We are a 245D licensed program which provides services to individuals who have mental illness, brain injuries, or substance use concerns. We operate 4 person group homes and individual apartment settings. Our residential services are rehabilitative, and our aim is to help individuals reach their personal goals for independence. This position is for a unique foster care program within Options Residential, which serves adolescent clients from 12-18. Schedule: Sun 3:30pm-11:30, Monday - Fri 6am-10am Starting Salary: $19.00 hourly, depending on qualifications. Benefits: Eligible 1st of the month after 60 days employment. Health insurance (ICHRA), Dental, Vision, Company Paid Life/LTD. 401k-eligible after 90 days employment. Must be scheduled 30+ hours per week to be benefits-eligible. What do Direct Support Professionals do? DSPs provide a variety of services to our residents, and duties vary from program to program, but may include (but are not limited to): Providing care giver services to our residents, including personal cares when appropriate and necessary Teaching and maintaining skills Cooking and feeding assistance Providing support with hobbies, studies, and social activities Medication administration And participating in fun recreational activities with residents! Our Adolescent DSP jobs are ideal for someone who has these minimum qualifications: 21 years of age or older Experience with adolescent population required Experience with behavior management for adolescents preferred Willingness to do personal cares (dressing, bathing/showering, toileting) with clients Valid US driver's license Basic computer skills What skills do Direct Support Professionals need? Scope of Role: Responsible for providing direct support within a single program or unit within a program, typically supporting 4 - 6 individuals in residential settings, or up to 10 individuals in periodic programs. Key Responsibilities: Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development. Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living. ESSENTIAL JOB FUNCTIONS To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below. Services and Supports: Direct Support: Provides direct support or assistance in accordance with individual service or program plans; assists with socialization and behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living. Records: Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality. Management of Individuals' Assets: Makes cost-effective and appropriate purchases within the budget of the individual served and according to their choice / preference Relationships: Maintains healthy and professional relationships with individuals, friends, families, guardians and case managers; greets family members and other visitors; implements Sevita Customer Service Standards. Reporting: Reports any instance of alleged abuse or neglect according to internal and external standards; reports medical, behavioral and other incidents in accordance with Sevita policy and external requirements. Rights: Maintains confidentiality, respects the rights of persons being served, according to applicable the bill of rights; practices universal precautions; assists individuals in exercising their rights. Health Care: Appointments: As needed, may accompany individuals to medical appointments; relays orders and information to and from medical providers as required. If an employee is determined to have lost their driving privileges by failing the company MVR driving record requirements per company policy, the employee will be required to sign a Non-Driver form and restricted from driving on behalf of the company until their MVR report satisfies the company requirements for a good driving record. Work Environment: Works at the program location and may accompany individuals into the community. At minimum, a High School Diploma/Equivalent is required All employees hired after 6/15/2018 shall submit to drug testing prior to employment. All employees shall submit to drug testing randomly and for cause/suspicion. Employees must submit to criminal background checks, and be determined not to be disqualified by the Department of Human Services prior to working with individuals served. About the Role The Advanced Practice Provider (NP/PA) provides exceptional, patient-centered primary care to help patients achieve their highest level of health within a community health setting. Key Responsibilities Patient Care Provide comprehensive, patient- and family-centered care within scope of practice, including health maintenance, preventive exams, acute and episodic care, chronic disease management, and patient education. Collaborate with an interdisciplinary team to manage care, coordinate services, and support patients in achieving their health goals. Complete full patient contact hours based on FTE. Participate in an on-call schedule as needed, based on clinic needs and provider group structure. Administrative Complete disability, FMLA, workers' compensation, and other necessary forms and letters within two regularly scheduled business days that the provider works. Review timecards to ensure work hours and PTO are accurately recorded. Quality & Compliance Adhere to timely documentation standards, including closing charts and addressing Epic in-basket messages within 48 hours. Complete peer reviews in a timely manner. Maintain all credentialing, licensure, and certification requirements. Follow SCHS policies and documented clinical protocols. Complete required Relias trainings, policy attestations, incident reports, assignments, and email follow-up in a timely fashion. Other Attend and actively participate in staff meetings and committees. Supervise (precept) students in the clinic as appropriate. Assist with other duties as assigned in support of SCHS's mission. Experience & Requirements Education: Advanced Practice Registered Nurse (APRN) or Physician Assistant degree from an accredited institution. Licensure & Certification: Current Minnesota license as an Advanced Practice Registered Nurse or Physician Assistant (PA-C) in good standing, with no pending or previous disciplinary actions. Current DEA license and BLS certification. Experience: Minimum 1 year of experience in a clinic-based or community health center environment required. 2+ years of experience in a clinic-based or community health center environment preferred. Technical: Experience with electronic medical records; Epic experience preferred. Language: Bilingual (English/Spanish) strongly preferred. Position Details & Benefits This is a 0.75-1.0 FTE, salaried, exempt position with a pay range of $128,000-$135,000 annually (based on 1.0 FTE), depending on experience.
