The position of the Remote Care Coordinator will perform telephonic encounters with patients on behalf of our practice partners each month. This is a 1099 Contractor position and Contractor will be responsible for their own taxes.
Esrun Health is seeking Medical Assistants to work part-time from their home office as independent contractors while complying with HIPAA privacy laws. You will set your own hours and will not be held to a daily work hour schedule. Esrun Health wants its team members to have the flexibility to balance their work-life with their home life. Part-time team members will typically need to dedicate an average of 20-30 hours per week to care for their assigned patients initially. This time commitment will increase as the patient assignment increases. This unique business model allows you to choose what days and what hours of the day you dedicate to care for your patients.
The Care Coordinator will be assigned a patient panel based on skill and efficiency level and is expected to carry a patient panel of a minimum of 100 patients per calendar month within the first three months of assignment. Care Coordinators will be expected to complete due diligence measures on 100% of assigned patients and billable encounters on 90 percent of the patients they are assigned each month unless patients are unable to participate due to current health conditions.
Compensation Structure
Esrun Health utilizes a
productivity-based pay structure
:
$ 8.00 per completed patient encounter up to 99 encounters/month.
$ 8.50 100-149 encounters/month
$ 9.00 150-199 encounters/month,
$10.00 200-249 encounters/month
$11.00 >250 encounters/month.
Payment tier increases require 3 months consistency to achieve.
There is a $1/encounter incentive compensation for bilingual contractors equal to $3/hr but is only applied if hired into a bilingual position.
Monthly outreach will consist of
cumulative time
to include chart review, contact attempts (calls/texts/emails), actual call time, care coordination, and documentation/billing.
This time is billed out in 20-minute units of service referred to as “encounters” and each patient can be billed for up to three units of service or “encounters” each month.
20-39m=1 encounter, 40-59m=2 encounters, >60m=3 encounters
EXAMPLE:
Chart Review: 8 min
Outreach Attempts: 6 min
Actual Call: 11 min
Care Coordination: 9 min
Total Time Spent: 44 min = 2 encounters
As a productivity-based position - there is no compensation outside of the billable encounters described in the compensation structure other than goal bonuses, referral bonuses, and employee engagement activities resulting in monetary prizes.
There is no pay for onboarding. Onboarding is self-led and can be completed in as little as 3 days (3-6hrs total time) - but can, depending on individual schedule, take up to 14 days.
What your impact will be:
The role of the Care Coordinator is to abide by the plan of care and orders of the practice.
Ability to provide prevention and intervention for multiple disease conditions through motivational coaching.
Develops a positive interaction with patients on behalf of our practices.
Improve revenue by creating billable Care Management episodes, increasing visits for management of chronic conditions.
Understand health care goals associated with chronic disease management provided by the practice.
Attend regularly scheduled meetings (i.e., Bi-Monthly Staff Meetings, monthly one on one's, etc.). These “mandatory” meetings will be important to define the current scope of work.
What we are looking for:
Certified Medical Assistants
A minimum of two (2) years of clinical experience - preferably in pain management
Hands-on experience with Electronic Medical Records as well as an understanding of Windows desktop and applications (Microsoft Office 365, Teams, Excel, etc.), also while being in a HIPAA compliant area in home to conduct Care Management duties.
Ability to exercise initiative, judgment, organization, time-management, problem-solving, and decision-making skills.
Skilled in using various computer programs

The perfect candidate will have thorough knowledge and understanding of geriatrics with the ability to provide compassionate care. Administrative proficiency to maintain patient records and excellent communication skills to effectively interact with patients, caregivers, healthcare professionals and team members is essential. A great deal of team collaboration is required in an effort to provide comprehensive patient care in this supportive role.
To excel in this role you should be personable, service-oriented, and have professionalism of the highest caliber with exemplary communication. Strong attention to detail is required as this position routinely handles patient information in electronic medical records. Having excellent time management and multi-tasking skills, strong organizational and planning skills, and the ability prioritize work are essential for this role.
The M.A.'s responsibilities include:
Acting as the liaison between Medical Staff, Patients, Pharmacies, and Hospitals.
Completing Triage duties while managing patient medical records.
Transmiting prescription refills as directed by the assigned provider.
Verifying all patient information is accurate and review medical histories to gather additional information as needed.
Obtaining Lab Results and documentation in the patient's medical records.
Collaborating with the Scheduling team while coordinating urgent and same-day patient visits.
Responding to patient and caregiver questions in a timely and knowledgeable manner.
Maintaining compliance with HIPAA and Health Care regulations.
Interested candidates should have:
Medical AssistantCertification
required.
High school diploma or equivalent; completion of a medical assistant program, preferred.
Primary Care experience preferred.
2 years of experience as an M.A. preferred.*
Knowledge of medical terminology, anatomy, and basic healthcare principles.
*Applicants with more than 1 year of experience, but no M.A. certification, MPS will assist the new hire in funding the exams cost. I
f an applicant does not have a full year of MA experience, and no certification, MPS will assist the new hire in funding the schooling and exam cost.

Job Summary
Part-time, 24 hours a week, 12 hour shifts
Night shift
Performs patient observation, documents patient care and response to care and communicates significant information to the registered nurse and other appropriate team members.
Essential Functions
Assists patients with activities of daily living.
Provide basic care to non-acutely ill patients.
Assist in the maintenance of a safe and clean environment.
Perform these duties under the direction and supervision of a Registered Nurse.
Provide direct patient care for patients on the Patient Care Unit.
Services are determined by the patient's plan of care and include personal care and activities under the direction of the Licensed Care Giver and in conjunction with the Respiratory and Rehab departments and the Laboratory.
Functions as a member of the Patient Care Team in conjunction with the Licensed Direct Care Giver.
Knowledge/Skills/Abilities/Expectations

Performs other related duties as assigned.
Qualifications Education
High school graduate or GED.
Successful completion of a Patient Care Training program.
Licenses/Certification

BLS certification required.
