Report to Nursing Management, this position is responsible for safe, competent nursing care for patients in accordance with facility policies, standards, and philosophy. Provide professional leadership and support to the healthcare team. The concepts of Patient C entered C are will provide the foundation for all nursing care. Demonstrates Service Excellence standards at all times. The Float RN supplements unit-based staffing during periods of high census fluctuation and staffing needs. Acts as team members on multiple units while being assigned to one unit per shift. The Float RN does not have a physical home unit, but instead functions out of a home department and is assigned to units on an as needed basis which can be daily or longer to cover vacancies and leaves. For providing maximum flexibility the Float RN will receive supplemental pay. Performs nursing process with minimal supervision and seeks assistance as necessary. Utilizes discretion and independent judgment in applying the nursing process to the provision of age-specific and developmentally appropriate quality patient care. Exhibits flexibility by rotating within the cluster for those units the Float RN has obtained competency. Functions in a skills mix environment and directly supervises and delegates tasks to licensed and unlicensed staff while working with various healthcare members as part of a multidisciplinary team on a particular unit. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and tasks may be assigned.
Knowledge of and comply with federal, state, local laws and regulations, including health care professionals practice act requirements as they pertain to home dialysis program and patient care requirements.
Train patient (and/or care partner) in the practice of self-care Peritoneal Dialysis or Hemodialysis upon meeting federal and state regulations governing Registered Nurse qualifications.
Conduct home visits to assess the patient's home environment per policy; initially and minimally annually thereafter. Routinely evaluate patient performance and assess the home environment more frequently as needed to improve care.
Communicate results of patient assessment, reassessment, and ongoing monitoring to the physician, team members, and others as appropriate to the individual needs of the patient.
Document all nursing services in the Electronic Medical Record including but not limited to training sessions, routine and non-routine in-person interactions, and phone conversations. Documentation should accurately reflect the patient status and nursing interventions and be written to ensure continuity of care.
Participate in infection control monitoring, implementation, and recording as requested.
Be familiar with emergency equipment and all emergency operational procedures. Communicate and regularly review Emergency Preparedness procedures with all home patients, including but not limited to emergency disconnection from dialysis equipment, what to do and who to contact if displaced from home.
Ensure clinic and patient supply inventory and usage is appropriate for patient prescriptions and place orders in timely manner to avoid fees and penalties.
Continually seek to improve patient and quality outcomes through tracking and trending of program and patient performance, use of critical thinking skills and root cause analysis to improve patient and quality outcomes. Assist in obtaining data for the continuous quality improvement activities.
Obtain routine and non-routine laboratory tests as ordered and communicate critical lab values and urgent patient needs to the responsible physician or physician extender in a timely manner.
Complete and document monthly review of patient medication profiles. Administer medications as ordered by the physician.
Responsible for performing on call nursing services, nights and weekends, on a rotational basis as needed or assigned.
Flexible with staffing locations and hours to accommodate patient and USRC home program needs.
Regular and reliable attendance is required for the position.
Home Hemodialysis Only:
* Coordinate home evaluation with technical services department to ensure that the necessary electrical, plumbing and drainage requirements for proper equipment operation are met prior to patient acceptance into home program.
* Ensure required and appropriate water sampling is complete per policy and product requirements. Monitor and report water sample and culture results.
QUALIFICATIONS/REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Requirements include:
Current RN license in applicable state. License must be maintained as current and in good standing.
12 months of clinical experience in providing nursing care and 3 months of experience in the specific modality for which the nurse will provide home dialysis training, except in states with a more stringent requirement such as Texas, which requires at least 6 months experience in the specific home modality.

CPR certification required prior to patient care assignment.

The nurse communicates utilizing the SBAR in all hand-off situations.
Performs documentation of patient care including: assessment, plan of care, implementation, evaluation, and the Watson Model of Care following unit specific and hospital documentation policies and procedures.
Anticipates variables affecting patient comfort and alters physical and psychosocial interventions accordingly, using a variety of modalities. Assists patient/family to maximize sense of control and actively participate in his/her recovery.
Demonstrates commitment to meeting the learning needs of patients and families. Utilizes appropriate resources to meet those needs and achieve positive patient outcomes.
Works with Care Managers during the patient's stay and takes a proactive role in coordinating interdisciplinary discharge planning for a specific patient population.
Reviews all orders on patients and communicates changes in patient condition with physician and other team members. Contacts the physician promptly with significant changes in patient's condition, collaborates professionally for required orders and follows-up with revised care.
Demonstrates ability to plan, supervise, instruct and evaluate ancillary nursing personnel, floats, orientees and registry staff.
Upholds professional appearance and demonstrates such through adherence to dress code. Wears hospital I.D. badge and promotes the standard of proper identification for peers and staff.
Maintains a safe and clean environment that complies with regulatory standards including Patient Safety Goals.
Participates in the implementation of the unit specific Quality and Performance Improvement Plan, completes monitoring forms and reports findings to the Clinical Unit Supervisor and department leader.
Supports own professional development by participating in committees, cross-training to various roles, and precepting new graduates, transition of practice, and new hires.
Responsible for ensuring department is ready for patient care at all times, including but not limited to preparing patient rooms, maintaining cleanliness, and maintaining department survey ready.
Provide basic hygiene and ADL's to patients.
The nurse will exhibit flexibility in transfer of location throughout the shift as patient census dictates.
Any other duties as assigned by the department leader.

POSITION REQUIREMENTS
A. Education
* Associate in Nursing, Nursing Diploma, or equivalent. Bachelor of Sciences degree in nursing preferred.
B. Qualifications/Experience
Minimum of one (1) year of clinical nursing experience in Emergency Department nursing. Internal candidates that have successfully completed the MLKCH Nurse Residency Program or Transition Fellows program or similar will be considered.
Current California Nursing license.
Basic EKG Competence
Certification in Emergency Department Nursing preferred.
