+ Initiates, develops, and implements basic nursing plan of care treatments, evaluating patient progress towards goals. Participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers. Modifies plan of treatment in response to changing patient status based on physician orders to achieve established or revised patient care goals.
+ Assesses patient needs and physical status at each visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order.
+ Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals.
+ Effectively and timely communicates with Naven Health's Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Naven Health's policy.
+ Under close supervision, coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care.
+ Demonstrates compliance with agency operations, Naven Health's policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards.
+ Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule.

Summary: Pine Belt Mental Healthcare Resources has an opening for a Registered Nurse at our Crisis Stabilization Unit (CSU) to provide services for a crisis management center in Gulfport, MS. This position plays a significant role in coordinating medical services and providing nursing needs for adults suffering from serious mental illness.
The ideal candidate will have previous experience in behavioral health as well as a professional and compassionate demeanor. The RN works in conjunction with the mental health care team to provide safe and effective treatment to the consumer during time of crisis in an effort to promote a positive environment in order to return to their community.
CSU is a 24 hour facility open 365 days per year.

The Registered Nurse (RN) delivers quality care consistent within the scope of practice as outlined by the local state nurse practice act for Registered Nurses. The RN works in accordance with local practice acts and regulations, delegates and ensures supervision of nursing activities and functions to other competent nursing personnel appropriate to their scope of practice. The RN assumes responsibility and accountability for the quality of care delivered; works to ensure a safe environment for themselves, the patient and other staff members. The RN acts as a patient advocate to promote the quality of health care delivered in the facility and serve as a leader at all times to promote best practices within the profession of nursing.
REGISTERED NURSE MINIMUM REQUIREMENTS
Graduate of an accredited School of Nursing
Licensure as a Registered Nurse in the state of Mississippi.
Possesses an active CPR certification.
Remains knowledgeable about specific state laws and regulations governing practice.
Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.
REGISTERED NURSE ESSENTIAL FUNCTIONS
Maintain accurate, detailed reports and records.
Administer medications to patients and monitor patients for reactions and side effects.
Record patients' medical information and vital signs.
Monitor, record, and report symptoms or changes in patients' condition.
Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
Modify patient treatment plans as indicated by patients' responses and conditions.
Monitor all aspects of patient care, including diet and physical activity.
Direct or supervise less-skilled nursing or healthcare personnel or supervise a particular unit.
Prepare patients for and assist with examinations or treatments.
Instruct individuals, families, or other groups on topics such as health education, disease prevention, and childbirth as well as develop health improvement programs.
Prepare rooms, sterile instruments, equipment, or supplies and ensure that stock of supplies is maintained.
Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition.

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patients and their families, and a referring agency. Assumes the responsibility for coordination of care.
Job Qualifications:
* Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
* Licensure: Current State license as a Registered Nurse, current Driver's License.

Environmental and Working Conditions:

Essential Functions:
* Completes initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients/clients . Completes assessments at appropriate time points, including Outcome and Assessment Information Set (OASIS) or other assessments as appropriate to the patient.
* Regularly re-evaluates the patient's/client's nursing needs and evaluates the outcomes of care.
* Develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care. Initiates appropriate preventive and rehabilitative nursing procedures. Refers to other services as needed. Plans for the discharge of the patient/client from services.
* Furnishes those services requiring substantial and specialized nursing skill. Counsels the patient/client and their family in meeting nursing and related needs. Uses infection control measures that protect both the staff and the patient (OSHA).
* Coordinates services. Informs the physician and other personnel of changes in the patient's/client's condition and needs.
* Monitors assigned cases to ensure compliance with requirements of third party payors.
* Prepares clinical and progress notes. Completes appropriate documentation in a timely manner.
* Demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency.
* Supervises, teaches and provides clinical direction to other nursing personnel. Assigns home health aides to specific patients. Supervises LPNs/LVNs and paraprofessionals providing services to patients/clients. May only conduct aide competency evaluations if qualified with two years of clinical experience and one year of home health experience.
* Promotes the Agency 's philosophy and administrative policies.
* Performs on-call responsibilities and provides on-call services to patients/clients and their families as assigned.
* Provides effective communication to patients/clients, their family members, team members, and other health care professionals.
