JOB SUMMARY
Under the general direction of the Transitional Care Director the Licensed Clinical SocialWorker will assist in the planning, developing, organizing, implementing, evaluating, and directing our social services and Discharge Planning Programs, and act as a coordinator between patient, family, physician, community agencies and resources, and this facility to assure that the medically related emotional and social needs of the patient are met/maintained in accordance with JCAHO, federal, state, and local guidelines, organizational and departmental policies and procedures. STANDARDS OF PERFORMANCE
Provides a psychosocial assessment of the patient, family, or appropriate support systems in order to implement crisis or support interventions and to maximize coordination of services.
Participates in patient care planning responsive to age-specific needs.
Participates in planning the socialwork component required in selected hospital programs.
Maintains verbal and written communication with the multidisciplinary team on patient's psychosocial adjustment, interventions, and goals through medical record documentation and consultation.
Participates in socialwork and hospital-wide quality assurance activities.
Participates in hospital interdisciplinary activities, such as committees, team planning meetings, informational meetings, and patient /family conferences.
Attends community socialwork, interdisciplinary health care, and community service agency meetings in order to represent Effingham Health System in planning for services, and resources.
Educates the community about Effingham Health System programs and services and education.
Provides consultation to physicians and hospital staff regarding: psychosocial issues, bioethical issues, and discharge problems.
Documents pertinent contacts regarding discharge plans in the Social Services notes of the patient's medical record.
Participate in discharge planning, development, and implementation of social care plans and patient assessments.
Actively seek out new models of patient education, social agencies, governmental agencies, health disciplines, etc., that can be used in our facility to further promote the well-being and health of patients and the surrounding community.
Perform administrative requirements, completing necessary forms, reports oral and written; charting, social service progress notes, etc.
Act as an information resource for the patient, family members (when appropriate), our facility, and the community as regards Medicaid/Medicare, social service programs, agencies, financial aid, transportation, and other services that may/may not be available in the community or to the patient.
Compile and maintain a social service directory listing available community services.
Assist in the development, administering, and coordination of the department policies and procedures.
Make routine visits to new admissions and perform services as necessary.
Follow up with the patient after discharge to ensure that the discharge plan has gone into effect.
Assist the patient and or family in completing and interpreting necessary forms to obtain services.
Perform other duties as requested, required, needed, and or assigned.
Requirements
Minimum Level of Education: Bachelor's degree from an accredited college or university with a concentration in SocialWork or related field required. Master's Degree in SocialWork (MSW) preferred.
Formal Training: Management skills with experience in planning, organizing, implementing, facilitating, interviewing, counseling, and verbal and written communications.
Licensure, Certification, Registration: Must have and maintain an unencumbered clinical license/certification to practice SocialWork with the State of Georgia and have and maintain a BLS CPR certification.

Actively enrolled in or willing to enroll in a Dementia certification course.

Supervise children, classroom, and all activities.
Ensure the children's safety.
Make ongoing, systematic observations and evaluations of each child.
Maintain positive relationships and effective communication with parents/guardians.
Engage parents as volunteers.
Maintain program site and equipment.
Maintain required program records including sign in/sign out rosters.
Monitor and engage in games and activities with children.
Attend and participates in family nights, program activities, staff meetings, and training.
Address emergencies as they occur.
Develop positive, authentic relationships with children from different backgrounds.
Other duties as assigned.
QUALIFICATIONS:
Meet educational and experience qualifications established by state licensing guidelines (State of GA Childcare Licensing).
High school diploma or equivalent required; one year of college preferred.
Must be at least 18 years of age.
Prior experience working with children in a developmental setting & with diverse populations preferred.
Bilingual (Spanish/English) strongly preferred.
PHYSICAL DEMANDS:
Ability to lead and participate in activities in both indoor and outdoor settings, in various temperatures.
Ability to walk, stand, sit, kneel, stoop, bend, crouch, see, hear, listen and lift (up to 50 pounds).

The School Counselor/School Mental Health Worker provides individual and group counseling to students experiencing emotional, behavioral, or social challenges. The School Counselor/School Mental Health Worker supports students in developing coping strategies, improving emotional regulation, and enhancing overall mental well-being. In collaboration with families, teachers, and other school staff, they help create a safe and supportive learning environment, facilitate referrals to community-based mental health services when needed, and contribute to the development of school-wide mental health initiatives.
Minimum Requirements:
+ Licensing/Certification according to state/facility/contract requirement
+ The Counselor may be required to have an Associate's Degree, Bachelor's Degree or Masters in Psychology, Addictions, Sociology, SocialWork, Counseling, Nursing, or some other Human Development major per state, contract and/or facility guidelines.
+ Complies with all relevant professional standards of practice
+ Current CPR if applicable
+ TB questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or state regulation)
+ Must meet all federal, state and local requirements
+ Must be at least 18 years of age
