Summary:
+ The Admission RN is responsible for the entry of patients and families into the spectrum of services provided by Penn Medicine Hospice. As a member of the Interdisciplinary Team (IDT), the Admission RN provides direct care to adult patients and demonstrates safe practice within the domains of clinical judgment and decision-making, nurse-patient/family relationship, clinical leadership, and clinical scholarship. The Admission RN works closely with others on the healthcare team to facilitate the coordination of care across the continuum. The individual reviews documentation of potential clients who are referred to hospice, arranges for initial contact in a timely fashion and communicates plans for contact with client and family and referral sources. In addition, the Admission RN provides information to potential hospice patients, assists with the decision making process, performs assessments that assure the appropriate level of care is delivered, and expedites entry into service delivery systems. The Admission RN nurse must attend to the values and rituals important to the patient-family-community environments as well as upholding nursing practice and safety standards. The Admission RN instructs the primary caregiver of immediate care needed to be provided and informs primary nurse of the initiation of the plan of care. In addition, the Admission RN is responsible for practicing within the Hospice regulations and reimbursement criteria. As such, the nurse is responsible for determining the skilled nursing care and visit pattern to maintain quality and meet the patient and family's goals. The Admission RN will have a working knowledge and understanding of hospice, the interdisciplinary team concept and their individual role. The Admission RN functions within the interdisciplinary team to facilitate communication to promote and support symptom and disease management.

The Observation Unit is a fast-paced 52-bed unit for both medical and surgical patients needing short-term care. As an extension of the ED, patients here have an anticipated discharge of about 24-48 hours; enough time to receive treatments and procedures before being discharged. Top diagnoses on the unit include chest pain, Afib, hypertension, PE/DVT, abdominal pain, trauma, TI/CVA, CHF, asthma/COPD, vertigo, AFR/electrolyte abnormalities, syncope, hypoglycemia/hyperglycemia, and pre/post procedure (cardiac cath, OR, pacer, ablation, etc.). This unit requires strong assessment and critical thinking skills, as patients condition can change rapidly.

The Penn Medicine Recruitment team is seeking full-time, RegisteredNurse for the Bed Management/ Transfer Center at The Hospital of the University of Pennsylvania.

The UPHS Bed Management/Transfer Center RN provides overall direction, guidance, and coordination to health care providers in the achievement of timely/safe patient placements and movement. The RN is responsible for appropriate admission and placement of all patients into HUP, by evaluating appropriateness of admission and level of care for all patients and using clinical knowledge to assign the most appropriate bed to each patient. The Bed Management/Transfer Center RN prioritizes all bed assignments by using clinical expertise and assigns Medical teams/Residents for all Medicine admissions. In the absence of the Pre-Cert RN, the Bed Management/Transfer Center Revaluates appropriateness of admission and level of care for all patients and can require a buy-back agreement for patients transferring from outside facilities. Additionally, the RN is responsible for facilitating inter-facility transfers through the Penn Medicine Transfer Center. The RN maintains liaisons with referring facilities, and is familiar with the UPHS clinical and strategic initiatives to triage patients to the appropriate facility.
Minimum Requirements:
-Must have a Bachelor of Science Nursing Degree and be licensed as a RegisteredNurse in the State of Pennsylvania
-Minimum of 5 years' clinical experience
-Critical Care/ED Experience preferred
-Ability to collaborate and communicate effectively with all members of care team is critical
