Department: ITS Provider Opex Weekly Hours: 40 Status: Full time Shift: Days (United States of America) The core role of the CT I is to become credentialed in one Epic application, deliver training to end users in a classroom and online setting, provide go live support for a defined period of time, optimize end user proficiency and conduct consistent rounding. This requires onsite rounding and providing 'help at the elbow' to end users for a period of time for extended shifts. Travel to hospital locations or physician practices is likely. All level advancement is at the discretion of the Training Management team.
ACCOUNTABILITIES

* Under the direction of a principal trainer(s) or others, lead classes, workshops, computer-based training, and online lessons in alignment with established application curriculum.
* Support the mission of ProMedica through professional classroom demeanor and high-quality teaching standards.
* Using ProMedica's learning management system software, maintain class rosters and grade classes correctly and promptly.
* Analyze and test application functionality; identify and resolve issues within scope of authority and notify appropriate individuals.
* Work with Principal Trainers to provide feedback on additional end user learning needs, remediation of identified issues and continuous quality improvement activities.
* Responsible for mastering one Epic application and the specific workflows of that application.
* Perform routine facility rounding and provide onsite/virtual assistance to end users as assigned.
* Other duties as assigned.
REQUIRED QUALIFICATIONS
Education: Associate Degree or equivalent relevant experience
Skills: Strong computer skills and a technical aptitude are necessary. Must have strong written and verbal communication skills. Strong organizational and time-management skills. Must be able to understand directions, communicate and respond to inquiries; requires effective self-motivation and interpersonal skills. Must demonstrate a dynamic presentation style, professionalism, and trouble-shooting capabilities. Display a personal passion for educating others; create an engaging learning environment and enhance the end user learning experience through creativity, enthusiasm, and high energy.

WHAT WE'RE LOOKING FOR:
The MDS Coordinator (LPN, RN) is responsible for overseeing the resident assessment and care planning process and ensuring compliance with federal and state regulations related to resident assessments, quality of care and Medicare/Medicaid reimbursement.
Key Responsibilities
Conduct and complete the Minimum Data Set (MDS) assessment to evaluate residents' physical, psychological and functional status, including the implementation of Care Area Assessments (CAA)s and triggers.
Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
Prepare and electronically transmit timely reports to the national Medicare and Medicaid databases.
Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available to the residents.
Ensure that MDS notes are informative and descriptive of the services provided and of the residents' response to the service.
Assist with completing the care plan portion of the residents' discharge plan. Evaluate and implement recommendations from established committees as they pertain to the assessment and/or care plan functions of the health campus.
Qualifications
Must have and maintain a current, valid state LPN or RN license
Three (3) to five (5) years' experience working in the MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment, preferred
Current, valid CPR certification required

Primary Function of Position
The FES Program Manager, reporting to the Sr Director of Finance Organization Development, is responsible for managing several programs and projects. This individual will work closely with the FES Organization Development team, members of the FES organization, and internal and external stakeholders to advance key initiatives for the Learning Academy, Leadership Academy, Finance Development Program (FDP), and Intern Program. This role will require strong communication skills, excellent attention to detail, and a tenacious drive to keep programs on track.
Essential Job Duties
* Partner with the team to define key objectives, milestone activities, and success metrics then develop a program management plan that ensures timely execution
* Support team in maintaining key resources such as the (intranet) team Leo pages, team calendar, dashboards, quarterly readouts and marketing collateral.
* Lead logistics and registration for the Learning and Leadership Academy programs as well as assisting in facilitating team trainings and larger programs
* Partner with the Sr Manager Finance Organization Development, and the University Hiring Program to lead the recruiting strategy for the FDP and Intern programs
* Manage the recruiting budget and identify creative strategies to attract top talent and a diverse candidate pool
* Ownership of the Intern Program including recruitment, hiring, onboarding, training, and program logistics

Facilitate group lessons, activities and community service with middle school and high school youth
Follow curriculum lessons provided and keep fidelity to funders
Travel to school location and field trips
Utilize positive youth development
Recruitment and retention of club youth
Data Entry and reporting using required databases
Collaborate with schools and community partners
Requirements:
Must be 21+
Valid Driver's license with clean driving history
Must be able to pass background checks
High school diploma (some college preferred)
Reliable transportation

Facilitate Youth Leadership Development (YLD) after-school program to build social-emotional development and increase leadership skills and connections to schools and communities for youth (ages 11-18) in need of positive supports.
