About this role: As a Home Therapies Registered Nurse, you will manage patient care in an independent home therapy program by providing education and support to patients and caretakers who have chosen to dialyze in the comfort of their home. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations. Identifies and evaluates candidates for the Home Therapies Program including assessment of patients' home environment if needed. Performs assessment and identifies barriers of the Home Dialysis candidate's home environment and caretaker readiness and ability to perform dialysis treatments in the home. Admits new patients to the Home Therapies Program using the appropriate processes. Coordinates patient care including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients. Assesses and manages patients' response to home dialysis training and treatment therapy by following prescribed predetermined protocols. Analyzes dialysis data for Home Therapy patients and documents and adjusts or modifies the treatment plan as needed. Provides initial and ongoing education to patient and family on home dialysis treatment and the operation and maintenance of all home therapies equipment. Ensures patient awareness related to transplant and treatment modality options. Administers medications as prescribed or in accordance with approved algorithm(s), and document appropriate medical justification and effectiveness. Initiates or assist with emergency response measures. Completes routine patient evaluations within the clinic setting. Ensures correct laboratory collection, processing, and shipping procedures are performed. Collaborates with the Interdisciplinary Team on the patient care plan. Makes referrals to Social Worker and Registered Dietitian as appropriate. Completes CAP requirements to either maintain or advance within the program. EDUCATION: Graduate of an accredited School of Nursing. Current appropriate state licensure. Current or successful completion of CPR BLS Certification. Must meet the practice requirements in the state in which he or she is employed. We operate a 39-bed Level II Pediatric Trauma Center that sees about 120,000 patient visits annually. More than two-thirds of the Hospital's patients are admitted through the Emergency Department. We have access to all of the Hospital's pediatric subspecialties and their associated physicians. The Department is staffed by board-certified pediatric emergency physicians. In addition to their years of experience in providing pediatric emergency care, our emergency specialists have undergone a minimum of three years of general pediatric training plus an additional two to three years of fellowship training in pediatric emergency medicine. The Clinical Supervisor, in collaboration with and under the direction of the Nursing Department Director/Manager, is accountable for ongoing shift leadership and operations of a clinical department and for nursing care safety and quality. The Clinical Supervisor uses sound human resource and budget principles to guide the daily provision of nursing services to patients and families. The Clinical Supervisor supports comprehensive patient and family services through effective participation with the interdisciplinary team. The Clinical Supervisor is responsible for supervising nursing department associates and providing patient care when needed. Coaches and guides employees under his/her supervision. Demonstrates Professionalism and Excellence by performing as role model for clinical and service excellence. Requirements EDUCATION: Graduate of an accredited school of Nursing. BSN preferred. National specialty certification preferred. CERTIFICATION/LICENSES: Current RN license in the State of New Mexico. Current BLS Certification. Current ACLS Certification or obtain within 6 months. Current PALS/NRP if applicable to area. Current TNCC if applicable to area or obtain within 6 months. Current CPI or equivalent must be obtained within six months for Behavioral Health and Emergency Department. BLS, ACLS and PALS must be issued through American Heart Association. SKILLS: Must be able to demonstrate the knowledge and skills necessary to provide care/service appropriate to the age of the patients served on the assigned unit/department.
