Job Overview: The Licensed Social Worker provides direct clinical social work services to patients and their families in an interdisciplinary team approach. The Licensed Social Worker collaborates with members of the healthcare team to facilitate the progression of the patient's treatment and discharge plan. This role incorporates assessments, care coordination, and education of the patient/caregiver/healthcare team to reach appropriate goals from hospitalization and post-hospitalization care. Qualifications: Required: * Master's degree in Social Work (M.S.W.) from an accredited School of Social Work Preferred: * Three or more years of Social Work experience in a healthcare-related setting * Epic experience * Case Management experience Certifications and Licenses Required: * New Jersey State Social Work Licensure (L.S.W. or L.C.S.W.)
