SUPERVISION RECEIVED

POSITION SUMMARY
The role of the Dual Diagnosis Counselor includes service delivery for adult residential clients. The Dual Diagnosis Counselor will work under the direction of the Residential Programs Manager and collaborate with a multidisciplinary team. The Dual Diagnosis Counselor has the skills to complete mental health assessments, collaborative treatment plans, individual therapy, and group therapy to support clients in recovering from the effects of substance use and mental health disorders, consistent with the mission and philosophy of Fora Health.
ESSENTIAL JOB FUNCTIONS
Clinical Evaluation, Treatment Planning, and Documentation
Gather data systematically from the client and other available collateral sources. Obtain and interpret all relevant assessment information.
Determine the client's appropriate level of care and stage of change.
Ability to utilize the DSM-5 to assess and diagnose substance use and mental health disorder at time of enrollment.
Formulate with the client mutually agreed upon and measurable treatment outcome statements for each need. Identify appropriate strategies for each outcome.
Inform client of confidentiality rights, program procedures that safeguard them, and the exceptions imposed by regulations. Protect client rights to privacy and confidentiality in the communications of client information.
Assess treatment and recovery progress and, in consultation with the client and significant others, make appropriate changes to the treatment plan to ensure progress toward treatment goals at regular intervals and/or when indicated by changing circumstances.
Describe and document treatment process, progress, competencies, and outcome relative to treatment goals and objectives. Apply placement, continued stay, and discharge criteria.
Conduct and document continuing care, relapse prevention, and discharge planning with the client and involved significant others.
Work collaboratively with all service areas within the agency to provide appropriate client referrals and communicate relevant client information as needed.
Communicate and collaborate with referral agencies regarding scheduled screenings, status of client, and treatment and discharge planning.
Contact referents within 24 hours of admit, at least once per month during the client's treatment, and before and during the client's discharge. Document these contacts in progress notes.
Progress notes are documented in client charts within 24 hours for individual treatment.
ASAM treatment plan reviews are completed and reviewed within 30 days of admit and every 30 days thereafter. Submit treatment plans to supervisor as required and posted in a timely fashion in order to obtain continuing authorization for clients.
Progress toward goals are updated bi-monthly and communicated during with both internal and external treatment providers.
A Discharge Plan is developed with the client 30 days prior to a planned discharge, and continuing care plan is completed with client in final session. When required by contract, County exit surveys are completed within the last 5 days prior to the planned discharge.
A Discharge Summary is completed within 5 days following discharge. When required, insurance is notified within 24 hours of discharge.
Counseling and Education
Establish a helping relationship with the client characterized by warmth, respect, genuineness, concreteness, and empathy.

In this role you are going to be rotating job functions between a Behavioral Health Consultant (BHC) and a Mental Health Counselor.

Assess the nature and extent of each patient's emotional, behavioral, MH, and/or COD presenting problem, and provide any brief interventions necessary, typically ranging from 1-5 face to face visits (with some ongoing telephonic outreach) to ensure behavioral barriers to appropriate care are reduced. Demonstrates awareness of co-occurring disorders and chronic conditions and how each impacts patient outcomes and care.
KEY RESPONSIBILITIES - MENTAL HEALTH COUNSELOR:
Assess the nature and extent of client's MH and COD presenting problem, formulate a plan for service that addresses MH and COD issues and their impact on client's interaction, and ensure complete and timely documentation pursuant to OAR 309.019.0140.
Ensure productivity requirements are met by providing mental health treatment to individuals, couples, families, and groups that improve the client's level of functioning.
Ensure clinical supervision requirements are met pursuant to OAR 833.030.0031.
QUALIFICATIONS:
Must have appropriate credentials and level of training and licensed as an LCSW in the state of Oregonor have a master's degree in counseling and be licensed (LPC/LMFT), or doctoral level education in Clinical Psychology (PsyD/PhD) and appropriate licensing and credentialing.

Working with children from 1st\-6th grade, you will inspire the next generation of scientists through our innovative camp program. Our staff is made up of teachers, ed\-techs, retired teachers, artists, farmers, aspiring teachers, people seeking diverse work experience, and all of the above!
As an Assistant staff to our Lead Instructor, you will help facilitate our science camp by helping with hands\-on projects, behavior management, kid check in and out procedures, outdoor games, and more. You will work with our Lead staff to create a fun and safe and environment for our campers.

As a Lead staff, you will be in charge of facilitating our curriculum. Most of the time, you will have the aid of our Assistant Instructors. You teach just one topic for the entire summer. Some topics include Robotics, Chemistry, Engineering, Inventing, Anthropology, and more! We do our best to match you with your desired themed for the summer.
Your daily hours come to about 6.5\-7 hours. After the first week, we build proficiency in our process. Please note, at our Portland location we run Aftercare, this runs until 5:30pm and we provide additional compensation. We have some weeks built in as days off, with the options to assist others or take the week off.

A dependable car as you will be required to commute to different locations throughout Maine and New Hampshire
Commit to the entire summer schedule. Some camp topics offer 1\-2 weeks off during the summer season.

The Integrated Behavioral Health Counselor works as part of an interdisciplinary team supporting patients inside of Outside In's downtown primary care clinic. Primary duties include providing brief, solutions focused psychotherapy, assessment, skill development and access to resources as needed. This position acts as a consultant to the primary care provider, providing diagnosis and care plan input and management. Integrated Behavioral Health Counselors work to increase access of behavioral health services. Outside In works to provide high quality care focused to people who have current and historical barriers to accessing healthcare including trans and queer people, houseless people, immigrants, people with disabilities and chronic illness and people who use drugs.
This position is eligible for a $4,000 hiring bonus. The selected candidate will receive half of the bonus on their first paycheck and the remaining half on the paycheck following their six-month anniversary. Both payments must be made prior to June 30, 2026, as the bonus is funded by a grant.
Work is performed at a Federally Qualified Health Center located at 1132 SW 13th Ave, Portland, OR 97233 , and potentially at other locations as needed. Clinical supervision is provided.
Essential Duties
* Provide behavioral health assessment and brief, solution-focused treatment for clients with medical illness, chronic disease, mental health and/or substance abuse concerns in a primary care
* Use brief interventions to educate and help clients build skills to better manage sometimes complex health concerns
* Provide care to patients of diverse ages including pediatric patients
* Coordinate care with other systems and make referrals to services.
* Conduct assessment and diagnosis of mental health and substance use disorders
* Support patient's in accessing gender affirming care, including writing letters for gender affirming surgeries
* Maintain accurate and timely documentation for all services provided.
* Consult with interdisciplinary primary care teams to provide education regarding psychosocial treatments
* Provide crisis intervention as needed, including assessment and appropriate referrals.
* Assess client need and suitability for access to in-house psych medication management, refer to community mental health as appropriate

Qualifications
