We are seeking a compassionate and clinically strong Primary Care NursePractitioner to provide comprehensive medical care to residents in skilled nursing and long-term care facilities. This role focuses on primary, preventive, and chronic care management for medically complex patients in a collaborative, interdisciplinary environment.
Responsibilities
Provide primary and preventive care to long-term care residents in skilled nursing facilities
Evaluate and manage residents experiencing acute changes in condition
Perform advanced care planning and chronic care management
Prescribe medications, including controlled substances, in accordance with Ohio law
Order, review, and follow up on diagnostic tests and treatment plans
Communicate regularly with nursing home staff regarding patient status and care plans
Collaborate closely with the primary care physician and interdisciplinary care team
Ensure accurate and timely documentation within the EMR
Required Qualifications
Current, unrestricted NursePractitioner license in the state of Ohio
Board Certification (Adult, Family, or Geriatric NP)
Active DEA license
Clinical background in adult, family, or geriatric medicine
Prior experience working in a long-term care or skilled nursing setting
Proficiency with electronic medical records (EMR)
Ability to manage patients with advanced illness and multiple chronic conditions
Strong teamwork skills within an interdisciplinary care model
Preferred Qualifications
Strong clinical reasoning and problem-solving skills
Ability to work independently while recognizing when escalation is needed
High level of risk assessment and patient safety awareness

Position Overview:
We are seeking experienced NursePractitioners and Physician Assistants to join a fast-paced urgent care environment. Key Responsibilities:
Perform physical exams, assessments, and preventive health services
Diagnose, treat, and manage acute and chronic conditions

Triage patients and manage patient flow and throughput

Strong clinical judgment and ability to multitask under pressure
Proficiency with EMR systems and computer documentation
Skilled in IV access, blood draws, and medication administration
Schedule Requirements
Full-Time: 3 shifts/week (30+ hours) - Must work one weekend day (Saturday or Sunday) each week
Part-Time: 1-2 shifts/week - Must work Friday, Saturday, or Sunday

Identify opportunities for preventive care, health promotion, home safety, and illness prevention.
Complete all documentation within 24 hours from visit.
Communicate urgent medical needs (including all prescription needs) with the member's Primary Care Physician. CCS NursePractitioners are not permitted to prescribe medication per CCS policy.
Document clinical escalations, i.e., urgent medical or non-urgent care management needs, in accordance CCS policy.

Perform, document, and communicate results of in-home Point of Care (POC) Testing when directed.
