Job Description
Deliverables/Principal Results Expected:
Properly instructs and guides chemical dependency technicians and LPN's in client care responsibilities and assignments.
Orientation of new employees to unit specific policies and procedures.
Offer positive constructive feedback and direction to students/interns on the unit.
Conveys accurate information in supportive and courteous manner to unit nursing and tech staff.
Seeks out supervision and direction from physician.
Completion of nursing assessments, transfers, discharges, treatment plans, progress notes, medication reconciliation, medication administration, and vitals.
Knowledge of detox protocols.
Assist medical doctor as needed, provide medical lectures.
Report any unusual occurrences to nurse supervisor/program director promptly.
Ensure room assignments are appropriate to clients' needs.
Attend and actively participate in departmental staff meetings.
Maintain medical equipment in proper working order.
Maintain confidentiality and proper boundaries.
Other duties as assigned.
Technical Competencies
: Strong background in D&A and Co-occurring Disorders, knowledge of HIPAA laws, Confidentiality, state licensing regulations, CPR, AED and First Aid, other trainings as required. Able to effectively meet the needs of the client, positive relationships with internal and external customers and contributing to the success of the corporation.
Education, Experience:
Current RN license in state of practice. Knowledge of continuous improvement techniques. Awareness and understanding of regulating bodies (CARF, Office of Mental Health, DDAP, RTFA). The RN Intern II assists with patient care specific to the age of the patient population served. The RNI-II provides quality care in an efficient and safe manner, consistent with departmental standards of care. The RNI-II demonstrates performance consistent with the mission, philosophy, and goals of the assigned unit and the organization. He/she remains flexible in adapting to changing systems and is expected to demonstrate quality and effectiveness in work habits and clinical practices, treating staff, physicians, patients, and families with consideration and respect.

The ideal candidate will have cardiac experience and be comfortable reading telemetry independently.
The unit prepares and recovers patients for procedures in the Cath lab, EP lab, and Minor Procedure Room as well as the Cardiac OR for surgery.
The types of procedures that we prepare and recover patients for include but is not limited to: Cardiac Catheterization (interventional and diagnostic), Electrophysiology procedures such as cardiac ablations, Cardioversions, surgery, and ICDs/Pacemakers etc.
Education:
• Bachelor of Science Nursing- BSN Required, or MSN

What you will do:
Coordinate care with post-transplant patients.
Facilitates clinic flow, transplant evaluation, and patient care management.
Provide weekly education sessions to patients and support member. Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.
Transplant Coordinator admits patient, provides support to patient/caregiver during the waiting period, and discharges transplant patient with instructions for going home.
Transplant Coordinator will manage a case load of patients, remain patient's point of contact through the transplant process, recovery and at home.
Registered Nurse, BSN should have 2+ years of acute care experience, good medical - renal knowledge, and understands chronic disease. BLS required. Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.
Outpatient clinic is open Monday - Friday between 8am and 4:30pm. Transplant Coordinator will have some scheduled call and have weekend call about every 11th week.

Sentara Wound Healing Center, provides state-of-the art treatment and comprehensive wound care to patients suffering from non-healing or chronic wounds. Our physician-directed treatment includes use of an innovative debridement device, as well as on-site Hyperbaric Oxygen therapy (HBO) when indicated.
Our specialized staff works closely with referring physicians to develop an individualized treatment plan to provide the most optimal clinical outcome for each patient. Ongoing communication with referring physicians ensures your doctor is aware of your treatment progress.
Our team uses a multidisciplinary care approach by working with and referring the patient to specialists from different areas of expertise such as vascular surgery, plastic surgery, orthopedic surgery, podiatry, infectious disease, endocrinology, pain management, and physical therapy.
Additionally, clinical nutrition counseling and diabetes education is offered to patients and family members. We are also happy to assist with Durable Medical Equipment (DME) needs as part of our continuum of care.
