The LCSW/LPC works in collaboration with physicians, behavioral health clinicians and other staff at The Wright Center for Community Health to address barriers to medical and behavioral health care. The Licensed Clinical SocialWorker/Licensed Professional Counselor addresses complex social needs of the underserved, uninsured, or underinsured community members. This role serves in an expanded clinical role to collaborate with community members, Primary Care Providers, Behavioral Health Providers, dental, community agencies, insurance companies, and other health/community-related entities to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. The Licensed Clinical SocialWorker/Licensed Professional Counselor‘s expertise is sought to assess, develop and implement member care plans as it relates to addressing the social determinants of health and optimizing the members' physical and psychosocial health status. This is to ensure the member of the program has the resources necessary to achieve their highest level of functioning. The age ranges are between 5-17.
REPORTING RELATIONSHIPS
This position reports to the Behavioral Health Manager. No positions report to this role.
DUTIES & ESSENTIAL JOB FUNCTIONS
Works with program leadership in the design, implementation, and evaluation of the program's objectives to the underserved, uninsured, underinsured, and newly insured populations.
Assists with state required functions for Medicaid members including Age Out Transition Procedure and resources for members in substitute care, waiver programs or facing an emergency placement situation.
Assists with developing coordinated care plans for members with complex medical/social/behavioral health needs. Fosters a collaborative team approach by working with the member, family, primary care provider, behavioral health clinician, community agencies, and other members of the treatment team to ensure coordination of services.
Assist to identify outreach, wellness and education planning needs of the community member and communicate findings to the treatment team.
Coordinates referrals between and among physical, behavioral and dental health providers and other community resources to improve overall community member outcomes. Ensures appropriate clinical management information is shared with peers, providers and outside agencies in a timely fashion while securing system privacy standards.
Works closely with members to appropriately apply insurance benefits or obtain insurance benefits. Serve as a resource for the member and the healthcare team.
Maintains required documentation for all program related activities. Collect data and utilize data to adjust the care plan when indicated.
Acts as a patient advocate in order to coordinate required services or to resolve emergency problems in crisis situations. Provides individual and/or family education/counseling to assist in establishing members' overall wellbeing.
Utilizes evidenced based counseling techniques such as motivational interviewing and solution focused skills with focus on conflict resolution, assertiveness, problem solving, and decision making to assist members with effectively negotiating the healthcare continuum.
Documents patient encounters and contacts made on behalf of patients in EMR; completes and submits monthly reports; maintains comprehensive electronic patient files, which include patient notes, release of information, assessments and other medical documents acquired on behalf of the patient.
Educates patients on the proper use of the Emergency Department and provides information for alternatives. Coaches patients in effective management of their chronic health conditions and self-care. Assists patients in understanding care plans and instructions. Motivates patients/patients to be active and engaged participants in their health and overall wellbeing.
Continuously expands knowledge and understanding of community resources and services. Facilitates patient access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and relevant mental health services. Assists patients in utilizing community services, including scheduling appointments with social services agencies and assisting with completion of applications for programs for which they may be eligible.
Facilitates communication and coordinate services between providers and the patients/patients. Coordinates and monitors services, including comprehensive tracking of patients' compliance in relation to care plan objectives.
Works collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the patients, providers, care managers, medical residents, and office staff. Works to reduce cultural and socio-economic barriers between patients and institutions.
Attends weekly huddles and morning/afternoon mini huddles.
Practices 40 hours of patient care per week.

Summary

Document all services provided, student progress, and other relevant information in accordance with Behavioral Health Associates policies and legal requirements.

QUALIFICATIONS
Master's Degree in Applied Behavior Analysis, School Psychology, Special Education, Counseling, SocialWork, or a related field from an accredited college or university, required.

As a Certified Peer Specialist, you will play an integral part in the provision of Peer Support services, all within a supportive team environment while helping individuals access mental health services in Wayne County. The Primary shift hours are Weekend Evenings Saturday & Sunday 4pm-12am or Weekend Overnights Friday & Saturday 12am-8am.
Essential Functions:
• Work with individuals utilizing telephone, walk-in, and mobile crisis intervention services and/or admitted to the Crisis Residential program.
• Work with individuals and the crisis team to assist in the development of recovery plans and goals.
• Assess, develop, implement and recommend modifications to a service plan that is short-term resulting from a crisis event.
• Coordinate with other members of the crisis residential multidisciplinary team to assist individuals with accessing community resources and meeting their treatment goals.
• Provide opportunities for individuals receiving service to direct their own recovery and advocacy processes. • Share personal experiences of your own path to recovery.
• Facilitate psychoeducational support groups and activities.
• Accompany crisis intervention specialists as needed on requests for mobile crisis response.
• Teach and support the acquisition and utilization of recovery-oriented skills.
• Provide supervision of individuals in treatment to ensure their safety and compliance with facility rules and requirements.
• Engage in the supervision processes required for peer support services.

Qualifications: Required:

• Has maintained at least 12 months of successful full or part-time paid or volunteer experience or has obtained at least 24 credit hours of postsecondary education.
• Successful completion of the Peer Support Specialist training will be required upon hire, if not previously obtained. • Identifies themselves as a mental health consumer in recovery or as a primary support person for an individual diagnosed with a serious mental illness.
• In recovery with at least two years sustained, current recovery history.

Functions as a member of a multi-disciplinary team for a defined population. Assesses social determinants of health, identifies needs and barriers, contributes to care plan development and goal setting, and identifies and facilitates linkage to network and community resources.
Job Duties
Performs biopsychosocial assessments to identify and assist with barriers to receiving care.
Assesses the need for community and/or governmental agency involvement.
Identifies appropriate resources and coordinates referrals within the network and community to assist patients in meeting their goals.
Collaborates with team members to assist in development of appropriate care plans addressing any social and financial barriers to care.
Fosters a team approach by working collaboratively with all practice and community-based team members.
Documents all outreaches and interventions succinctly and in a timely fashion in the medical record, in accordance with departmental and network policies and procedures.
Promotes patient self-management and empowers patients/families to achieve maximum levels of wellness and independence, and acts as patient/family advocate.
Minimum Qualifications
Bachelor's Degree SocialWork
1 year Medical and/or Case Management in Community Setting or
Less than 1 year with successful completion of a BSW internship in a medical and/or case management in community setting.
Ability to recognize the role of cultural, social, economic, and behavioral factors in the accessibility, availability, and delivery of public health services.
Ability to adjust course of action and plans in relation to others' actions.
Knowledge of community resources and services across multiple counties.
Strong assessment and documentation skills.
