Key Responsibilities:
Direct overall hospice branch operations, ensuring compliance with state, federal, and accreditation standards (CMS, Medicare, Joint Commission, etc.)
Provide clinical oversight and ensure timely and accurate documentation of hospice services
Interview, hire, supervise, evaluate, and support interdisciplinary hospice staff including RN Clinical Managers, RNs, LPNs, social workers, chaplains, CNAs, and volunteers
Oversee patient admissions and care planning, ensuring a smooth and efficient intake process
Promote a culture of excellence in hospice nursing, end-of-life care, and family support
Conduct performance evaluations, assign workloads fairly, and address employee concerns
Lead continuous quality improvement (CQI) efforts and maintain survey readiness
Monitor financial performance: manage expenses, payroll, supply costs, and budgeting while driving branch revenue growth through census development, referral management, and efficient resource utilization
Maintain strong relationships with physicians, referral sources, and the broader community
About You:
Current RN license in the state of employment (required)
Bachelor's degree in Nursing, Healthcare Administration, or related field (preferred)
3+ years of experience in hospice, home health, or healthcare operations leadership
Strong understanding of hospice regulations, compliance, and Medicare conditions of participation
Proven ability to manage teams, budgets, and interdisciplinary operations
Exceptional communication, organizational, and leadership skills

Summary:
A Registered Professional Dialysis Nurse is responsible for the delivery of safe, high-quality patient care through the processes of assessment, diagnosis, planning, implementation, and evaluation. This role actively participates in the development and implementation of individualized care plans for each patient, providing both direct and indirect care throughout the dialysis treatment process, including pre- and post-treatment assessments and the administration of prescribed medications.

Responsibilities:

Demonstrates appropriate hand hygiene, use of PPE, and facility hygiene, including cleaning and disinfection of work surfaces and equipment-pre-treatment, intra-treatment, and post-treatment.
Operates all available dialysis equipment and supplies based on safety, effectiveness, and cost in accordance with organizational policy to provide expected patient outcomes; documents in the medical record and records change appropriately.
Follows policy and procedure in performance of water treatment procedures and testing; properly demonstrates electrolyte addition to acid concentrate per physician order.
Provides documented pre- and post-dialysis nursing assessments that include but are not limited to: patient weight and fluid removal calculations, lung sounds and presence of edema, vital signs, review of current labs, and assessment and care of the dialysis access.
Obtains pre- and post-dialysis lab specimens per physician order and organizational policy.
Initiates, monitors, and terminates each dialysis treatment per organizational policy and procedure.
Follows organizational policy and procedure in response to dialysis-related complications, emergencies, and technical issues. Nurses are required to obtain approval from leadership identified per organizational policy, for lab culture collection from dialysis catheters via the House Supervisor prior to specimen collection.
Follows organizational policy and procedure in the administration of all medications during dialysis, including as-needed (PRN) medications, blood and blood products, and erythropoiesis-stimulating agents (ESAs).
Ensures patients are transported in a timely manner to and from dialysis treatments to minimize delays in care and maintain treatment schedules. Nurses should transport patients themselves when necessary to prevent delays in care or to ensure timely discharge.
Participates in the development and implementation of an individualized plan of care for each patient.
Verifies the safety of each dialysis treatment by performing and documenting required equipment safety checks, including acid and bicarb expiration dates and chlorine/chloramine testing before every treatment and Minncare residual testing as needed.
Responsible for accurately entering charges for each patient they provide care for, ensuring timely and complete documentation of billable services in accordance with departmental procedures.
Manages time so that workflow is efficient and consistent.

What You Will Do In Your Role:
You will be responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for his/her assigned area of responsibility in alignment with the goals of the Case Management Department
You will be accountable for the overall day-to-day oversight and management of the case management program including coordination, supervision, and administrative oversight of the case management team

What qualifications you will need:
Basic Cardiac Life Support must be obtained within 30 days of employment start date

Patient Care:
Completes initial and ongoing OASIS assessments of patient and family to determine home care needs Or Initial and ongoing comprehensive assessments of the impact of the terminal disease of the patients physical, functional, psychosocial and environmental needs.
Provides a complete physical assessment and history of current and previous illness(es).
Regularly re-evaluates patient nursing needs. Implements/develops/documents the plan of care to ensure quality and continuity of care and recommend revision to the plan, as necessary.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
Uses health assessment data to determine nursing diagnosis.
Develops a plan of care, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventative and rehabilitative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
Counsels the patient and family in meeting nursing and related needs.
If applicable, consult with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs. Prioritize any needs with the members of the IDG/T. Provide training to other staff as needed.
Provides health care instructions to the patient as appropriate per assessments and plan of care. Determine scope and frequency of services needed based on acuity and patient/family needs.
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Communication
Prepares clinical notes and updates primary physician when necessary and at least every sixty (60) days.
Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
Communicates with community health related persons to coordinate the care plan. Inform physicians and other personnel of changes in the patient's needs and outcomes of intervention.
Additional Duties
Participate in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services are available.
Provides clinical directions to the Aides and LPN/LVN to ensure quality and continuity of service provided.
Instructs, supervises, and evaluates HH and/or Hospice aide care provided every fourteen (14) days.
Adhere to all the Company's policies and procedures.

DUTIES AND RESPONSIBILITIES:
* Incumbent's primary duties are to serve as a flight nurse and a program manager.
* Operate as a Medical Aircrew Member and First Responder during ICE removal missions and provide direct patient care for individuals in ICE custody, IHSC clinics, or during DHS missions in accordance with agency procedures, nursing guidelines, and/or clinical protocols.
* Serve as medical aircrew member on a national level, with responsibilities of performing a variety of unusual and complex professional tasks without the benefit of close guidance in remote and austere conditions. Impact of error may cause serious impairment to program, agency, and division and diplomatic relations.
* The incumbent works in small aircrafts, business jets, large airliners, or helicopters, which may be civilian, military and U.S. government local, state, or federal assets. The operations may also include care and transfers on ground vehicles, as well as on sea vessels in inclement weather.
* Assist with providing direct patient care to migrants aboard cutters, small boats, fixed and rotary-wing aircrafts during Alien Migrant Interdiction Operations (AMIO). Ensures official representation of U.S. government health services at sea.

The ISOU Flight Registered Nurse (FN), is a Medical Aircrew Member (MAM), stationed at an ICE facility. The incumbent's duties are divided into primary and secondary responsibilities. Flight nurse and Program Manager duties are primary. Senior staff nurse duties at IHSC facility, as available, are secondary duties. The incumbent participates in domestic and global removal missions through ISOU Medical Air Operations (MedAirOps). MedAirOps provides medical services for individuals in ICE custody during domestic transfers, international removal, and in support of ICE/ERO objectives on land, air, and sea.
The incumbent supports ICE ERO removal missions in charter flights and commercial air carriers. Serves as sole health care provider and first responder while in-flight and overseas. Ensures continuity of care and initiates emergency treatment in the absence of a physician for in-flight medical emergencies. ISOU provides comprehensive training to the incumbent from the following agencies to name a few: Federal Aviation Administration (FAA), Department of State (DoS), Defense Medical Readiness Training Institute (DMRTI), and Federal Emergency Management Administration (FEMA), Department of Interior (DOI). MedAirOps provides guidance and support to ERO/IAO on air transport matters related to migrant care transport, aerospace medicine and flight nursing. Incumbent may be tasked to serves as an RN Responder in other operations including providing direct migrant patient care on ground operations and transport and support of DHS/USCG Migrant Interdiction Operations (MIO) at sea aboard sea vessels. The incumbent works in conjunction with ICE officers interacting with civilian and government organizations, contract staff, U.S. and foreign dignitaries, and domestic and foreign law enforcement agencies to provide continuity of care.
IHSC has a multi-sector, multidisciplinary workforce of more than 1600 employees, including U.S. Public Health Service (PHS) commissioned officers, federal civil servants, and contract staff. The IHSC provides on-site direct patient care to ICE noncitizens at 20 detention facilities throughout the country and manages the provision of off-site medical care for noncitizens housed in approximately 112 additional Intergovernmental Service Agreement (IGSA) facilities.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and tasks may be assigned.
Knowledge of and comply with federal, state, local laws and regulations, including health care professionals practice act requirements as they pertain to home dialysis program and patient care requirements.
Train patient (and/or care partner) in the practice of self-care Peritoneal Dialysis or Hemodialysis upon meeting federal and state regulations governing Registered Nurse qualifications.
Conduct home visits to assess the patient's home environment per policy; initially and minimally annually thereafter. Routinely evaluate patient performance and assess the home environment more frequently as needed to improve care.
Communicate results of patient assessment, reassessment, and ongoing monitoring to the physician, team members, and others as appropriate to the individual needs of the patient.
Document all nursing services in the Electronic Medical Record including but not limited to training sessions, routine and non-routine in-person interactions, and phone conversations. Documentation should accurately reflect the patient status and nursing interventions and be written to ensure continuity of care.
Participate in infection control monitoring, implementation, and recording as requested.
Be familiar with emergency equipment and all emergency operational procedures. Communicate and regularly review Emergency Preparedness procedures with all home patients, including but not limited to emergency disconnection from dialysis equipment, what to do and who to contact if displaced from home.
Ensure clinic and patient supply inventory and usage is appropriate for patient prescriptions and place orders in timely manner to avoid fees and penalties.
Continually seek to improve patient and quality outcomes through tracking and trending of program and patient performance, use of critical thinking skills and root cause analysis to improve patient and quality outcomes. Assist in obtaining data for the continuous quality improvement activities.
Obtain routine and non-routine laboratory tests as ordered and communicate critical lab values and urgent patient needs to the responsible physician or physician extender in a timely manner.
