Scope of work
Care Management team members provide quality patient care under the direction of the Care Management Leadership team according to Corewell Health policies and procedures and recognizes the impact of care and partnerships with patient/families to reach high quality patient outcomes. Care Managers drive patient-centered care forward. They are key contributors on the interdisciplinary team, ensuring that the patient's voice is heard and that their needs are met. Not only are the team members experts in their respective fields, but they are also master problem-solvers, detectives, and advocates, connecting patients to necessary services. Depending on where your Care Manager is located, you may notice expert care given in support of an end-of-life journey, a significant transition from home to the next level of care, therapy provided to support a mental health journey or life change, or special attention to a certain diagnosis that needs education, support, and resources to improve a patient's overall health outcome. The Care Management Teams focuses on whole person care, paying special attention to social factors that are impacting a patient's overall wellness.

Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies.
Keeps abreast of socialwork trends and knowledge. Participates in in-service program.
Coordinates the total plan of care and maintains continuity of client care by liaising with other health professionals. Attends client care conferences. Initiates client care conferences for complex and/or multidisciplinary clients when needed or helpful to ensure good coordination of care.
Participates in the agency's quality improvement program.
Cooperates with appropriate staff to provide staff education when requested and arranged by the client care manager.
Demonstrates a desire to set and meet objectives and to find increasingly efficient ways to perform tasks.
Completes work, care and documentation with accuracy and within agency time frames.
Requires minimal supervision and is self-directed.

Responsibilities:
Provides socialwork services in accordance with the physician's plant of treatment. Accepts only clients referred by the client care manager.
Assists the physician and other members of the home health/hospice team to understand the significance of social and emotional factors related to the client's care. The goal is to teach and promote a healthy lifestyle and psychosocial wellness for eventual patient & caregiver independence, utilizing agency materials as appropriate.
Assesses and reassesses social and emotional factors in order to help the client and family to cope with problems in daily living and to help them follow medical recommendations to maximize the client's health condition.
Counsels clients and families concerning the facilitation of the medical plan of treatment.
Recognizes and utilizes community and family resources to assist the client in long-term plans for life in the community or to learn to live within the client's disability.
Develops the plan of care with the case manager, physician, client and family to deal with personal, financial and environmental difficulties that may interfere with the health and wellness continuum. Sets appropriate goals for the patient. Participates in discharge planning.
Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies.
Keeps abreast of socialwork trends and knowledge. Participates in in-service program.
Coordinates the total plan of care and maintains continuity of client care by liaising with other health professionals. Attends client care conferences. Initiates client care conferences for complex and/or multidisciplinary clients when needed or helpful to ensure good coordination of care.
Participates in the agency's quality improvement program.
Cooperates with appropriate staff to provide staff education when requested and arranged by the client care manager.
Demonstrates a desire to set and meet objectives and to find increasingly efficient ways to perform tasks.
Completes work, care and documentation with accuracy and within agency time frames.
Requires minimal supervision and is self-directed.

POSITION SUMMARY:
Under general supervision of a Mental Health Supervisor (Children's Services), provides psychotherapy and some case management services to child and youth consumers and their families for Saginaw County Community Mental Health Authority (SCCMHA). Is responsible for providing comprehensive diagnostic assessments, treatment planning, and therapy services for a diverse caseload including children, adolescents, and families experiencing mental health issues, communication challenges, or family conflict. ESSENTIAL DUTIES AND RESPONSIBLITIES:
1. Provides a full-range of psychotherapeutic services, with emphasis on the use of evidence-based treatments, and promising practices to the identified consumer and their family using both outpatient and in home service delivery modality.
2. May provide case management services (linking, monitoring, and advocacy) to the identified consumer and their family, as clinically appropriate, using both outpatient and in home service delivery modality.
3. Completes and documents assessment interview with identified consumer and their family to obtain data concerning personal, social, emotional, and mental history of identified consumer and their family to inform accurate diagnosis and treatment planning.
4. Provides direct mental health services. 5. Works with parents and providers to develop an action plan that identifies appropriate interventions for a youth who needs mental health support and oversees implementation of the plan.

Requirements
Master's degree in SocialWork from an accredited school
Preferred: One year experience in Home Health Care
Must possess a valid Driver's License, car insurance, and reliable transportation
What you'll do:
Conduct initial assessments, and help create goal-directed care plans
Collaborate with physicians and participate in interdisciplinary team meetings, sharing insights about patients' needs
Record all patient and family interactions clearly and on time in patient records
Offer reassessments and ongoing counseling for patients and families as needed
Engage in crisis intervention and provide compassionate support to patients and their loved ones
Actively advocate for patient and family needs
Stay connected with community agencies to facilitate smooth referrals
Provide thoughtful counseling around end-of-life issues when needed
