Specific Responsibilities:
Provides and accurately documents direct social services to patients planned to restore the patient to optimum social and health adjustment; including:
Assesses social and emotional factors, to estimate impact on the patient's capacity and potential to cope with problems of daily living
Helps the patients and caregivers to understand, accept, and follow medical recommendations
Assists patients and caregivers with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care; and
Identifies resources, such as caregivers and community agencies, to assist the patient to resume life in the community, including discharge planning
Provides these services according to physician order and in accordance with home care policies and procedures.
Educates and supervises designated socialwork assistants in the implementation of the social service program.
Provides inservice training to other home care team members, focusing on psychosocial needs of patients, families and staff, as requested.
Contributes to applicable QI activities as requested.
Serves as Utilization Review and/or Advisory Board Member as requested by Administrator/Director of Services.
Maintains knowledge base of home care social services procedures and regulations as well as professional standards through attendance at workshops and inservice and individual research and reading.

6. Encourage awareness of substance abuse issues throughout the facility.
7. Develop linkages with community resources for substance abuse services.
8. Promote the development of positive social skills through modeling appropriate behaviors and intervening when inappropriate behaviors are observed.
9. Maintain accountability of inmates and property; adhere to safety practices.
It is expected that the incumbent shall perform other duties as assigned within his/her capabilities as determined by management.
**Education and Experience Requirements:**
Bachelor's degree in socialwork, counseling, psychology, criminology or related field and where applicable, must possess appropriate certification. Two (2) years of substance abuse treatment experience preferred. Directly related experience may be considered in lieu of formal education requirements if approved by management. Valid driver's license in the state of Florida with an acceptable driving record required, unless waived by management.

Responsibilities
The licensed socialworker administers social services to patients requiring intermittent professional services and teaches the patient, family, and other members of the health care team. These services are performed in accordance with the physician's orders and the established plan of care.
Qualifications
Education and Experience (ESSENTIAL):
Must possess a bachelor's degree from a school of socialwork accredited by the council on socialwork education.
Must be certified or licensed in the state where they currently practice.
Must have at least one year of socialwork experience in a health care setting.
If employment with the hospice branch of Enhabit Home Health & Hospice began prior to
December 2, 2008, may not require supervision by a licensed master of socialwork.
Education and Experience (DESIRED):
May have a bachelor's degree in psychology, sociology, or other field related to socialwork; must be
certified or licensed in the state where they currently practice.
Medicare home health or hospice experience is preferred.
Education and Experience (STATE SPECIFIC):
ALABAMA - A BSW can work under an MSW; MSW is preferred.
ALASKA - A BSW can work under an MSW; MSW is preferred.
ARIZONA - A licensed BSW must work under a licensed MSW; LMSW is preferred.
ARKANSAS - Must be an LMSW, at minimum.
COLORADO - Must be a masters level LSW.
CONNECTICUT - Must be an MSW.
FLORIDA - No state liensure is required, certification only. Must provide masters degree transcript.
GEORGIA - Must be an LMSW, at minimum.
IDAHO - A BSW can work under an MSW; MSW is preferred.
ILLINOIS - A BSW can work under an MSW; MSW is preferred.
INDIANA - A BSW can work under an MSW; MSW is preferred.
KANSAS - An LBSW must work under a LMSW; LMSW is preferred.
KENTUCKY - An LBSW must work under a LMSW; LMSW is preferred.
LOUISIANA - Must be an LMSW.
MARYLAND - An LBSW must work under a licensed clinical socialworker; a masters level LCSW is preferred.
MASSACHUSETTS - An LSW can work under a LCSW; LCSW is preferred.
MISSISSIPPI - A BSW can work under an MSW; MSW is preferred.
MISSOURI - State licensure is not required. A BSW can work under an MSW; MSW is preferred.

Qualifications:
Must be organized and detail-oriented.
Must be able to communicate effectively both orally and in writing.
Must be capable of making independent decisions relevant to the patient's needs.
Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Must function well in atmosphere of stress and interruption.
Requirements:
Must possess a valid state driver license
Must maintain automobile liability insurance as required by law
Must maintain dependable transportation in good working condition
Must be able to safely drive an automobile in all types of weather conditions
Must possess CPR certification for the healthcare provider
