Qualifications Required:
· A Master's Degree in Psychology, Social Work or related field.
· 3-5 years experience working in a clinical and/or coaching environment within the drug and alcohol treatment and/or mental health treatment industry.
· Knowledge of and fluency in the use of psychological tools, recovery resources, insight into motivation and behavior, understanding of the role of coaching as a change agent.
· Clinical license is a plus but are not mandatory.
· Clinical experience PLUS coaching and leadership experience a significant plus.

Oversees the coordination and execution from patient identification to implant. Case Managers are responsible for empathetic communication, education, and management of implant logistics for patients, ensuring a smooth and efficient path to implantation of ReActiv8 . This individual will take ownership and manage the ReActiv8 patient funnel and provide timely and accurate updates to multiple team members. Develops, implements, and manages a plan of action for each patient in collaboration with the Therapy Manager (TM) and patient. Demonstrates strong documentation skills for each plan of action within a web or cloud-based system.
Regularly collaborates with team and field teams to ensure patient barriers are removed or addressed, and internal and external resources are aligned to facilitate the implant process.
Works with patients and care providers to ensure quick and easy access to surgical scheduling, and implantation of ReActiv8 .
Manages multiple priorities and deadlines simultaneously, while handling a high patient volume.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
