Your Role:
Provide supportive 15 - 30-minute behavioral health services to primary care providers (PCPs) and their patients (warm hand-offs), partnering with the PCP in improving the overall of health the patient
Provide 30 - 60-minute behavioral health services to patients that can include complete history, assessment, diagnosis, treatment, and/or referral within or outside the health center
Conduct and facilitate support and treatment groups with patients of any age
Refer those cases that require support (e.g., social work, WIC) or specialist services (e.g., psychiatry, inpatient treatment), but maintain responsibility, assuring that continuity of care is provided
Ensure all documentation meets ethical and legal standards for every encounter with a patient
