Our patient population ranges from pneumonia and general medical conditions to minor surgical procedures and complex medical/psychosocial needs. A special focus of our team includes the complex issue of IV drug related infections that require up to 6 weeks of medical care. This special team consists of a Licensed Clinical Social Worker, Peer Recovery Specialists, Addiction Physicians, Psychiatric Technicians, Clinical Nurse Specialists, and many others who are passionate about helping in the recovery of patients who have often lost hope for a future life. Our patients have lovingly named our unit “The Hope Unit”. The LPN (inpatient) practices practical nursing within the parameters set forth in Carilion Clinics Plans for the Provision of Patient Care and functions under the direction of a registered nurse or physician. Partners with the RN in collecting data, planning, implementing and evaluating patient/resident care. The LPN (inpatient) Participates in the nursing process for patients of all ages served. Communicates frequently with team members, ancillary departments and physicians. Contributes to the development of the patient/resident database and interdisciplinary plan of care. Identifies patient/resident/family/caregiver educational needs, incorporates education into plan of care and documents. Applies knowledge of growth and development to appropriately care for patients/residents. Implements prescribed therapeutic treatment plans and procedures. Administers prescribed medication to patients/residents. Advocates for patients, protector of patient's rights, and provider of psychological/social/cultural support. Contributes to Carilion's performance improvement activities and supports service area/facility/organization initiatives. Contributes to the orientation and career development of others. May assign tasks to as needed. What We Require: Education: Graduate of an accredited school of practical nursing. Experience: None Licensure, certification, and/or registration: Licensed to practice practical nursing in the state of Virginia. In Psychiatry and Behavioral Health: de-escalation training must be successfully completed within four months of hire and then maintained going forward. Completes patient assessment to include height, weight, vital signs, chief complaint, current medications, etc. Prepares patient for provider evaluation, including obtaining specimens, positioning for exams, listing medications, assisting during exams and diagnostic and therapeutic procedures. Coordinates tele-retinal and tele-dermatology clinic set up. Administers medications and injections as directed by the provider. Maintains work area, exam, treatment and x-ray rooms and lab in a clean, safe manner; restock with supplies and instruments; sterilize reusable equipment as needed. Documents and maintains patient information in the electronic medical record in a timely manner to include update treatment procedures, medication refills, test results, patient concerns, etc. Contributes to Carilion Clinic's performance improvement activities. Provides patient teaching as indicated and directed by the provider, including lab results, diet instructions, medication instructions, appropriate follow-up, and reinforcing provider instructions as needed. Assists with referral authorizations for specialists and pre-authorizations for testing and ancillary services. Assists with lab procedures, including collecting urine specimens, collecting and preparing blood samples for reference lab, labeling all specimens with accurate patient ID. Observes all safety precautions and standards; assures proper disposal of specimens and contaminated waste. LAB (if applicable) Performs lab procedures as directed by the provider including phlebotomy, peripheral sticks, collecting and preparing urine specimens, throat smears; conduct lab testing onsite after appropriate training, including waived and moderate complexity studies. Prepares equipment to begin testing lab specimens. Runs daily quality control tests as directed by lead clinical/lab staff. Adheres to quality control measures with all lab procedures, performing proficiency testing and conducting control testing and calibration on all equipment. Maintains Q/A Plan. Prepares equipment to begin testing lab specimens. Runs daily quality control test as directed by the clinical/lab staff. RADIOLOGY (if applicable) Performs x-ray procedures as directed by the provider. Takes measures to prevent excessive exposure to patient and self. Completes all documentation and labeling of x-rays. What We Require: Education: Graduate of accredited school of practical nursing required, or new graduate of approved program eligible to sit for the Virginia licensure exam.
