Job Summary: Responsible for integrating patient-centered pharmaceutical care with traditional distributive and drug control activities. Provides consultative/clinical services including antibiotic dosing of vancomycin and aminoglycosides, initial warfarin dosing, renal dosing, and IV-to-PO conversions. Requires strong clinical and communication skills for providing drug information to nurses, physicians, and other health care providers. Capable of advising the patients' medical team on drug interactions, contraindications, and other relevant information to ensure proper, accurate, and optimal medication therapy. Responsible for adhering to and practicing within the guidelines of Georgia State Pharmacy Law. Core Responsibilities and Essential Functions: Knowledge of drug appropriateness - Evaluating and monitoring drug regimens for appropriateness. - Interpreting physicians orders and screening them for various types of interactions. - Providing patient and nursing education when requested. - Reviewing medication profiles to maintain accuracy and optimize medication therapy. - Able to perform all duties of a Pharmaceutical Care Pharmacist General Pharmacy Practice - Preparing IV Admixtures, compounding, and repackaging a wide variety of pharmaceuticals - Entering and verifying orders into the pharmacy computer system, and proper dispensing of medications - Able to follow patients for antibiotic and anticoagulant dosing - Participates in daily rounds - Works with other healthcare providers to assist in the patient education for new medications and their side effects - Assists with medication reconciliation and discharge counseling Education Skills - Providing supervision and guidance to all personnel working in their area - Demonstrating skill and competency in patient assessment, and providing patient and nursing education when requested. Required Minimum Education: B.S. from an accredited School of Pharmacy. Pharm.D. from an accredited college of pharmacy is preferred. Completion of a PGY1 Pharmacy Residency is preferred. Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated. Reg Pharmacist Additional License(s) and Certification(s): BCPS: Board Certified Pharmacotherapy Specialist preferred Upon Hire Required Minimum Experience: Hospital pharmacy experience is preferred. :The Pharmacy Retail Buyer Level I is proficient with wholesaler and Retail ordering systems, understands inventory management and contract compliance. This position is responsible for ensuring standard operating procedures are established and followed for each area and analyzing those purchases. The buyer facilitates changes in Point of Sale system in regards to billing as new pricing becomes available. The buyer will also work under the guidance of a Pharmacist in charge on drug selection to completely meet the drug needs of our patient population. Coordinates with the Ambulatory Director and/or Business Manager based on identified changes in medication availability or significant price changes. The retail buyer holds responsibility for the placement of all medication and supply orders for the retail stores. This position also works closely with the Specialty service line, and assumes responsibility for the 340B medication accumulations and purchases. This role may involve occupational exposure to hazardous drugs. The lead pharmacist also takes on additional responsibilities, including regulatory compliance and clinical initiatives, and may assume the pharmacy leader's duties in their absence. Strong communication, problem-solving, and organizational skills are essential for success in this fast-paced, patient-focused role. This role may involve occupational exposure to hazardous drugs. Training and education will be provided on any specific procedures, risks and safety protocols associated to the handling of these drugs.
