Our team works collaboratively with local county and community-based agencies to provide compassionate, trauma-informed services; including evidence-based mental health treatment, case management, housing and homelessness support, and family-based interventions across Child Welfare, Behavioral Health, and Juvenile Justice systems of careas well as support for individuals and families experiencing complex challenges who fall outside of these social services. We are currently looking to add aSocial Worker, I, II, III, or IVto our dynamic team supporting our WRAP Program! Depending on the education and professional scope of service, the Social Worker provides therapeutic and social work services to individuals, families, and groups experiencing various challenges and issues. They assess client needs, develop and implement comprehensive care plans, and provide therapeutic interventions and functional life skill development in various settings, including homes, schools, and the community. The Social Worker may serve as the team facilitator, a primary role, necessary to coordinate and facilitate interdisciplinary meetings. In addition, the Social Worker must demonstrate effective integration of services to support client recovery and overall well-being. Location: Remote (see below for remote provisions) Employees living outside a 50-mile radius of the San Luis Obispo Main Office (SLO) will be assigned the nearest FCNI office for in-office workdays. Direct Service Employees are classified as Remote, as their primary worksite is considered their home. Employees are still required to attend all meetings, supervision and client meetings in-person. Anticipated start date: January 20, 2025 COMPENSATION Social Worker I & II - Pay Grade 4 & 6 ($23.00 - $42.75) Social Worker III & IV - Pay Grade 8 ($34.00 - $51.00) Full Time, Non-Exempt classification per FLSA Spanish Bilingual differential (If applicable) - 10% increase on the employee's hourly base rate of pay Additional compensation for participating in after hours on-call Mileage reimbursement $0.70 cents per mile BENEFITS Eligible to enroll into benefits on the 1st of the month following employee's first day of employment. Social Worker II (MHRS): Bachelor's degree in Social Work, Psychology, or a related field preferred and four (4) years full-time equivalent experience in a Behavioral Health setting as a specialist in the fields of physical restoration, social adjustment, or vocational adjustment. Social Worker III:Master's degree in social work, Psychology or related field required. In process and/or proof of active intern registration with the Board of Behavioral Sciences, as an Associate Clinical Social Worker or Associate Marriage Family Therapist. Direct experience in a Behavioral Health or Social Services setting such as outpatient clinics, behavioral health facilities, and/or related community-based organizations. Social Worker IV (Licensed): Master's degree in Social Work, Psychology related field required. Current active and unrestricted California license as a Licensed Clinical Social Worker, Licensed Marriage Family Therapist, Licensed Professional Clinical Counselor, or Psychologist. Direct experience and knowledge of Behavioral Health and Social Services Core Practices in settings such as outpatient clinics, behavioral health facilities, and/or community-based organizations. Develop individualized care plans in collaboration with clients to address their specific needs and goals. Provide direct behavioral support, evidence-based interventions, and life skill development to support clients with achieving their goals. Provide psychoeducation on mental health and substance use topics, such as symptom management, relapse prevention, and developing healthy coping skills. Engage clients in meaningful activities and social interactions to promote mental and emotional well-being, including providing transportation and/or accompanying clients to access community resources and support services. Provide training and support to families, caregivers, and other stakeholders on implementing behavior management strategies and techniques, promoting understanding and collaboration Engage clients in risk assessment and safety planning. Following established protocols and procedures for de-escalation and intervention, provide support, guidance, and the development of coping strategies for managing crisis situations. Participate in supervision to review cases, receive guidance, and collaborate on therapeutic interventions to assist in providing evidence-based, culturally responsive client services. Adhere to ethical standards, professional codes of conduct, and legal regulations governing clinical practice. Address ethical dilemmas and boundary issues that may arise in clinical work. To ensure that interventions are effective, and goals are being met, monitor and document client progress and adjust plans as needed. Maintain accurate and timely clinical documentation in accordance with agency timelines, policies, and confidentiality standards. Ensure documents meet Medi-Cal and other regulatory requirements. Coordinate and facilitate team meetings and collaborate with families, caregivers, teachers, and other professionals to ensure coordinated care for clients. Participate in professional networking and collaboration with other contractual and/or community organizations to enhance clinical services and available resources for clients. Participate in professional development activities and training to remain informed about Evidence Based Practices and to support high-quality innovative client services.     Salary Range: $35.40 - $42.07 Pay rates are determined based on experience and internal equity. * Reports to the Director of Case Management * Provides biopsychosocial assessment, crisis intervention, counseling, and linkage with resources for inpatients, outpatients, their families and significant others. * Assists patients and their families in coping with psychosocial problems related to acute and chronic illness, treatment regimes, and adjusting to an altered lifestyle in order to promote effective and timely discharges. Position Duties/Responsibilities: * Assesses patient's biopsychosocial needs and options. Interviews patient and family members to determine medical-social and emotional needs and identifies personal, family, and community resources that are available. * Facilitates referrals to other services ( e.g. substance abuse treatment, specialty care and community resources) as needed. * Applies knowledge of developmental issues of all ages, from early childhood to the elderly, in order to relate to patients of all ages. * Provides crisis, short-term and extended social work treatment to patients and families to maximize appropriate use of medical treatment. * Develops discharge/treatment plans with patients and families specific to spiritual and cultural values, including professional assessment of psycho-social strengths and weaknesses in the context of the illness and treatment program. * Communicates the discharge/treatment plan to attending medical and nursing staff. * Maintains documentation of all services in the medical record. * Participates in and/or facilitates team meetings. * Sensitive to issues of cultural diversity. * Maintains knowledge of LVMC Policies and Procedures. * Manages clinical resources for patients and regulatory compliance. * Performs related duties as requested. Essential Functions: * Maintains appropriate Social Worker certification, as required. * Demonstrates familiarity and comfort working with issues related to homelessness, substance abuse, mental health diagnoses, and history of trauma. * Demonstrates experience in psychosocial assessment, individual/group therapy, short-term, evidence-based counseling, and crisis intervention * Demonstrates the ability to function as a team member. * Demonstrates the ability to have positive personal interactions with staff, patients, visitors, and physicians. * Accepts appropriate supervision. * Communicates concerns appropriately using the established chain of command. Position Qualifications: * Education: Master's level social worker (MSW). * Experience: At least 1 year of appropriate experience preferred. * Skills/Ability: Excellent verbal and written communication skills. Computer skills required. * Exhibits the ability to organize work assignments and follow through with accuracy. * Exercises good judgment, demonstrates initiative, emotional stability, tact, and poise. * Bilingual preferred. * LVMC reserves the right to modify the minimum requirements depending on the needs of the organization.
