Summary/Objective
The SocialWorker-Case Manager is accountable for a designated patient caseload, while supporting the physicians and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction; as well as, managing the cost of care and providing timely and accurate information to payers.
Essential Job Duties

1. Proactively screens all cases on assigned caseload to identify cases at risk, using approved screening tool, for adverse health consequences post-discharge without adequate discharge planning while following established reporting procedures.
2. Actively monitors and documents information pertinent to care coordination and the progress of patients on assigned caseload; proactively intervening and ensuring tasks are completed accurately, and in a timely manner to promote communication and prevent delays in treatment and discharge of patients.
3. Communicates all elements critical to the plan of care, including the expected date of discharge to the patient/family and members of the healthcare team; as well as, referring cases with complex issues or barriers to discharge to the Case Management Director or for peer review as appropriate.
4. Collaborates and communicates with multidisciplinary team in all phases of discharge planning process, including initial patient assessment, planning, implementation, interdisciplinary collaboration, and teaching and ongoing evaluation. Ensures/maintains plan consensus from patient/family, physician and payor.
5. Initiates the implementation of the discharge plan prior to discharge to avoid delays in discharge and/ or avoidable readmissions to the hospital; evaluating post discharge needs on assigned cases and develops a discharge plan in collaboration with the patient and support person(s) based on needs indicated by evaluation.
6. Delivers regulatory notices including the Important Message, HINNs, and Observation Notice within the timeframe designated to appropriate patients on assigned caseload.

Educate/ communicate with residents and/or responsible parties about Palliative Care vs. Hospice Care and assist in the referral/transition process of residents to end-of-life services and end-of-life decision making.
Procures prior authorization numbers for residents with Medicaid who require ambulance transportation to non-emergency medical appointments.
Assist with scheduling transportation for residents to medical appointments.
Prepare a Social Service Evaluation prior to each resident's care plan to assess changes / areas of need since their last care plan.
Document interactions with residents and/or responsible parties that are reflective of assessments performed, assistance provided and issue resolution.
Discharge preparations with residents and/or responsible parties throughout stay in facility to culminate all community services requested /required.
Prepares care plans including: Advance Directives, DNR, resident personal preferences, behavioral/psychosocial issues.
Facilitates resident room changes including five-day relocation notice, follow-up with resident, responsible party, roommate, physician, and nursing and documentation of aforementioned process.
Reviews resident's psychosocial wellbeing due to loss of a family member, friend, or roommate.
Attends Resident Council meetings only if invited by the Council members and assists in resolution of any issues presented.
Facilitates proper procedure on initiation/completion of Grievance Reports and assists in maintaining the facility Monthly Grievance Log.
Attends and provides quarterly information for Performance Improvement / Quality Assurance meetings including all resident referrals made in last quarter, all behavioral issues addressed/resolved in last quarter, tracking and trending of grievances within the facility during last quarter.
Performs bi-annual reviews of all resident charts to ensure that assessments, documentation, directives and care planning are current, consistent and appropriately placed in the chart.
Maintains knowledge of federal and state regulations for long-term care facilities.
Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patients / residents.
Keep up to date with current federal and state regulations as well as professional standards, and make recommendations on changes in policies and procedures to the department director or Administrator.

What You'll Do
As a key member of the interdisciplinary team, you will:
Clinical Care
Provide individual, family, and group therapy to LTC residents.
Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances, and other conditions common in LTC settings.

Maintain complete and timely documentation using our Geriatric Notes Tool (GNT).
Follow ethical, professional, and regulatory guidelines for clinical documentation, confidentiality, and care delivery.
Facility Partnership
