Department: WAC Fitness Weekly Hours: 0 Status: Per Diem Shift: Variable (United States of America) As a Registered Dietician, you provide personalized nutrition counseling to ProMedica ForeverFit members through assessing dietary needs based on fitness goals.
You will create individual meal plans and provide education on healthy eating habits. You work collaborate with trainers to optimize your client's overall health and performance.
In this role, you provide group nutrition care in areas such as oncology and diabetes based on provider referrals and provide content or recipes for monthly newsletters.

REQUIREMENTS
* Requires a minimum of a bachelor's degree in nutrition and dietetics or related field and completion of a dietetic internship.
* Registered Dietitian (RD) status and/or successful completion of the national registration examination (RD) within 120 days of hire.
* License: Ohio dietitians must be licensed with the State Medical Board of Ohio(LD) and/or obtain licensure within 120 days of hire.
* Certification: Registered Dietitian with the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics and/or be eligible to successfully complete the examination within 120 days of hire.

Summary:

Responsibilities And Duties:
Performs nutrition assessment using age-appropriate standards, utilizing validated assessment tools and nutrition focused physical assessments. Includes documentation of nutrition related diagnosis when indicated.
Demonstrate proficiency in accordance with Standards of Practice and Professional Performance for Registered Dietitian Nutritionist while providing age-appropriate plan of care for patients and families.
Coordinates nutrition therapy with physicians and other health care professionals. Participates in multidisciplinary rounds and care planning sessions.
Plans, implements, and adapts nutritional care plan to meet physician orders, age, and disease specific physiologic needs. Intervention plan includes appropriate feeding route, mode, and nutrition prescription.
Teach cognitive behavioral therapy techniques to promote positive lifestyle changes.
Serves as an active member on multidisciplinary teams.
Monitors and evaluates patient response to medical nutrition therapy using specific goals and determines change if indicated.
Performs patient, family, and care giver education related to nutritional therapy and incorporates best method of learning style and communication into sessions.
Supports the development and revision of nutrition related department tools, training materials, policies, and procedures. Remains current on research developments and other methods of practice and incorporates this into current programs and services.
Participates in the evaluation of clinical nutrition products and equipment.
Conducts telemedicine consultation sessions with clients to assess and analyze diet and disease prevention strategies.
Educates and trains interdisciplinary team members, students, and interns.
Fosters teamwork and establishes effective strategies to implement innovative programs which leads to optimal clinical nutrition and financial outcomes. Supports quality improvement, research, and outcome studies. Identifies and communicates opportunities for waste reduction and service improvements. Documents accurately and completely, including appropriate utilization of order writing privileges in electronic medical record. Complies with all regulatory standards, including federal, state, and accrediting agencies and adhere to facility confidentiality, HIPAA regulations, and patient rights policies. Participates in and attend departmental meetings, staff development, and professional programs, as appropriate. Ensures national program standards are met and always maintained for certified and/or recognized programs, as applicable. Other duties as assigned.

Provides nutritional services for the facility's in-center and home patients as applicable in order to maximize the patient's nutritional status and improve clinical outcomes. PRINCIPAL DUTIES AND RESPONSIBILITIES:
Assesses patient's knowledge of diet and kidney disease and provides education appropriate to patient's learning needs regarding the relationship of diet and kidney disease and the impact on the treatment process.
Provides patient specific detailed education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process.
Completes nutritional assessment and participates in the interdisciplinary comprehensive assessments.
Participates with the interdisciplinary team in the development of a written, individualized comprehensive plan of care that specifies the services necessary to address the patient's needs, in accordance with established Federal and, where applicable, state licensure guidelines.
Calculates diet prescription according to standard of practice.
Completes progress notes on all chronic dialysis patients according to established FMCNA guidelines. Maintains complete and accurate documentation of all appropriate information in medical record according to established FMCNA policies and Federal and state regulations.
Provides patient with ongoing nutrition assessment and outcome-oriented nutrition counseling necessary to assist patient in achieving and sustaining an effective nutritional status.
Identifies malnourished patients as well as patients at risk for malnutrition and works collaboratively with interdisciplinary team to identify appropriate interventions, resources or solutions. Reviews each patient's albumin level and body weight at least monthly. Implements plan of care with involvement of patient/family/caregiver.
Monitors adherence and response to nutrition therapy, adjusts therapy as needed to achieve and sustain an effective nutritional status, documents why the patient was unable to achieve the goals and implements the plan of care to address issues impacting the patient's ability to achieve the goals.

Collaborates with the interdisciplinary team on management and recommendations for mineral metabolism and renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status and sodium/volume control.
Adheres to FMCNA algorithm and medical record policies.
Reports on nutrition QAI results and participates in the interdisciplinary QAI program.
Collaborates and coordinates with team members in provision of Pre-ESRD education as appropriate.
When appropriate, works with Case Manager to coordinate plan of care for patient education and identifies candidates to refer to physician for nutritional supplements, as per disease management agreements.
Reviews nutrition related lab results:
Counsels patient and/or caregiver and formulates appropriate action.
Recommends treatment changes to the interdisciplinary team as appropriate.
Communicates with physician and/or facility staff regarding lab results as appropriate.
Communicates lab results and recommended treatment changes to providers of patient care in nursing facilities (i.e., hospitals, short term and long-term care facilities).

JOB RESPONSIBILITIES:
As part of the sales team, promote the enteral feeding program and maximize enteral sales in an assigned territory (including hospitals and outpatient doctor offices).
As a subject matter expert, provide recommendations, develop, test and implement nutrition education tools to be used in the field
Work with Sales and Marketing teams to increase Enteral business
Conduct enteral feeding pump education.
Review clinical documentation to determine if patients qualify for Medicare coverage.
Work on outreach efforts to increase community knowledge of the SHC Enteral program
Participate in community events and health fairs
QUALIFICATIONS:
Current Registered Dietitian licensure
Minimum 2 years' experience as a Registered Dietitian
Excellent communication and interpersonal skills
Ability to travel within your assigned territory, and to company meetings.
Valid Drivers License
Ability to sell a service and promote a product line
Experience with enteral patients, pediatric enteral experience preferred
Bilingual English/Spanish preferred

Summary: Responsible for the assessment and planning of the nutritional care of the residents in accordance with applicable federal, state, and local standards, guidelines and regulations, policies and procedures, and in coordination with the Food Service Director/Manager to ensure that qualify food service and nutritional care is provided to the residents.
Essential Functions:
Maintains clinical nutrition documentation for residents in accordance with Nursing Center policy and procedure, as well as state and federal regulations.
Ensures that all assessments and progress notes documented in the medical records are accurate, informative, and descriptive of the services provided and of the residents response to the plan of care.
Ensures all documentation is completed within the required timeframes and in accordance with Nursing Center policy and procedure, as well as state and federal regulations.
Participates with the interdisciplinary team in developing, implementing, and evaluating plans of care; attends meetings/conferences to discuss the clinical care of the resident; and communicates resident concerns and response to interventions to interdisciplinary team members and direct care staff.
Interviews residents, family members, legal representatives, significant others, and staff to obtain and update information needed to develop individualized plan of care, protect and promote residents rights, and accommodate individual needs and preferences.
Involves the resident/family in planning objectives and goals for the resident.
Ensures a dietary kardex or similar record of diet orders and food preferences is utilized in the Food Service Director.
Coordinates with the Food Service Director/Manager the review and customization of the regular and therapeutic menus to meet the food preferences of the residents in accordance with established policies, procedures, Diet Manual guidelines, standard of practice, and regulations.
Coordinates with the Food Service Director/Manager to ensure that all diets ordered by the physician are planned on the therapeutic menu.
Routinely obtains feedback and suggestions from residents via resident council, meal rounds, and/or interviews with residents, family, and staff to improve the menu.
Provides feedback and suggestions to improve the menu to the Menu Development Coordinator at the corporate office.
Monitors food temperatures and quality at point of service in the various dining locations via test trays per company procedures and standards.
Monitors the distribution of nourishments, supplements, and meals to residents and takes corrective action when necessary.
Assists the Food Service Director/Manager with processing diet orders and changes and ensuring that an accurate tray card is available for each resident receiving meals.
Ensures that new residents are visited upon admission to obtain a Diet History and food preferences, on the company approved form.
Conducts meal rounds, test trays, and interviews staff and residents to ensure that residents are receiving foods in the amount, type, consistency, and frequency required to maintain or improve nutritional status, results of findings reported to Nutrition services department and administration.
Routinely inspects the food service area(s) and practices for compliance with company policies, procedures, standards, and applicable federal, state, and local regulations. Completes evaluations and develops action plans with the Food Service Director/Manager and the Administrator for items that need improvement results of findings reported to Nutrition services department and administration.
Participates in the Nursing Center's "Performance Improvement Program." Serves on various committees as required by company standards, regulations, and/or appointment by the Administrator.
Participates in the long-term care survey process. Instructs staff in matters of conduct and disclosure.
Maintains a presence at the Nursing Center as directed by the Administrator while surveyors are on-site and directs the timely collection of information required by the survey team. Undertakes corrective action while survey is in progress, if appropriate. Works with the Food Service Director/Manager and other Nursing
Center departments to develop survey plan of correction, as needed.
Maintains good communications and working relationships with all departments in the Nursing Center.
Routinely communicates issues, concerns, and opportunities for improvement to the Administrator.
Conducts an entrance and exit conference with the Administrator or designee at each visit if employee is assigned to multiple Nursing Centers.
Reports all hazardous conditions/equipment to the Administrator immediately.
Assists the Food Service Director/Manager with purchasing decisions for food, supplies, equipment, etc. within approved budgets.
Assists the Food Service Director/Manager with training Nutrition Services employees.
Provides in-service training to Nursing Center staff on topics related to Nutrition and Foodservice.
