Position will remain open until filled Job Description Job duties include: Provides, assists with, or coaches individuals on personal care needs with limited supervision; provides crisis interventions (including evidence based practices to maintain the safety of individuals); monitors youth interactions and behaviors; monitors medication administration as prescribed; monitors and documents mental and physical condition of individuals. Essential Functions and Responsibilities: Identify risk, safety plan, make referrals, coordinate services, and plan for a healthy future by providing case management services and coordinating client needs with collateral agency services. Responds to calls, walk-ins, and crises. Provide emergency assistance, confer with law enforcement and prosecutors.
