The Role
Under the general direction, the Nurse Practitioner is responsible under direction of a physician for providing general
clinical care including assessments, medication management and medication administration. The incumbent will work autonomously, collaborating with physicians, peers and others in the health care community. This work is carried out in support of the mission and goals of Samaritan Daytop Village.
Responsibilities

Qualifications
Who You Will Be
NYS RN and NP (Psychiatry and Mental Health) licensures.
Valid DEA registration.
BLS certification.
eMEDNY Provider.
Master's degree from an accredited advanced Nurse Practitioner program in Psychiatric-Mental Health.
At least one-two years' experience working as a Psychiatric Nurse Practitioner preferably in addiction medicine or substance use and/or mental health treatment programs.
Knowledgeable about federal and state regulations governing medical/clinical treatment in substance use treatment programs.
eMedNY Provider or willingness to enroll upon hire for Permanent Staff.
Active NPI.
Ability to demonstrate sensitivity to patients with a variety of psycho social issues.
Ability to work with a medical and clinical interdisciplinary staff to ensure appropriate management of clients.
Computer literacy including proficiency in Microsoft Office Suite and EMR.
Excellent interpersonal, communication, analytical, presentation and organizational skills.
Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.

The base hourly range for this role is $59.10 to $65.50 per hour (plus evening and weekend differential) based on a full-time schedule. Overview: The Psychiatric Provider/Psychiatrist is responsible for delivering high-quality psychiatric services in collaboration with the medical staff, Associate Psychiatric Director, and Chief Psychiatric Officer.

Responsibilities:
Conduct outpatient psychiatric evaluations for children, adolescents, and adults
Diagnose and treat mental, emotional, and behavioral disorders across the lifespan
Collaborate closely with CMCS' outpatient counseling team to ensure integrated care
Prescribe, adjust, or discontinue medications in accordance with the Collaborative Practice Agreement and state regulations
Maintain timely documentation of assessments, progress notes, and reports for agencies and funders
Respond promptly to calls and emails-within 24 hours or sooner if clinically necessary
Provide support to Provident's 24/7 crisis clinicians during client emergencies
Requirements:
Master's or Doctorate Degree in Nursing with focus on Psychiatric nursing
Graduation from a state-certified advance practice program specializing in psychiatry/mental health
3+years of experience as a psychiatric provider
Experience in tele medicine
An unrestricted license to practice as a Psychiatric Nurse Practitioner in the state of New York
Must have a valid DEA number and unrestricted DEA license in New York
Office of Mental Health Clinic Experience
Bilingual English/Spanish preferred

The Forensic Assertive Community Treatment (FACT) Team Psychiatric Nurse Practitioner is a member of the community based multidisciplinary treatment team, and in conjunction with the Clinical Director of Forensic Assertive Community Treatment Team provides oversight of the delivery of clinical services. The FACT Team Nurse Practitioner has three main functions: 1) Provision of psychiatric assessment and treatment utilizing an individual centered approach while taking into consideration complex medical issues; 2) sharing knowledge of psychiatric illness and treatment, with the goal of educating and guiding the multidisciplinary team; and 3) oversight of the development, maintenance, and supervision of medication administration. Duties include:
1.Upon admission, conduct person-centered psychiatric assessments.
2.Collaborate with nursing staff regarding individual medical needs and recommendations for referrals to community physicians for further assessment and treatment. Collaborate with medical providers as needed.
3.Meet with individuals at the office and in their homes to prescribe psychiatric medications, provide medication education, assess efficacy of medications, and monitor for side effects. Conduct regular assessments of therapeutic responses and side effects and educate individuals regarding psychotropic medications.
4.With other team members, provide support, education, and counseling to family members of individuals to help them understand mental illness and how to best support the recovery of the individual, as needed and if indicated.
5.Actively collaborate with inpatient providers during the acute or long-term hospitalization of an individual.
6.Participate in and supervise the development of the comprehensive treatment plan for each patient at treatment planning meetings.
7.Direct and collaborate with the registered nurses to develop, revise, maintain, and supervise team pharmacological and medical treatment as well as medications policies and procedures.
8.Provide transportation to individuals when necessary. In addition, an employee must be approved to drive agency and personal vehicles for business purposes by maintaining a valid NYS driver's license and must be approved by the agency's vehicle insurance carrier. An employee must always drive in a safe manner and use all safety devices.
9.Mohawk's programs and offices are multi-level with stairs only. Individual services in the community include visits to individuals' apartments which can be on the second and third floors. Employees must be able to travel to all levels of the facility in a safe and reasonable time frame.

Job Description
The Youth ACT team (Assertive Community Treatment) provides mental health services to youth and families who are at risk of entering or returning to a higher level of care such as inpatient hospitalization or residential setting with the goal of maintaining safety and stabilization. The Youth ACT team works closely together to address mental health concerns of youth and the impact these concerns have on the family as a unit. Each member of the team provides individualized treatment through assessment, counseling, and clinical services to youth and families in a variety of settings such as in the home, and other community-based settings.
Position Summary:
We are seeking a compassionate and experienced Psychiatric Nurse Practitioner to join our Youth Assertive Community Treatment (ACT) team. This role is an invaluable resource and plays a crucial role on a multidisciplinary team. The ideal candidate has a passion for community and family work. The Psychiatric Nurse Practitioner provides psychiatric evaluations and medication management for youth and their families in community and home settings. Responsibilities include treatment planning, collaboration with therapists and community organizations, medication oversight, emergency assessments, and participation in case reviews and quality improvement initiatives.
Major Responsibilities/Activities:
· Provide psychiatric evaluation and medication management to youth and adults receiving services in the outpatient clinical services programs;
· Review and sign client treatment plans, if needed;
· Provide consultation to agency staff regarding client treatment; participate in group supervision and case review meetings
· Complete progress notes for client services following appropriate Current Procedural Terminology;
· Provides any information necessary for the proper dispensing of medication at home, and when needed at school;
· Communicates any medication, behavioral or primary health concerns of child to parents and child's therapist;
· Document any concerns in the child's chart and monitor monthly;
· Participate in various program committees e.g. Special Incident Review, Case Conferences, and Consumer Advisory as necessary;
· Review all Laboratory test results and discuss any concerns with the nurse if necessary;
· Work collaboratively with mental health therapists regarding behavioral health concerns and outcomes;
· Demonstrates appropriate communication skills both written and verbal;
· Shows respect and courtesy in all interactions and communication internal or external to Agency;
· Interest, knowledge and support for the Agency's mission, values and philosophy;
· Problem solving and conflict resolution skills in both organizational and interpersonal matters;
· Supports and communicates with supervisor and coworkers in a cooperative manner;

· Must maintain and submit all of the required documents and comply with all aspects of the Vehicle Safety and Usage Policy.

· Participates and contributes to program Quality Improvement process.
Minimum Requirements:
· Psychiatrist or Nurse Practitioner from a recognized accredited school
· Current NYS registration
· Board Certified in Psychiatry
· Two years' experience with children and families required
· Two years' experience as a Psychiatric Nurse Practitioner is preferred
· Ability to relate well to children and other staff members
· Valid NYS driver's license and access to reliable transportation
Competitive Pay Range of $150,000-$175,000 annually based on experience

Aesthetic Medical Practitioners will be responsible for assessing and providing patients with aesthetic medical services including but not limited to cosmetic injections (i.e neurotoxins, dermal filler), laser therapy (i.e BBL, HALO) and laser hair removal.
What You Will Do
* Provide client consultations, develop personalized treatment plans and skillfully perform treatments tailored to each patient's needs and objectives
* Educate patients on all treatment processes, including pre and post treatment care instructions
* Acknowledge and respond to relevant patient queries, needs and expectations
* Apply best practices and stay up to date with market trends and techniques for treatment
* For mid-level providers and above (NP, PA, MD, ND and DO), provide in clinic GFE (good faith exam) support when needed, as licensure permits
* Ensure patient safety at all times
* Suggest and promote retail products and additional services offered at the company
* Clean and maintain equipment and inventory of products
* Follow established treatment protocols
* Keep track of and update patient charts; take before and after photos
* Generate and maintain a patient base
* Attend trainings and meetings
* Assist with other duties as assigned
* Occasional travel may be required to our other locations for training
What You Will Bring
Skills, Knowledge & Expertise
* Active Registered Nurse (RN), Nurse Practitioner (NP), Physician Assistant (PA), Medical Doctor (MD), Doctor of Osteopathy (DO), or Naturopathic Doctor (ND) license required to practice in the state of employment
* 2+ years of experience desired
* Experience in a clinic setting preferred
* Laser experience preferred
* Existing clientele is a plus
* Exceptional interpersonal and communication skills; must be able to communicate effectively with patients, peers and managers
* Ability to multi-task while being attentive to clients and remaining flexible to the needs of the business
* Ability to work as part of a team and take initiative independent of direct supervision
* Proficient with email
