Responsibilities:
Provides socialwork services in accordance with the physician's plant of treatment. Accepts only clients referred by the client care manager.
Assists the physician and other members of the home health/hospice team to understand the significance of social and emotional factors related to the client's care. The goal is to teach and promote a healthy lifestyle and psychosocial wellness for eventual patient & caregiver independence, utilizing agency materials as appropriate.
Assesses and reassesses social and emotional factors in order to help the client and family to cope with problems in daily living and to help them follow medical recommendations to maximize the client's health condition.
Counsels clients and families concerning the facilitation of the medical plan of treatment.
Recognizes and utilizes community and family resources to assist the client in long-term plans for life in the community or to learn to live within the client's disability.
Develops the plan of care with the case manager, physician, client and family to deal with personal, financial and environmental difficulties that may interfere with the health and wellness continuum. Sets appropriate goals for the patient. Participates in discharge planning.
Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies.
Keeps abreast of socialwork trends and knowledge. Participates in in-service program.
Coordinates the total plan of care and maintains continuity of client care by liaising with other health professionals. Attends client care conferences. Initiates client care conferences for complex and/or multidisciplinary clients when needed or helpful to ensure good coordination of care.
Participates in the agency's quality improvement program.
Cooperates with appropriate staff to provide staff education when requested and arranged by the client care manager.
Demonstrates a desire to set and meet objectives and to find increasingly efficient ways to perform tasks.

Mental Health and Social-Emotional Supports
Provide classroom consultation to staff regarding child mental health, social-emotional development, and behavior regulation.
Conduct classroom observations and collaborate with teaching teams to develop individualized social-emotional support plans.
Provide short-term individual and small-group interventions for children with identified needs.
Develop and implement Plans of Care for children requiring targeted intervention, including obtaining parental consent and engaging the family.

As a Medical SocialWorker (MSW), you'll contribute to our success in the following ways:
Ensures that all activities are performed aligned with the vision of Elara Caring's board of directors, executive team, and the leadership of the Home Health team.
Assesses patients to identify the psychosocial, financial, and environmental needs of patients as evidenced by documentation, clinical records, case conferences, team report, call-in logs, and on-site evaluations.
Make the initial socialwork evaluation visit and reevaluate the patient's socialwork needs during each following visit.
Communicates significant findings, problems, and changes in condition or environment to the Supervisor, the physician and/or other personnel involved with patient care.
Reports unsafe conditions and the outcome of each visit to the appropriate Supervisor by the end of the day.
Implements the plan for patient safety, using patient, family, and community resources.
Participates in implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning.
Verifies the Plan of Care prior to each visit and provides care according to physician's orders, assessment data, and established standards and guidelines.
Initiates and revises the Plan of Care in response to identified patient care issues.
Writes physician orders to cover additional visits and changes to the plan of care, per agency policy.
Incorporates patient care goals established in the plan of care, as evidenced by documentation in clinical note.
Performs appropriate skilled services/interventions in accordance with accepted standards of practice and certified by the patient's physician.
Counsels, instructs, and includes the patient and family in following the Plan of Care and meeting socialwork-related needs.
What is Required?
Master's Degree or Doctoral Degree in SocialWork from a school of SocialWork accredited by the Council of SocialWork Education
1+ year of socialwork experience in a healthcare setting.
Current State License as a SocialWorker
Excellent verbal and written communication skills
Socialwork skills as defined as general socialwork standards
50% travel required
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

POSITION SUMMARY:
Under general supervision of a Mental Health Supervisor (Children's Services), provides psychotherapy and some case management services to child and youth consumers and their families for Saginaw County Community Mental Health Authority (SCCMHA). Is responsible for providing comprehensive diagnostic assessments, treatment planning, and therapy services for a diverse caseload including children, adolescents, and families experiencing mental health issues, communication challenges, or family conflict. Compiles data to obtain complete primordial and current history of personal, social, and emotional situations. Treatment will utilize individual, family and group treatment methods with emphasis on the use of evidence-based treatments, and promising practices as clinically appropriate; consultation and education with other community agencies and groups; and crisis intervention services. This position will be deeply familiar with additional services and resources within the community and have the ability to effectively and efficiently communicate with others to provide information and support when required. Treatment is provided in-home, in the agency, or in the community will be based on clinical need. This position will be knowledgeable about and actively support culturally competent recovery-based practices; person centered planning as a shared decision-making process with the individual/family, who defines their life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process.
ESSENTIAL DUTIES AND RESPONSIBLITIES:
1. Provides a full-range of psychotherapeutic services, with emphasis on the use of evidence-based treatments, and promising practices to the identified consumer and their family using both outpatient and in home service delivery modality.
2. May provide case management services (linking, monitoring, and advocacy) to the identified consumer and their family, as clinically appropriate, using both outpatient and in home service delivery modality.
3. Completes and documents assessment interview with identified consumer and their family to obtain data concerning personal, social, emotional, and mental history of identified consumer and their family to inform accurate diagnosis and treatment planning.
4. Provides direct mental health services. Formulates individual plans of service designed for specific symptoms; completes social assessments; monitors progress of service and treatment from individual, family, and group therapy sessions; completes quarterly reviews and considers treatment options.
5. Works with parents and providers to develop an action plan that identifies appropriate interventions for a youth who needs mental health support and oversees implementation of the plan.
6. Provides programmatic consultation on certain elements of a program to address specific issues that affect more than one child, staff member and/or family.
7. May routinely and regularly make home, school, hospital, court, and visits to other designated sites in order to provide a full range of mental health services. Primary location of service provision is based on unique clinical needs of identified consumer and their family.
8. Interacts with other SCCMHA units and coordinates treatment with Department of Health and Human Services (DHHS), Juvenile Court, and other agencies.
9. Works to heighten community awareness of the mental health needs of children, adolescents, and families within settings in the system of care for youth.

Requirements
Master's degree in SocialWork from an accredited school
Preferred: One year experience in Home Health Care
Must possess a valid Driver's License, car insurance, and reliable transportation

RESPONSIBLE TO: Executive Director of Special Education and/or Supervisor of Special Education.
SALARY: $44,133 - $90,774
SUMMARY OF POSITION: The School SocialWorker provides direct and/or indirect socialwork services to students with IEPs (meets eligibility for special education programs/services). Direct service, as identified in the IEP, will vary in form, length, intensity, and methodology depending upon the students' identified needs. Indirect service will consist of serving as a liaison, consultant, and advocate on behalf of the student, family, and/or school. The School SocialWorker may assist teachers, principals, counselors, and parents in understanding and treating the social and emotional behavior of students in special education or those suspected of having a disability. School SocialWorkers are a crucial member of the behavior intervention planning team.
ESSENTIAL JOB RESPONSIBILITIES:
Assessment/Multidisciplinary Evaluation Team Responsibilities
The School SocialWorker participates with the Multidisciplinary Evaluation of students suspected of having a disability as required by law or as identified in the Evaluation Review process.
Assist with the coordination of referrals, and pre-referral consultations.
Explain parental rights as part of the referral process.
Review the contents of the Parent Handbook with parents/guardians.
Secure parental permission (consent) to test students suspected of being a student with a disability.
Gather developmental data and write a social history with parental input.
Observe students suspected of being emotionally impaired in various settings.
Assist with the initial and re-evaluation process as specified in rule.
Assist in completion of Functional Behavioral Assessments as per operational procedure.
Assist in the development of Behavioral Intervention Plans.
Counseling
Help special education students who are experiencing adjustment problems, by providing direct therapy as written in the IEP or by referring them to a community agency.
Attend IEP Team meetings as necessary for special education students needing and/or receiving services.
Utilize resources found in the school and community to meet the needs of special education students.
Work with parents who need assistance in understanding child growth and child management techniques.
Share with the school staff knowledge of child growth and development and principles of human behavior as they affect the child in the school setting.
Help classroom teachers, administrators, counselors, and parents understand and plan for the social-emotional problems that special education students experience.
Assist in conducting in-service training programs for parents, teachers, and/or administrators on request.
Serve as a liaison to community agencies.
Provide crisis intervention on behalf of special education students.
Reports and Records
Complete necessary and appropriate reports.
Write goals and instructional objectives for all students receiving direct service.
Complete and/or assist in the completion of student progress reports.
Maintain appropriate and orderly student files.
OTHER RESPONSIBILITIES:
Carry out Primary Caseload Provider responsibilities for students where the SSW is identified as the only service provider.
To use proper phone etiquette at all times.
Other duties as requested by Director.
Maintain professional competency by attending staff meetings, conferences, workshops, and participating in professional organizations.
Willingness to learn and utilize basic keyboarding computer skills.
Willingness to receive additional training to improve job skills.
Participate in the District Planning Process and Work Teams.
MINIMAL QUALIFICATIONS:
Meet the State Special Education requirements to be a School SocialWorker as listed in Rule 340.1012.
Demonstrate oral and written communication skills.
Demonstrate conflict resolution skills.
Demonstrate skills to work effectively with staff, school administrators, and outside agencies in the delivery of special education services.
Demonstrate the ability to work successfully with students, parents, staff, community, and advocacy groups.
Demonstrate the ability to function in a total organizational context.
Must have a valid driver's license and acceptable transportation.
