Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Marriage and Family Therapist in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.

Utilizes SW interventions, education, counseling, and support services to address the identified psychosocial needs of patients and caregivers.
ESSENTIAL FUNCTIONS:
Collaborates with hospital providers and staff IDT to develop a plan of care, complete initial and ongoing comprehensive assessments, and plan/evaluate SW interventions.
Conducts admission assessments/information visits and follow-ups on hospitalized patients
Conducts high-risk admission assessments/follow-ups in hospitals where psychosocial issues are present, including but not limited to patients living alone, inadequate caregiving/decision-maker situations, and significant mental health/substance abuse concerns.
Documents in accurately and concisely at the time the service is provided.
Initiates, coordinates, and revises the plan of care on an ongoing basis with input from the IDT, patient, caregiver, hospital providers, and staff.
Participates in hospital discharge planning, when needed, collaborating with hospital case management on the plan.
Participates in IDT meetings to report on the plan and update the comprehensive assessment.
Provides education and support to patients and caregivers regarding diagnosis, care, treatments, medications, and end-of-life expectations in a method or manner that they understand.
Provides education and support to hospital staff.
Supports special needs of patients and families including the pinning of Veteran patients through the We Honor Veterans program.
Communicates effectively with organizational staff, physicians, patients, families, hospital staff, outside contractors, and community agencies.
Anticipates, assesses, and responds to pain and other symptoms of physical, emotional, and spiritual suffering.
Works in collaboration with IDT members, managing the psychosocial care of assigned caseload.

Want to make a difference in the life of a young person? As a Student Assistance Program Specialist at Caron, you'll join a diverse team of over 75 behavioral health professionals and educators. Our Specialists provide services to kids K-12, as well as college students and even athletes.
This position is rooted at the very heart of our communities. You'll work on-site at local schools and child serving agencies to provide the behavioral health support young people need, from prevention of high-risk behaviors to intervention services related to substance use and mental health concerns. Working in collaboration with the school professionals and parents is a primary aspect of the position, and our goal is to build resilience for the young people we work with.
This position offers a full-time (37.5 weekly hours), seasonal work schedule. We offer work hours within a traditional school-year schedule, meaning your summers are free. Applicants should possess at least one year of experience working with youth in an educational or public health setting, and a solid understanding of the effects substance use, including nicotine, has on their development.
This position is contracted for the Solanco School District. Starting salary between $40,000-$42,000, depending on education level and experience. Salary for the position is based on working 1,575 hours during the school year. Salary is paid over 12-month period.
DUTIES AND RESPONSBILITIES:
Provide consultation, assessment case management and referral for students identified by the student assistance teams in each individual school when needed.
Provide technical assistance (i.e. Team Maintenance session's etc.) to all school SAP teams requesting such.
Facilitate entrance to treatment programs for students and school districts when needed.
Deliver Project CONNECT and approved student assistance curriculums to schools and other agencies when requested. This delivery involves group, individual, classroom and assembly facilitation.
Provide in-services on alcohol, tobacco and other drugs to faculty or agency staff when requested.
Provide parent education on alcohol, tobacco and other drugs when requested.
Participate in staff meetings and report progress as well as areas of concern.
Complete all documentation (i.e. KIT entries, reporting etc.) and submit to supervisor.
Market and promote programs to all schools as requested by supervisor.

EDUCATION/ EXPERIENCE QUALIFICATIONS:

Completion of the Project CONNECT Facilitator Training, upon hire when deemed appropriate.
Completion of Caron's Commonwealth Approved Student Assistance Program training, upon hire when deemed appropriate.
To complete other trainings as assigned such as Life Skills, and PATHS.
If in recovery, 2 years of uninterrupted sobriety preferred.
KNOWLEDGE, SKILLS AND ABILITIES:
Effective communication skills including interactive public speaking and active listening skills.
Computer literate and able to work with Microsoft products and its network components.
Fluid communication skills that foster teamwork and high morale. Further, must be able to promote services in the media such as radio, print and television.
Must be familiar with and be able to demonstrate knowledge of the principles of various 12-step fellowships, upon hire.
Knowledge of adolescent development.
Strong presentation and group facilitation skills.
Strong knowledge of chemical dependency, and tobacco cessation and the impact of this on school-based services.
