Position Summary
The certified PhysicianAssistant is a critical part of our team of committed, self-motivated people who provide critical services to promote and maintain our service, care, and our values.
The role of the Proliance certified PhysicianAssistant is well-rounded, ideal for a self-motivated person who wants to maximize their skill set and expand their clinical knowledge. In this role, you will act as first assistant in the ASC operating room and provide clinic-based care and post-operative care; and may provide surgical assistance in the hospital OR, and/or perform rounding on patients within the hospital.
Key Duties and Responsibilities
The key duties and responsibilities of the PhysicianAssistant include, but are not limited to:
Greeting/ identifying patient & completing and documenting an accurate history and physical examination of patients; may include moving and positioning patients
Conducting comprehensive or episodic health assessments and developing patient care plans
Ordering and interpreting tests, treatments, and therapies, within the scope of practice
Prescribing medications, diet, supplies, prosthetic devices, and other home health aides according to clinical privileges
Administer and/or inject medications
Wound care; apply or remove casting, splinting, wound dressing; suture and staple removal, etc.
Instruct and counsel patients regarding the total treatment plan & therapeutic regimes
Providing clear, concise, and accurate information regarding patient care in the medical record
Obtain/maintain technical familiarity and proficiency for specific surgical procedures prior to their performance
Remain proficient in the utilization of commonly used clinical protocols and guidelines
Assist in Surgical Procedures
Hospital rounds, including patient care, charting & all paperwork/documentation, and interacting with the patient and family/care givers, staff, consultants, and discharge planners.
Primary or secondary trauma call coverage
Emergency Room: includes evaluating and treatment of patients
Answering service coverage and responsibilities
The ability to initiate Basic/Advanced life support in response to emergency situations.
Provides education to patients, families, and significant others using established education materials
May provide other clinical duties within the scope of practice as directed under a patient's care plan
Advocates for patients, families, co-workers, the community, and the health care profession
Integrates evidence-based knowledge and clinical expertise in decision-making
Seeks experiences and formal and independent learning activities to maintain and develop clinical and professional skills and knowledge
Establishes an effective relationship with the supervising physician
Complete duties and assist others as directed

Education/Experience
Graduate of accredited Physician's Assistant Program
Washington State PhysicianAssistant License, Current
Current Certification as a physicianassistant by the National Commission of PhysicianAssistants (NCCPA)
Current CPR and BLS from the American Heart Association
Two years prior experience in surgery preferred

This is a hospital-based position that works cooperatively with the medical, surgical, emergency and social work services to address the complex medical and behavioral health needs of patients receiving care at Virginia Mason Medical Center. This includes providing care for people experiencing delirium, and mood, psychotic, anxiety, substance use, and somatic symptom disorders, among others. The Consultation-Liaison Psychiatry service also works closely with patients in the hospital under the Involuntary Treatment Act with Single Bed Certification. The Consultation-Liaison APP will have an important role in the education of clinical teams about behavioral health care, including multiple residency programs. The collaborative nature of this position, and partnership with physician and advanced practitioners, medical and surgical trainees, nurses, social work and other health professionals in a team-based environment is particularly rewarding.
Schedule/Call
The consult psychiatry team works weekdays taking consults from 8-5.
There are currently no overnight or weekend responsibilities for team members as this is currently covered by an outside tele psychiatry service.

The Chief Advanced Practice Provider (APP) will work as a first or second assistant during open-heart and other vascular and thoracic surgical procedures under the direction of the attending physician. This provider will assist with the preoperative, intraoperative, and postoperative care of cardiac surgery patients within the operating room and serve as Chief APP for the Cardiac Surgery First Assist APP and Cardiac Surgical Assistant team.
Primary Duties and Responsibilities
Clinical Responsibilities:
Review and interpret patient charts, medical histories, and clinical imaging as part of preoperative planning.
Recognize and respond appropriately to abnormalities on electrocardiograms in the OR and ICU settings, assisting in the stabilization of arrhythmias and hemodynamic parameters.
Assist in the cardiac surgery operating room, including but not limited to:
Performing midline sternotomy.
Preparing patients for arterial and venous cannulation.
First-assisting on cases including coronary artery bypass grafting, valve repair/replacement (open and robotic), aortic arch repair, mechanical circulatory support, and heart transplantation.
Assisting in the percutaneous insertion of devices.
Harvesting and preparing conduits for CABG, including internal mammary artery, greater saphenous vein, and radial artery.
Placing epicardial pacing wires and mediastinal/pleural drains.
Closing the sternum.
Teaching and mentoring less experienced staff.
Leadership Responsibilities (Chief APP):
Supervise and coordinate work activities of APPs within the cardiac surgery service line to ensure efficient, high-quality patient care.
Oversee clinical, teaching, and research activities within the team.
Develop, implement, and monitor quality assurance and quality control programs; ensure clinical competencies are met.
Participate in recruitment, hiring, training, performance management, and?when necessary?disciplinary actions in coordination with the department?s leadership.
Ensure all APPs maintain appropriate licensure and certification; monitor and implement updates in regulatory requirements.
Organize onboarding, orientation, and continuing medical education programs for APP staff.
Stay current with advancements in cardiac surgery and patient care, communicating relevant updates to the team.
Coordinate preceptorships for students in accredited NP/RNFA or PA programs.
Facilitate process improvements and organizational changes to align with service line and system goals.
Maintain an active clinical practice as defined above.
Minimum Qualifications
Education:
Graduation from an accredited PhysicianAssistant program or Acute Care Nurse Practitioner program.
RN First Assistant certification required for Nurse Practitioners.
Experience:
Minimum of 5 years of cardiac surgery care experience preferred.
At least 2 years of operating room experience required.
2 years of leadership or supervisory experience preferred.
Licenses and Certifications:

Pay and Benefit Expectations
We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. The Opportunity
The Nurse Practitioner is primarily responsible for providing medical care within Sound Family Medicine locations. In this role, you will be responsible for exercising considerable judgment in applying professional knowledge in managing patient problems within established policies and practices. Unlike employed practices, these are governed by you and your colleagues. You will contribute to the success of the practice by providing prompt, efficient, and friendly patient service via in person, electronically or over the phone as needed.

Assess patient health status including state of wellness, adherence to care plan, and determine appropriate diagnostic and therapeutic procedures.

JOB OVERVIEW:
This role will provide direct clinical care within the DESC Opioid Recovery and Care Access (ORCA) Center. The ORCA Center is open 24 hours per day, 7 days per week, including holidays. It will offer four overlapping types of services, delivered by a team of prescribers, nurses, medical assistants and peer specialists, with operational leadership support:
Care to individuals in stable condition who have experienced an opioid overdose, brought to the ORCA Center by first responders or transferred from local emergency departments.
Rapid initiation of medications for opioid use disorder (MOUD), including buprenorphine (sublingual and long-acting injectable) and methadone (under the ?72-hour rule?, or Code of Federal regulations Title 21 ยง1306.07 (b)), for people with opioid use disorder (OUD), regardless of whether an opioid overdose has recently occurred.
Harm reduction counseling and OUD-related physical and behavioral health services, such as hepatitis C testing and treatment and HIV pre-exposure prophylaxis.
Follow-up care, at the ORCA Center or at times field-based, with individuals who have started MOUD to ensure support in MOUD continuation.
This is a clinical role without administrative or supervisory duties.
About DESC:
DESC is a multi-service agency that specializes in providing permanent supportive housing, emergency shelter, residential crisis services, and comprehensive behavioral health outpatient services for people with long-time experiences of homelessness and serious behavioral health disabilities. We utilize harm reduction, trauma informed care, and housing first practices, striving to deliver high-quality, outcomes-oriented, flexible services that accommodate consumers? unique needs. The ORCA Center builds upon DESC?s successful and innovative clinic- and field-based outpatient OUD treatment team and will collaborate closely with this team.
PRIMARY RESPONSIBILITIES:
Clinical Care
Provide evidence-based treatment for OUD, including initiation and management of buprenorphine (sublingual and long-acting injectable) and, according to DESC protocols, methadone initiation.
Conduct harm reduction counseling and provide services related to infectious diseases such as syphilis, hepatitis C, and HIV (including PrEP).
Manage and treat conditions commonly seen in individuals with OUD, such as skin and soft tissue infections and linkage to mental health care.
Perform initial health assessments and follow-up visits for individuals starting or continuing MOUD.
Educate patients on health-related topics, treatment plans, and harm reduction strategies.
Document clinical care in accordance with DESC and regulatory standards.
Care Coordination
Collaborate with an interdisciplinary team of clinicians, peer specialists, and social workers to provide holistic care.
Ensure continuity of care for individuals transitioning between ORCA Center services and other health care providers.
Professional Development
Stay updated on best practices in addiction medicine and harm reduction through continuing education.
Participate in in-service training and case discussions to maintain high clinical standards.
Requirements:
MINIMUM QUALIFICATIONS:
Licensed as a Nurse Practitioner (Family or Adult NP) or PhysicianAssistant in the State of Washington.
Registration with the Drug Enforcement Agency.
Experience or training in addiction medicine and harm reduction.
Strong commitment to serving individuals with complex needs, including homelessness and co-occurring behavioral health conditions.
Familiarity with trauma-informed care, harm reduction principles, and equity frameworks.
Ability to remain open and receptive to constructive feedback.
Strong ability to approach those with differing opinions with curiosity & active listening.
Ability to work collaboratively in a multidisciplinary team environment.
PREFERRED QUALIFICATIONS:
Experience in low-barrier buprenorphine programs or Federal Opioid Treatment Programs.
Experience in an acute care setting (e.g., emergency department or overdose prevention center).

Key Duties and Responsibilities
The key duties and responsibilities of the PhysicianAssistant include, but are not limited to:
Greeting/ identifying patient & completing and documenting an accurate history and physical examination of patients; may include moving and positioning patients
Conducting comprehensive or episodic health assessments and developing patient care plans
Ordering and interpreting tests, treatments, and therapies, within the scope of practice
Prescribing medications, diet, supplies, prosthetic devices, and other home health aides according to clinical privileges
Administer and/or inject medications
Wound care; apply or remove casting, splinting, wound dressing; suture and staple removal, etc.
Instruct and counsel patients regarding the total treatment plan & therapeutic regimes
Providing clear, concise, and accurate information regarding patient care in the medical record
Obtain/maintain technical familiarity and proficiency for specific surgical procedures prior to their performance
Remain proficient in the utilization of commonly used clinical protocols and guidelines
Assist in Surgical Procedures
Hospital rounds, including patient care, charting & all paperwork/documentation, and interacting with the patient and family/care givers, staff, consultants, and discharge planners.
OPTIONAL Primary or secondary trauma call coverage
OPTIONAL Emergency Room: includes evaluating and treatment of patients

The ability to initiate Basic/Advanced life support in response to emergency situations.

Education/Experience
Graduate of accredited Physician's Assistant Program
Washington State PhysicianAssistant License, Current
Current Certification as a physicianassistant by the National Commission of PhysicianAssistants (NCCPA)
Current CPR and BLS from the American Heart Association
Two years prior experience in surgery preferred
