We are hiring DSPs who are professional, energetic, and nurturing. You will provide hands-on support to individuals in their homes as they work on daily activities and long-term goals. This includes activities such as meal preparation, enhancing social skills, providing companionship, community outings, and more.
Location & Schedule
Note: Your shift will not vary and you must be able to work the entire shift, every week.

If Part-Time (34 hours per week):
Friday: 2 PM - 12 AM
Saturday: 9 AM - 9 PM
Sunday: 9 AM - 9 PM
If Full-Time (40 hours per week):
Friday: 2 PM - 12 AM

A 6-hour shift on the same day each week, usually 4 PM - 10 PM

Role Purpose
The physician will provide professional medical services within the practicing Specialty to the best of physician's ability through direct patient care and spend additional time as necessary to perform other related duties such as completing medical records, providing MyChart consultations and inbasket management, conducting patient-specific education and collaborating with advanced practice providers and care team members.
Responsibilities
Manages the medical care of patient panel by providing or otherwise arranging for inpatient hospital care of physician's patients, either through regular hospital rounds, making arrangements with one or more hospitalist(s) or other qualified physician to provide coverage for physician's hospitalized patients consistent with Medical Staff requirements.
Collaborates with patients, families, and members of the care team to ensure excellent patient care outcomes at the clinic location(s) designated by BJC and any other BJC clinical outreach location to which physician may be assigned as patient care demands.
Performs and documents medical histories and physicals in the patient's medical record as required by hospital medical staff bylaws.
Provides or arranges for call coverage for clinic patients and inpatient call coverage in a manner acceptable to BJC and in accordance with Medical Staff bylaws, while observing and following all BJC policies and procedures and all applicable legal, ethical and professional standards.
Collaborates and teaches advanced practice providers, support staff or any care team member assigned in the care of physician's patient panel.

Maintain medication administration authorization by attending annual training, following all policies and procedures, and minimizing preventable medication errors.
Serves as safety chair at assigned site; assures safe, clean, and sanitary conditions inside and outside the home including maintenance and housekeeping.
Includes completing and documenting drills; completing monthly and quarterly safety inspections, making corrections and follow ups as needed; training new staff on drill procedures; following up on noted concerns in drill reports; and scanning all documents into safety binders.
Submits all client-related documentation according to established deadlines, communicating any potential delays or barriers to the QIDP. May be asked to collect program and client documentation from other sites and assure that it delivered or scanned to the appropriate place.
Monitor availability of client spending money; reconciles tracking and requests funds as needed from QIDP.
Provide transportation for clients in personal or agency vehicle for work, shopping, appointments, social events, etc.
Must have knowledge of client's medical conditions and be able to monitor and relay information to and from medical appointments.
Leads by example in: attending and/or prioritizing all required trainings as scheduled on or prior to due date to meet licensing and program requirements, attending required staff and agency meetings, and having excellent attendance and punctuality as defined by staff guidelines.

The Direct Support Professional (DSP) is responsible for the implementation of the client's Individualized Strategies, under the direction of a Qualified Intellectual Disabilities Professional. The DSP demonstrates this through daily documentation of service provided including but not limited to recreation, leisure, and activities of daily living for each client to whom they are responsible. The DSP will assure client rights, dignity, and privacy, while maintaining a clean and safe environment. The DSP may work with clients with maladaptive behaviors, which can include but are not limited to verbal aggression, physical aggression, self-injurious behavior, property destruction, and elopement. The DSP may also work with clients who require intensive care including but not limited to toileting and bathing, special diets with personal and/or enteral feeding, and enhanced funding. The DSP is assigned to a primary location, however, must be able to work across any site assigned, as determined by program needs.

The DSP - CDS is responsible to utilize the person's Implementation Strategy or Individual Plan and Behavior Support Strategies and implement the supports, care, and goals or objectives within. What you will be doing?
The DSP - Community Day Services (CDS) is assigned to work directly with someone who has an identified, specialized medical support need or behavior support need. The DSP - CDS is responsible to provide meaningful activities, training, and classes under the direction of the DSP - CDS Instructor and/or Program Manager. The DSP - CDS is responsible to utilize the person's Implementation Strategy or Individual Plan and Behavior Support Strategies and implement the supports, care, and goals or objectives within. The DSP - CDS is responsible for the implementation of the Behavior Support Strategies as assigned, providing support to clients with maladaptive behaviors. Some behaviors may be serious or require intervention, and may include (but is not limited to) physical aggression, verbal aggression, self-injurious behavior, property destruction, and/or elopement. The DSP - CDS will support people in conformance with the agency's mission, as assigned by the Program Manager, Director, or designee, and according to the needs of the person, program, and/or the agency as a whole. The DSP - CDS maintains all applicable documentation, records, and reports related to provided services and supports, and functions as a member of the Interdisciplinary Team. The DSP - CDS provides input and information to the IDT as needed. The DSP - CDS is responsible for the accuracy and quality of all assigned sub-contract work performed, if assigned. The Community Day Service program and the DSP - CDS provides services for people from diverse backgrounds, embraces differences in individuals, and promotes a positive attitude with peers and community members.
Essential Job Responsibilities
Provide enhanced support and supervision as defined in the client's Implementation Strategies or Individual Plan and Behavior Support Strategies where assigned
Actively engage, teach, and involve clients in daily, meaningful activities in a variety of settings, including in the community
Participate in all program activities alongside clients, including volunteering, outdoor activities, swimming (where applicable) and community outings
Compute daily attendance counts and accurately document to enter into the payroll system as assigned (where applicable)
Maintain a safe environment for self and clients, and react appropriately to emergency situations whether at the base location or elsewhere
Assist clients with personal care tasks, and implement training programs for individuals to learn to complete personal care tasks independently, as assigned
Demonstrate good verbal and written communication skills in order to provide appropriate instruction to clients, and as well as accurate documentation
Understand and implement objectives, support needs, and personal goals from Implementation Strategies or Individual Plans for clients as assigned
Facilitate programming or activities as directed, including client swim programs, outdoors, and outside of base location or elsewhere
Complete and submit all required documentation in a timely manner, including data sheets, incident reports and attendance sheets
Remain informed of assigned clients medical and dietary support needs and provide those supports daily and consistently
Understand and implement Behavior Support Strategies as assigned, and respond to emergency behavioral incidents should they occur
Participate as an active member of the interdisciplinary team, giving input and information to the team in order to provide insight, feedback, and recommendations to meet the client's needs effectively
Works cooperatively and respectfully with others

Complete trainings as assigned, including, but not limited to:
DSP training within 120 days of hire or sooner
Obtain and maintain current certification in CPR/FA
Obtain and maintain current certification in CPI
Complete annual OIG Rule 50 training

Complete other training as assigned by program
Abide by the AID Code of Ethics
Adhere to all AID program policies and procedures

Transport clients to and from base CDS location as assigned for both community outings or regular transportation purposes
Provide culturally competent and sensitive service and interactions with clients and families
Provides support and/or transportation for client medical or therapy appointments as requested from CDS management during the typical CDS hours
Works collaboratively with other community groups as well as volunteering contacts and other program's staff members
Complete and/or maintain Medication Administration training and authorization in order to pass medication if required at regular base location
Accept reassignment to another AID location or client(s) and/or cross-training as assigned due to programmatic needs or program closure

Essential Job Responsibilities

Must maintain a clean, sanitary environment, inside and out of all assigned buildings and vehicles, on a daily basis. This includes but is not limited to surface, deep, and sanitizing cleaning tasks as assigned.
Must be alert to any emergency situations that may occur and take appropriate action per program and agency policies and procedures.
Provide transportation for clients in personal or agency vehicle for work, shopping, appointments, social events, etc.
Must have knowledge of client's medical conditions and be able to monitor and relay information to and from medical appointments.

Assist in the orientation/training of new staff.
Participate in the planning and implementation of community and in-house activities.

Sunday: 9 AM - 9 PM
If Full-Time (40 hours per week):

A 6-hour shift on the same day each week, usually 4 PM - 10 PM

You must meet the following requirements:
Pass:
A fingerprint criminal background check
A child abuse and neglect check
A drug screen.
Provide:
An original document (no photocopies) showing high school completion or high school equivalency. Examples include: High school diploma, GED certificate, college transcript
Residency for the past 2 years
Current & original documents that verify your identity & ability to legally work in the United States
Two verifiable employment references within the last 3 years
Proof of valid drivers license at the time of interview
