Job Summary Works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. Maintains ethical principles and professional standards of practice in adherence to the discipline's professional code of ethics. Job Responsibilities Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate. Collaborates with the interdisciplinary Team to assess for appropriate level of care and risk factors; develops and implements care/discharge plan and coordinates consults/referrals to remove barriers and promote optimal clinical and financial outcomes. Utilizes specific therapeutic methods/strategies/interventions in accordance with individualized care plan. Monitors progress of interventions and revises plan to meet patient/family needs. Provides education to patient/family with respect to meeting established goals including but not limited to disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens. Provides direct patient care that may include: case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end-of-life care, referral and discharge planning. Refers patient to appropriate levels of care and available resources that promote the individualized care plan. Maintains timely patient-centered documentation that reflects the patient and client systems' pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies. Performs clinical diagnosis in conjunction with conducting a comprehensive bio-psychosocial assessment and establishment of a care plan. Provides evidence based in-home treatment to parents/infants focusing on attachment and needs to both the caretaker and infant. Champions integrated care model treatment guidelines in the primary care setting by providing approximately 6-8 sessions of short-term, focused treatment. Independently able to provide clinical social work interventions to all patient populations. Participates in administrative and development activities including team meetings, clinical supervision of staff and staff education. Provide social work field supervision to second year social work students. Supports the development of hospital and/or unit-based programming to meet the clinical and strategic needs of the patients and the milieu. Works as a team member to contribute to improved patient experience. Ensures the cost-effective utilization of resources. This position is remote, located in New York/New Jersey and requires up to 50% travel. Starting pay is $72,134.40 and may be subject to change on experience, education, and cost of living. DUTIES AND RESPONSIBILITIES Provide culturally and linguistically appropriate clinical case management services to UC with significant needs. Carry a caseload of 5 to 10 EPRS cases. Conduct brief clinical interventions to help families address crises and manage conflict, as needed. Complete individualized needs assessments with unaccompanied children (UC) and sponsors, and develop case plans with particular attention to culture, language, and special circumstances. Maintain a high standard of record-keeping according to USCRI and ORR policies and procedures. Provide information, education, referrals, outreach, advocacy, and support to ensure that each child receives the services they require. Assess the safety and appropriateness of the placement on an ongoing basis using child welfare principles and social work best practices through phone/video calls, interviews, and home visits. Build a network of local pro bono legal resources and service providers to facilitate access for unaccompanied children (UC) and sponsors, and track individual progress. Coordinate and maintain communication with key stakeholders. Participate in regular training and communication sessions with supervisor and headquarters staff. Other duties as assigned by supervisor. Experience in child welfare, child placements, family preservation, behavioral health counseling, and social service provision is preferred. Knowledge of trauma-responsive service provision and crisis intervention skills. Must be skilled at writing professional, thorough assessment reports. Must possess an extensive knowledge of immigrant community local resources. Excellent organizational, verbal, written, and interpersonal communication skills, and ability to prioritize duties in a fast-paced environment. Must have a valid driver's license, insurance, a clean driving record, and personal transportation. Position requires up to 50 percent long distance travel with limited notice. Position is remote and the candidate must have an appropriate workspace to complete documentation, client phone calls, and video conferencing. Fluency in Spanish is required; proficiency in additional languages a plus. Candidate must demonstrate a commitment to comply with mandatory state reporting requirements for child abuse and neglect.
