Job Summary: This position is primarily responsible for assisting and providing safe, consistent, and appropriate patient care within his or her scope of practice. He or she is an advocate for the patient when working with the RN or Providers. Ability to handle a diverse range of situations and responsibilities is the primary duty of an LPN. Assess plans and delivers nursing care in collaboration with a professional Registered Nurse. Role & Responsibilities: Performs with opportunity for independent judgment under general supervision. Gives medication as directed by the provider, vital signs, such as blood pressure, temperature, and weight, basic wound care, injections, and immunizations. Entering information into Electronic Medical Record (EHR). Scheduling appointments, keeping medical record current, and working with insurance companies. Assumes self-responsibility for all direct patient care activities as indicated by the Registered Nurse (RN) within the scope of the Nurse Practice Act and Shenandoah Valley Medical Systems (SVMS) policies and procedures. Reinforces instructions given to the patient by the Provider and RN, refers new needs for instruction to RN. Assists in preparing and directing patients for examination to meet SVMS objectives. Performs effective in-take assessments including a brief patient history, chief complaint, current medications, allergies, and significant previous medical/psychosocial problems. Screens assigned telephone messages and processes inquiries or refer to appropriate medical personnel per SVMS protocols. Effectively performs specific laboratory tests/Point of Care testing that has been approved by the Laboratory Director Performs nursing procedures and treatments, and medication administration according to SVMS protocols upon request of the provider. Notifies the RN of significant findings or changes in patient's physical or mental condition. Maintains a thorough knowledge of special programs offered by SVMS including family planning, EPSDT and cancer control, and of eligibility and guidelines of these programs which are available to patients Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary. Demonstrates courteous and cooperative behavior with patients, families, co-workers, subordinates, supervisors, and other department staff. Always maintains patient confidentiality and privacy according to SVMS policies and procedures; uses discretion when discussing patient matters. Acts as role model for ancillary personnel Practices within the limits of the Nurse Practice Act and understands legal consequences of nursing action. Other duties as assigned Essential Skills and Education Requirements: Must be a graduate of an accredited practical nursing school. Must have a license from the West Virginia Board of Examiners of Practical Nurses. Must have the demonstrated ability to function effectively within a multi-disciplinary team. Knowledge of lab testing, diagnostic equipment, and medical procedures and protocols is required. Must have excellent skills in patient communication and education. Annual basic CPR certification is also required. Previous experience working as an LPN is preferred. Physical Requirements: Work is typically performed indoors with frequent sitting, standing, and bending necessary to perform daily job requirements. Occasional lifting may also be necessary in this position.
