Job Summary The Pediatric Provider (MD, DO) will provide primary medical care services to patients of Sea Mar Community Health Centers in accordance with training, experience, certification, insurance and licensure in a manner consistent with guidelines established in the principles of practice at Sea Mar and its mission statement. The provider will work collaboratively with staff to provide team-based coordinated care. The physician prescribes and/or dispenses required medication to patients according to the individual prescriptive authority granted by the Washington State Board. The physician participates in organizing and directing preventive health programs such as immunization, disease detection and disease prevention activities, including educational programs for patients and community groups in the service areas with approval from the Medical Director and/or Health Center Administrator. We are recruiting for the following position: Sea Mar is a mandatory COVID-19 and flu vaccine organization Position Summary Working under the supervision and guidance of a licensed nurse (RN or LPN), the CNA is responsible for assisting with the direct patient care within the scope of their practice, as well as other work in the community which supports the patient milieu. The CNA assists licensed staff to ensure optimal patient care and participates in the healthcare team effort to provide and maintain a clean, safe and attractive environment for residents. The work will include components of direct resident care, nutritional support, observation and documentation, and general maintenance of the resident's environment. The CNA is responsible for maintaining his/her own clinical and mandatory education requirements, and for meeting the established standards of the profession; he/she is an advocate for the residents under his/her care and their families. He/she collaborates with other professional disciplines to ensure effective and efficient resident care delivery and the achievement of desired resident outcomes. The duties and responsibilities of a CNA include, but are not limited to the following: Engaging/communicating with residents on an ongoing basis to develop a trusting, cordial relationship with the residents. Observing residents, and taking vital signs as per plan of care. Reporting any health issues and changes of conditions immediately to the supervising nurse. Taking care of a resident's personal hygiene, including assisting with showers/baths, shaving, hair care, nail care, etc. Remind and/or assist residents with toileting as per care plan. Emptying catheter bags. Monitoring vital signs including temperature, pulse, respiration, blood pressure, pain level, and weight as assigned. Serving meals, clear dining area, meal tray delivery to resident's room as approved by licensed nurse. Making beds, changing linens, making laundry ready and available for laundry aide. Keeping the residents' space tidy on an ongoing basis, to include washing dishes in the resident's unit, discarding food older than 3 days, etc. Assisting the resident to activities, meals, etc. Provide CNA care as identified in the Resident's Care Plan and as directed by the supervisor. Report any resident's refusal of care or any change in condition to the licensed nurse, Wellness Coordinator or Director of Nursing Services or to the med tech if no LN available. Maintain a safe environment with working knowledge of emergency equipment and appropriate usage as necessary. Consistently demonstrate principles and practices of infection control in the performance of duties. Identify any potential abuse situations and report immediately to the licensed nurse or Wellness Coordinator or Director of Nursing Services and call state abuse hotline. Complete documentation of care provided and other required documentation must be completed before reporting off of assigned duty. Refer inquiries for circumstances beyond scope of practice, knowledge and skills, to the appropriate person (see number 2 for particular staff). Know and observe HIPPA regulations in the dissemination of resident information. Participate in general monthly in-services up to 12 in-service hours per year. Participate in self-evaluation. Maintain established standards of care within the scope of CNA practice. Recommend program change and development; equipment, environmental or personnel changes that streamline the operation without negatively impacting quality of care. Know and utilize community policies and procedures. Participate in and/or attend organization sponsored and external community events. Performs other duties as assigned. Qualifications Must be able to communicate effectively with all members of the healthcare team. Must be sufficiently flexible to perform a variety of duties, some or all of which may undergo frequent change. Must successfully pass a criminal background check, as well as pass any state-specific abuse/fraud registry checks. EXPERIENCE/SPECIAL SKILLS Must be able to speak, write, read, and follow instructions in English. LANGUAGE SKILLS Bilingual in Spanish/English preferred. MATHEMATICAL SKILLS Ability to do simple calculations such as addition, subtraction, multiplication and division. REASONING ABILITY Employee must display good judgment and decision-making ability. Education and Experience, Certificates, Licenses, Registrations, and Medical Screening EDUCATION AND EXPERIENCE High school diploma or GED required. Must have successfully completed an accredited Nursing Assistant Program and be currently licensed by the professional licensing entity for CNA in WA state. Maintains current CPR/BCLS certification at all times. Long Term Care Experience is preferred but not required. ADDITIONAL REQUIREMENTS Must complete a pre-hire and annual TB test screening. ADMINISTRATIVE DUTIES Assists in insuring agency compliance with WAC's, RCW's, and other pertinent laws, regulations, and contracts. Ensure that charts, prescription refill requests, and all information or paperwork the psychiatric prescriber will need is prepared ahead of time and organized in the fashion preferred by the prescriber. Ensure medication storage, maintenance, dispensation and waste logs are maintained in accordance with Sea Mar policies and procedures. Any area of non-compliance should be reported immediately to the supervisor/manager, if applicable. Ensures that all medical, nursing, health & safety and personnel policies and procedures are being followed and implemented. CLINICAL DUTIES Assists psychiatric staff with client flow and provides quality of care in a timely manner. Completes routine checks and monitors clients to detect unusual or harmful behavior and reports observations to psychiatric staff. Prepare the patient for their office visits with the provider including obtaining subjective data and vital signs, ensuring charting is in accordance with Sea Mar's policy. Works as part of an integrated care team that may include psychiatrists, social workers, behavioral health professionals, chemical dependency professionals and case managers. Provides mentally impaired or emotionally disturbed clients with routine physical, emotional, psychological, or rehabilitation care under the direction of psychiatric staff. Receives and processes medication refill requests from pharmacies by preparing refill requests for provider review and approval, and ensures prescriptions are faxed to the pharmacy. Checks EHR to confirm requests are valid. If valid, the aide will send the prescriber a refill request through EHR. If invalid, the aide will communicate with the pharmacy. Checks medication history in EHR as requested by prescriber. Collaborates with prescriber to determine next steps. Fields phone calls from clients regarding questions about side effects and/or status of refills. Facilitates client enrollment in prescription assistance programs.
