANCC). At least 3 years of hospital experience both as RN & NP in Behavioral Health An equivalent and satisfactory combination of education and experience may be considered Additional Preferences Demonstrate advanced knowledge of psychopathology in assessing patients in need of mental health treatment, including mental status examination and diagnosis. Join a team that's breaking down boundaries to deliver proactive, preventative screenings at no cost to the patient. What You Will Do You'll provide annual in-home assessments (IHA) for members of Medicare, Medicaid, and Affordable Care Act plans. These 90-minute visits identify care gaps, health risks, and social determinants of health to help PCPs and health plans promote better outcomes. You will collect patient histories, document patient concerns, and provide health education. You are not responsible for lab tests, blood work, or treatment. You will be asked to: Perform high quality assessments to meet patient needs and health plan expectations. Document and submit chart notes within 24 hours of the visit. Address charting queries within 48 hours of receipt. Attend periodic training sessions and meetings pertinent to the position. In-home assessments help identify the social determinants of health that you can't see from a doctor's office. They also give patients the time they need for meaningful conversations, ensuring their questions are thoroughly addressed. Qualifications Our Ideal Candidate Demonstrates empathy in every patient interaction. Can make critical decisions while following Emcara Health policies and procedures. Has an advanced understanding of risk adjustment models (or is curious to learn). Is proficient with electronic medical records (EMR). Is comfortable entering new homes and engaging with new patients. As an IHA provider, you will be required to travel up to a 50-mile radius within your hired area.You are reimbursed for mileage greater than 30 miles in one day at the current government rate. Member outreach works to cluster your appointments in one geographical area as much as possible. Can sign up for a minimum of 30 appointment slots or more per month. General Physical Demands: Ability to lift 20 lbs. Ability to bend or kneel for extended periods of time if needed while performing assessments. Education and Experience: Master's or Doctorate degree from an accredited NP or PA program. Home health experience (Preferred) 1 year of in-home assessment experience (Preferred) Certification and Licensure: Current RN and NP or PA licensure in the present state of practice. Board certifications accepted: FNP, ANP, AGNP, ACNP, AGACNP, and AGPCNP. Current BLS certification Valid unencumbered state driver's license Valid proof of sufficient car insurance coverage for the state appointments are scheduled to be completed in.
