Responsibilities

Qualifications
Bilingual in Spanish preferred.
Master's degree from an accredited college or university with a concentration in early childhood education/development, early childhood special education, special education, psychology or related field.
Must be a Board Certified Behavior Analyst (BCBA); Marriage Family Therapist (MFT) or licensed psychologist preferred.
Three years of related professional experience working with children with Autism Spectrum Disorders (ASD) in a multi-disciplinary team setting preferred.
Advanced knowledge of scientifically-validated methodologies and approaches found to benefit children with ASD; familiar with current related research findings.
Expertise in approaches to intervention based on the science of Applied Behavior Analysis. Competent in employing and directing behavior analytic methodologies including Pivotal Response Training (PRT), Natural Environment Teaching (NET), Picture Exchange Communication System (PECS), Behavior Skills Training (BST), and Experimental Functional Analysis (EFA).

* Develop case management plans to support self-sufficiency, independence, and life goals. Assess the level of family and individual functioning and make appropriate referrals to community agencies and organizations for support services, including, but not limited to:
* Homeless housing project stability
* Evaluate homeless individuals using the Vulnerability Index Service Prioritization Decision Assistance Tool (VI-SPDAT)
* Shelter for victims of domestic violence
* Therapy
* Employment stability and job placement
* Educational program
* Child Welfare Services
* Adult Protective Services
* Financial stability, including money management
* Utility assistance
* Child Care
* Home weatherization
* Design and implement structured activities for clients.
* Provide assistance to "special needs" families requiring services, such as translation (English/Spanish), transport (handicapped) and crisis counseling for families and individuals experiencing acute domestic violence trauma.
* Provide direct services such as peer counseling, information and referrals, transportation and advocacy as needed.
* Interview and assess needs of individual's protective or preventative services.
* Perform outreach efforts with landlords/property managers to facilitate access to homeless housing project when appropriate and needed.

* Working in the Domestic Violence Assistance, Elder Abuse, and Human Trafficking Programs will require:
* Complete 40 hour Domestic Violence Certification Course
* Collaboration with Kings County Behavioral health
* Working in the Cal Learn Program will require:
* Completing Positive Youth Development Training
* Other duties as assigned.
* Follow safety protocol and standards.
* Assist supervisor, Program Manager, or IPSS Director as required/requested.
* Participate in staff meetings and attend other meetings and seminars as directed.
EXPERIENCE/EDUCATION REQUIREMENTS:
* Bachelor's Degree in social services or related field in psychology, rehabilitation, or social work from an accredited four-year college, AND, one year work experience in related social service field
OR
* Associate's Degree in social services or related field in psychology, rehabilitation, or social work from an accredited college; AND three years' work experience in a related social service field.
OTHER REQUIREMENTS:
* Travel and attend out of area meetings and conferences as required per the funding source(s).
* Work variable hours and days, including those outside regular Monday through Friday, 8 a.m. to 5 p.m. business hours.
* Meet and maintain KCAO hiring requirements which include:
* Criminal Record Clearance including California DOJ, FBI, Child Abuse Centralized Index (CACI), National Sex Offender Public Website (NSOPW), and the California Sex and Arson Registry (CSAR)(Megan's Law).
* Reliable, insured transportation and valid California Driver License
* Motor Vehicle Report (MVR) acceptable to KCAO's vehicle insurer.

Summary: The Workforce Group, a LEMOINE company, is seeking an experienced Case Managers supporting the Administration for Children and Families (ACF), Office of Human Services Emergency Preparedness and Response (OHSEPR) under the U.S. Repatriation Case Management and Financial Support Services Program. Case Managers will provide direct support ensuring safe reception, service coordination, and access to critical resources.
As part of the ACF/OHSEPR Repatriation Case Management and Financial Support Services Program, this position may require travel and is contingent on contract award, with salary subject to change.

Duties and Responsibilities:
Provide end -to -end case management support, including intake, needs assessment, service coordination, and case closure in accordance with OHSEPR policy.
Meet repatriates at ports of entry or final destinations (as directed), ensuring safe reception and transition to temporary housing, medical, or other support services.
Coordinate with federal, state, and local partners, as well as community service providers, to connect repatriates with appropriate resources (housing, food, medical care, transportation, etc.).
Support complex cases involving vulnerable populations such as unaccompanied minors, individuals with medical or mental health needs, or other at -risk groups.
Document case management activities, assessments, and referrals in OHSEPR's case management system within established timelines.
Monitor client progress during the 90 -day temporary assistance period, conducting regular outreach and providing updates to OHSEPR.
Prepare case notes, reports, and correspondence that are clear, concise, accurate, and compliant with program requirements.

Qualifications, Knowledge, Skills and Abilities: Bachelor's degree in Social Work, Psychology, Human Services or a related field. Experience providing direct client services and conducting needs assessments.

ABOUT THE ROLE
The Community Connection Case Manager position will support the flow and responsibilities of the Clinical Department and Access Center Programming, and will work closely with a caseload of members towards a variety of youth centered goals. This role is an integral part of case management and clinical services at SPY with both field- and Access Center-based responsibilities.
WHAT YOU'LL DO:
Support the SPY Clinical and Access Center Teams in day to day operations and programming.
Utilize innovative outreach strategies to enroll young people into SPY's case management programs.
Support members in building and deepening social connections through planning and facilitation of community field trips and onsite gatherings with transitional age youth.
Conduct intakes, assess needs, and create youth centered goal plans to foster youth development, social connection, housing stability, and wellness.

Support youth with reunification, reconnection, and relocation as needed.

Provide mediation, crisis intervention, and restorative justice member accountability processes with youth, as needed.

The Case Manager works out of the Lighthouse Transitional Living Program in West Los Angeles. Lighthouse TLP is a housing first low-barrier, high tolerance program providing housing and comprehensive services to youth ages 18-24 who are experiencing housing instability. Interact and build relationships with residents, demonstrating the ability to support resident success, solve problems and enforce house rules.

Requirements
WHAT YOU'LL NEED:
Bachelor's Degree in Social Work, Psychology, Sociology or related field is preferred. Related work or lived experience of housing insecurity or homelessness will be strongly considered in place of or in addition to a Bachelor's degree.
1-2 years of experience providing direct services to high-risk transitional age youth (TAY) is required.
1-2 years of experience accessing social services (homeless services, DCFS, probations, DPSS, etc.) is a plus.
Best practice models including: Housing First, Positive Youth Development, Authentic Youth Engagement, Motivational Interviewing, Harm Reduction, Trauma-Informed Care, Critical Time Intervention
A self-starter who is prepared to drive innovative solutions within the organization
