Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. Job Overview: The PACE Physical Therapist is responsible for the physical therapy program for PACE participants. The PACE Physical Therapist is responsible for evaluating, assessing, and treating PACE participants with musculoskeletal conditions, disabilities, or movement impairments by developing individualized care plans utilizing acute and maintenance physical therapy interventions. The PACE Physical Therapist is a member of the PACE Interdisciplinary Team (IDT) and is responsible for completing physical therapy assessments for new PACE participants and physical therapy reassessments on existing PACE participants as part of the PACE IDT care planning process. The PACE Physical Therapist is responsible for providing physical therapy education to participants, caregivers, and PACE staff to address mobility and functional issues.
