Job Summary
Odyssey House Detox Program is currently looking for a full-time Licensed/Licensed-Eligible SocialWorker or Counselor. The schedule is M-F 8am to 5pm, rotating holidays and weekends as needed. The Counselor will provide specialized services for the assigned caseload, provide core and ancillary group therapies weekly, individual counseling and when appropriate, family therapy. The Counselor has the main responsibility of maintaining communication with the referral source, court systems, and other involved human service providers as related to the client's ongoing treatment and discharge planning. The Counselor is responsible for working within the multidisciplinary team to develop treatment planning, treatment case reviews, and all written documentation as required by the agency, state and federal courts, and all funding sources.
Responsibilities and Duties
It is the duty and responsibility of the Counselor to:
provide Clinical services to assigned clients; conduct weekly group therapies, individual counseling and/or family therapy as outlined in treatment plans; co-lead group therapies; conduct family assessments/therapy;
function as a liaison to outside agencies, participate in case planning conferences, and treatment meetings and, when necessary, attend court hearings and represent the agency and client(s);
maintain contact with and is available to Agency personnel, client's drug court officer, and family members regarding client's progress, problems and crises;
maintain clinical case file for each client to include: assessments, Individual Treatment Plans,
Case notes, and discharge summaries;
apply ASAM Criteria to treatment planning;
maximize units of services and maintain contract units of service by working with clients to stay engaged in treatment;
provide crisis management when indicated by client need;
assist with the planning and organization of therapeutic client actives both at and outside of the facility;
ensure all written reports are up-to-date in case file;
ensure completion of electronic medical record documentation;
attend staff and other meetings;
attend regularly scheduled in-service training;
participate in quarterly training with staff;
complete all Incident Reports prior to the end of scheduled shift as required by Incident Policy and Procedure;
adhere to all Louisiana Department of Health and Agency contract requirements;
maintain COA standards and participate in COA site audits;
collaborate with other departments;
work with Bayou Health providers for initial authorization and concurrent authorizations;
ensure that all sign in sheets are printed for group and all groups are batched for billing; and
perform all other duties as assigned.
Qualifications and Skills
Minimum qualifications include the following:
Master's Degree in Clinical SocialWork or Counseling
A valid Louisiana State issued license in either counseling or socialwork
At least 1 year of substance use disorder treatment modalities (emphasis on Minnesota Model) and DSM-V experience
Excellent communication skills

Odyssey House Louisiana, Inc. is seeking a full-time SocialWorker for the Community Health Center. The schedule is Monday-Friday 8am-5pm. The SocialWorker will provide comprehensive assessment and diagnosis of behavioral health clients. The position will also assist primary care providers and Program Managers with implementing disease management programs; participate in the development, review, and evaluation of the family and/or client's plan of care; utilize information from various sources to identify patients who would benefit from disease management and case management services; educate and engage patients in their own care management; identify social and/or medical barriers that impede health outcomes; link patients to appropriate levels of care/appropriate and supportive services; work collaboratively with a multidisciplinary inter-agency team to facilitate achievement of desired treatment outcomes, monitor patient ED and hospitalization utilization rates; and utilize an Internet based care management system to track patient progress and health outcomes.
Responsibilities and Duties
It is the duty and responsibility of the SocialWorker to:
coordinate/facilitate patient care;
provide direct care to patients within the Scope of Practice for which the provider is licensed;
provide direct care to clients/patients to support the agency's department;
manage patient care under Best Practices, UDS recommendations, and as prescribed to maintain accreditation;
conduct thorough documentation of visits and medical orders to support billing charges;
complete billing components of visit to optimize revenue within dictated time frames;
work collaboratively and maintains communication with patient care team (providers, nurses, etc.) to provide effective, timely, and appropriate patient care management;
assist patients and providers to adhere to evidence-based treatment protocols for specific disease states;

monitor patient care by tracking patient charts, counsel patients during physician visits, follow up through telephone calls and home visits as needed, and utilize Internet based case management tracking system;

collaborate with relevant community-based social services and health care organizations (socialworkers, home health care, school nurses, hospice providers, etc.) to address patient needs/barriers to improved health outcomes and to secure necessary care and equipment;
address/resolve system problems impeding progress; proactively identify and resolve delays and obstacles to expedite care;
utilize advanced conflict resolution skills as necessary to ensure timely resolution of issues;

refer cases and issues to PCP and follows up as indicated; follow up on referrals from PCP as appropriate;

participate in training as necessary to successfully complete job responsibilities, including training in disease management protocols, care management strategies, use of network software programs, and data analysis skills;

Job Description
The Opportunity
Walter L. Cohen College Prep High School is seeking a dynamic School SocialWorker who will bring unique knowledge and skills to the school and the student services team. School SocialWorkers are concerned with a student's functioning in school especially when school adjustment difficulties, poor school attendance and/or social problems impede pupil educational advancement. The School SocialWorker might serve on the Schools MTSS Team and as a member of the Pupil Appraisal Team. This includes but is not limited to conducting formal and informal assessments, communicating with parents, providing behavioral interventions and connecting families with community resources.
Essential Functions
Serve as member of the in-school Student Support Services Team (SSS Team) for initial and re-evaluations.
Completes psychosocial histories as part of the student evaluation including a parent and student interview.
Completes classroom observations and teacher interviews in order to provide information on school performance and give impressions and recommendations for next steps.
Provides individual and group treatment as indicated on Individual Education Plans (IEP) and documents all clinical activity.
Writes annual IEP goals and notes monthly progress and advisory reports related to IEP goals
Documents efforts to reach non-responsive students and keeps monthly contact sheets.
Conducts functional behavioral assessments and creates behavior intervention plans (FBA/BIP) as needed for students.
Completes risk assessments for suicidal/homicidal ideation
Participates in annual IEP meetings and Manifestation Determination Hearings, as needed.
Participates on the RTI/MTSS Team and provides services and interventions as assigned including FBA/BIPs, individual counseling, parent interviews and psychosocial histories, community referrals, etc
Advises and collaborates with teachers, case managers, teams on students who exhibit behavior problems and / or failing grades.

Credential and/or Education Requirements:
Possesses a proven track record in developing and managing successful socialwork programs;
Previous experience conducting formal and informal assessments, communicating with parents, providing behavioral interventions, and connecting families with community resources required
Experience working with students from a low income community highly preferred
Minimum of Masters Degree required;
Three years successful socialwork experience required;
The employee should be a Licensed Clinical or Master SocialWorker (LCSW) (LMSW) or Professional Counselor (LPC), licensed in the state of Louisiana.
Recognized as a School SocialWork Specialist and certification as a Qualified School SocialWorker and Child Welfare and Attendance Supervisor by the Louisiana State Department of Education highly preferred
Possesses knowledge of federal, state and local socialwork guidelines.

JOB SUMMARY:
The SocialWorker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and helps coordinate a plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members.
MINIMUM QUALIFICATIONS:
LMSW required - must be under supervision for LCSW; LCSW preferred.
Working knowledge/experience in case management, payors and reimbursement.
Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
Organized and efficient in establishing priorities and performing tasks within specific timeframes.
Self-motivated requiring minimal supervision.
Views problems as a challenge and investigates alternatives which demand creativity, flexibility and the ability to work under stress.
Responsible for maintaining strict confidentiality of all verbal & written information and documents. Familiarity with and appropriate use of community resources.
Preferred Qualifications:
LCSW preferred. Experience in psychosocial and therapeutic counseling preferred.
PHYSICAL DEMANDS:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 50 pounds is required.
