Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES • Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. • Serves as a primary coordinator of all disciplines for well-coordinated patient care. • Notes and carries out physician and nursing orders. • Assesses and coordinates patient's discharge planning needs with members of the healthcare team. • May round with physician in an inpatient setting. • Applies the existing body of evidence-based practice and scientific knowledge in health care to nursing practice, ensuring that nursing care is delivered based on patient's age-specific needs and clinical needs as described in the department's Scope of Service. • As an SSM Health nurse, I will demonstrate the professional nursing standards defined in the professional practice model. Uses the ANA Code of Ethics for Nurses to guide his/her response to the current and evolving health and nursing needs of our patients and our patient populations. • Works in a constant state of alertness and safe manner. • Performs other duties as assigned. EDUCATION • Graduate of accredited school of nursing or education equivalency for licensing EXPERIENCE • No experience required PHYSICAL REQUIREMENTS • Constant use of speech to share information through oral communication. • Constant standing and walking. • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. • Frequent sitting, reaching and keyboard use/data entry. • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. • Frequent use of smell to detect/recognize odors. • Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc. • Occasional lifting/carrying and pushing/pulling objects weighinglbs. • Occasional lifting/moving of patients. • Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements. • Occasional driving. • Rare crawling and running. The Staff Nurse, RN as the primary care giver provides direct patient care and works collaboratively with other care team members, the physician and the patient/family to achieve optimal patient outcomes. The Staff Nurse, RN works under the direct supervision of the Nurse Manager and under the indirect supervision of the Director of Nursing. Position requires 30 minute call. Duties/Responsibilities Assessment and Diagnosis: collects comprehensive data pertinent to the patient's health or the situation and analyzes the assessment data to determine the diagnoses or issues. Outcomes / Evaluation Identification, Planning and Implementation: identifies suspected outcomes for a plan individualized to the patient or the situation and implements the identified plan, coordinating care delivery and employing strategies to promote health and a safe environment. Evaluates progress toward attainment of outcomes and accurately completes all documentation. Quality of Practice: systematically enhances the quality and effectiveness of nursing practice. Education, Professional Practice Evaluation and Research: attains knowledge and competency that reflects current nursing practice and evaluates one's own nursing practice in relation to professional practice standards and guidelines, relevant statutes and regulations. Integrates research findings into practice. Keeps current on knowledge within scope of practice. Chapter 4723-4 Ohio Administrative Code (OAC) requires the RN to remain current. Collegiality: interacts with and contributes to the professional development of peers and colleagues. Collaboration: collaborates with patient, family and others in the conduct of nursing practice. Ethics: integrates ethical provisions in all areas of practice. Resource utilization: considers factors related to safety, effectiveness, cost and impact on practice in the planning and delivery of nursing services. Minimum/Required Qualifications Graduate of an accredited program in Nursing. Bachelor of Science in Nursing (BSN) required within five (5) years from the date of hire.
