What You'll Do:
Provides care during patient visits, discussing chief complaint and obtaining vital signs and complete health history for the patient. Develops a baseline physical assessment.
Performs phone triage with patients to determine chief complaint and assessment of next steps and follow up care needed.
Documents health history, assessments, actions and medications in the Electronic Medical Record (EMR) system.

Qualifications:
Associate's Degree in Nursing. Bachelor of Science Degree in Nursing is preferred.

RN license required for the state of practice.
Dual licensure in Washington and Oregon is required within 120 days of hire

Proactively identifies and evaluates patients and families for case management from a variety of sources such as discharge/disposition planning, referrals, the Medical Evaluation Board process, the healthcare system, employers and facility staff.
Conducts systematic, on-going, thorough collection of patient's physical, emotional, psychological, social and medical status. Develops an appropriate patient-specific plan of care to include short- and long term goals, objectives and actions.

Behavioral health services are offered through the Hopi Tribe at Hopi Health Care Center. Dental care is also available. Special ambulatory services are provided on-site by IHS staff based in Phoenix or private contract specialists. Ground and air transport are available for emergency care. The Hopi Service Unit uses the medical home model and open access model to delivery patient services; a multi-disciplinary team approach to patient care; the Service Unit also provides community-public health programs/services, including public health nursing, social services, mental health, substance abuse, health education, environmental health, and nutritional services.

We are seeking a dedicated Patient Care Manager to join our hospice team. Reporting directly to the Executive Director or Administrator, you will oversee patient care activities to ensure the delivery of high-quality, compliant hospice services. This leadership role involves managing clinical operations, coordinating care, and developing staff to uphold the highest standards of patient care and regulatory compliance.
As a Hospice Patient Care Manager, You Will:
Oversee and direct patient care services to ensure the delivery of high-quality hospice care
Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits-making decisions based on clinical judgment and priority
Triage incoming phone referrals and medical concerns in a timely, compassionate, and professional manner
Lead or facilitate Interdisciplinary Group (IDG) meetings, including presenting patient updates, reviewing charts, coordinating care plans, and documenting minutes
Serve as a mentor, trainer, and clinical support to associates, supporting development, performance, and compliance
Ensure all care delivery meets federal, state, and local regulatory standards, as well as company policies and procedures
Support performance improvement initiatives, quality assessments, and operational efficiencies within your location
About You
Qualifications - What You'll Bring:
Current RN license in the state of employment
Graduate of an accredited nursing program
Minimum of 3 years of direct patient care experience, including 2 years in a hospice or home-based care setting
Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC)
Knowledge and compassion for terminally ill patients and their families
Ability to lead a diverse care team while adapting to the emotional and clinical needs of patients from various backgrounds
Strong interpersonal, organizational, and leadership skills
Competency in clinical documentation and technology systems
Preferred Qualifications (Not Required):
Bachelor of Science in Nursing (BSN)
Previous leadership, team management, or supervisory experience in hospice or home health
Familiarity with EMR systems and telehealth platforms
Certification in Hospice and Palliative Nursing (CHPN)
