Principal Duties and Responsibilities: Assesses and prioritizes patient/client treatment needs including discharge needs. Implements plan of care and collaborates with the clinical team to assure a uniform plan of care. Provides individual, group, and family therapy or interventions; psychosocial assessment, including assessment of co-occurring disorders. Provides input to the interdisciplinary treatment plan, functioning as a team member. Establishes therapeutic relationship with assigned patients/clients including engaging, deescalating, screening, and managing crises. Serves as a liaison between community resources, family, patient/client, and hospital- or clinic-based providers of care. Assures appropriate length of stay based on continual reassessment of patient's/client's progress towards goals. Responsible to monitor for and report to the manager, attending physician, or appropriate team leader any potential risk to patient safety that could lead to an adverse outcome. Responsible to document and report all necessary information according to established policies and procedures. Utilizes processes to conserve financial resources. If full or temporary approval from MDE is not in place a waiver can be submitted to MDE allowing the social worker to enroll in the necessary classes to obtain the SSW-310. With the waiver in place, GRPS has agreed to waive the requirements under Article 14, Section J.1.a which requires two years of employment at GRPS before a GREA member is eligible for tuition reimbursement. This waiver only applies to the classes required to obtain the SSW-310 certification. Candidates that access tuition reimbursement will also be required to sign an agreement indicating they will remain employed by GRPS for three years after obtaining their SSW 310 certification. These interventions may include, but are not exclusive to: alternate site coordination, discharge planning, addressing all forms of identified abuse/neglect, adoption, substance use disorders, assisting with end of life decisions, providing appropriate referral resources, bereavement support, adjustment counseling, facilitating guardianships and behavioral health concerns (including psychiatric transfers). * Conducts psychosocial assessments with patient and or family to assist with identified psychosocial needs or who may be identified to require an alternate site of care post discharge and executes a plan. * Communicates with alternate sites of care and/or community agencies in an effective and timely manner, to best address the patient's needs. * Completes and documents the assessment and plan in the electronic medical record, accurately reflecting the patient's current condition, situational factors, transition of care needs and psychosocial imperatives. * Provides consultation and resources to members of the healthcare team. * Responsible for maintaining relevant and current knowledge of community resources. * Participates or assists in department, regional, or statewide stakeholder committees, projects etc., as assigned/requested. * Develops and maintains current knowledge of federal and state regulations as they pertain to role.
