Specifically supporting: The IM+CANS assessment and treatment planning tool and any related components of the tool or related topics that are necessary. Integration of the IM+CANS into Systems of Care work and Wraparound for Care Coordination and Support Organizations staff and leadership.
* Develop training and coaching curriculum, including on-site training and web-based training on intermediate and advanced use of the comprehensive behavioral health assessment tool and its relevance to effective treatment planning for children and families. Provide feedback to Field Services Coordinator and Workforce Development Team as Subject Matter Expert on new services, the IM+CANS implementation efforts to guide future curriculum development and statewide support strategies.
* Plan and deliver both web-based training, including questions and answers, and on-site training statewide for: The IM+CANS assessment and treatment planning tool and any related components of the tool or related topics that are necessary. Integration of the IM+CANS into Systems of Care work and Wraparound for Care Coordination and Support Organizations staff and leadership. Training of newly launched Medicaid services to support providers in the successful development, implementation, and analysis.
* Other duties as assigned. Some analysis of training effectiveness may be required. This training will be based in Systems of Care (SoC) principles. Statewide travel may be necessary several days per week in order to effectuate trainings.

Knowledge, Skills and Abilities
Self-motivated and able to work semi-independently to train peers in assessment and care planning. Working knowledge of change management strategies, established training, coaching and transfer of learning methodologies, case study and clinical case presentation methodologies, established evidence-informed child behavioral health practices and clinical interventions, the impact of trauma on the child and family system, child and adolescent development, issues related to diagnosis and co-morbidity, and other behavioral health issues.
Appointment Information
This is a 100% full-time Academic Professional position, appointed on a 12-month basis. The expected start date is as soon as possible after the search posting closes. The budgeted salary range for the position is $65,000- $75,000. Salary is competitive and commensurate with qualifications and experience, while also considering internal equity. Ensures that all activities are performed aligned with the vision of Elara Caring's board of directors, executive team, and the leadership of the Home Health team.
Assesses patients to identify the psychosocial, financial, and environmental needs of patients as evidenced by documentation, clinical records, case conferences, team report, call-in logs, and on-site evaluations.

Initiates and revises the Plan of Care in response to identified patient care issues.
Writes physician orders to cover additional visits and changes to the plan of care, per agency policy.
Incorporates patient care goals established in the plan of care, as evidenced by documentation in clinical note.
Performs appropriate skilled services/interventions in accordance with accepted standards of practice and certified by the patient's physician.
Counsels, instructs, and includes the patient and family in following the Plan of Care and meeting socialwork-related needs.
What is Required?
Master's Degree or Doctoral Degree in SocialWork from a school of SocialWork accredited by the Council of SocialWork Education
1+ year of socialwork experience in a healthcare setting.

50% travel required

* Provide counseling service to students as indicated on IEPs, 504 plans, or after following the counseling referral process for any student needing counseling services
* Conduct student group activities as deemed appropriate by counselor in collaboration with principal
* Collaborate with socialworkers and administrators to determine needs of students
* Stay current with education reform and challenges facing today's students
* Maintain and submit accurate and timely records/reports as required
* Work with individual teachers to assist with SEL situations that will help improve the teacher's effectiveness in working with elementary students.
* Be available, when possible, for student conferences.
TERMS OF EMPLOYMENT:

While performing the duties of this job, the employee is required to handle work which deals mostly with people, objects, and equipment in a general setting. The employee is occasionally required to stand, walk, bend, stoop, and kneel. The employee may be required to lift up to 20 pounds.
Specific vision abilities required by this job include close vision, depth perception and ability to adjust focus with or without correction.
The employee must recognize differences in sound, such as voices/noises that are loud and playful instead of angry and combative. The employee must have the ability to differentiate tones and volumes in conversation. The employee should hear in the normal audio range with or without correction.
WORK ENVIRONMENT:
The noise level in the work environment is usually moderate.
MENTAL FUNCTIONS:
While performing the duties of this job, the employee regularly is required to compare, analyze, communicate, coordinate, compile, instruct, synthesize, evaluate, use interpersonal skills, negotiate, and maintain emotional control under stress. The employee frequently is required to compute. The employee occasionally is required to copy.

Your Key Responsibilities
