Job SummaryProvides safe, ethical, and competent evidence-based nutritional therapy that meets the standards of practice and the scope of practice as stated by the Academy of Nutrition and Dietetics. Job Duties
Using the Academy of Nutrition and Dietetics' and Nutrition Care Process, assesses individual needs through synthesis of information obtained through the assessment interview, food and nutrition and related histories, review of the medical record, and discussion with other members of the interdisciplinary health care team in assigned area
Develops a plan to meet assessed needs including recommendations regarding: the composition of diet and meal patterns and supplements; and the volume, rate, and composition of parenteral and enteral feedings.
Confers with the health care team to develop goals related to patient care.
Documentsin accordance with established standards.
Evaluates nutritional status of individuals and makes recommendations regarding provision of optimal care.
Counsels individuals, families, and care provider on normal nutrition and therapeutic diets with consideration given to the individual's educational, cultural, and socioeconomic background.
Plans and conducts group classes for individuals in various areas of nutrition asindicated.
Writes nutrition related orders for hospitalized patients in accordance with the Clinical Nutrition policy as indicated.
Consults, educates, and advocates on behalf of individuals, groups, and special populations regarding food, culinary, nutrition and health issues, food security and insecurity, nutrition programs, nutrition wellness and resources.
Plans for continuing nutrition care for individuals, as needed,using ancillary resources.
Self-evaluates results and monitors for trends and changes in productivity related to workload.
Compiles statistics and completes Clinical Productivity worksheets on a timely basis.
Participates in departmental initiatives to meet strategic goals.
Actively participates on appropriate committees for the organization and represents the Clinical Nutrition department in decisions made by these committees.

Summary of Position
The Staff RegisteredDietitian (RD) provides personalized nutrition plans and medical nutrition therapy to individuals seeking care through telemedicine Nutrition visits on the Teladoc platform, while creating a professional yet comfortable environment. Utilize Teladoc-approved resources for member-facing support documents and programs, and adhere to quality standards.
Refer to other specialists as needed whether on or off the platform.
Demonstrate and utilize knowledge of other services available to members both internally at Teladoc and externally with the health plan or community.
Work in a collaborative way with internal and external customers to assure members receive optimal care.

Recommends standardized macronutrient/energy prescriptions and standard formula for enteral nutrition support. On physician consult request, able to write orders for enteral nutrition support. Assist in review of interdisciplinary clinical protocols for enteral nutrition support. Collect patient assessment data relevant to parenteral nutrition prescriptions to established protocol and plan for parenteral when appropriate. When appropriate, recommends standardized macronutrient/energy prescriptions for parenteral nutrition support of individuals. Recommends nonstandard parenteral nutrition support prescriptions based on metabolic alterations.
Evaluates dietetic services; provides and gives regular input regarding efficiency of patient food service; assembly, temperature, taste, appearance, distribution and accuracy of trays. Assists in implementing and monitoring the patient menu program, recipe compliance, recipe/menu analysis, production sheet compliance, policies, procedures, guidelines, nutrition education material, job descriptions, and competencies.
Appropriately incorporates knowledge of modified diets, cooking/preparation methods, food composition and local market availability in education sessions. Enhances education with specialized material, websites, visuals, or food models as appropriate. Documents education in the medical records according to procedure.

Using the most current, science-based practices, you will recommend therapeutic diets based on patient nutritional needs, preferences, and changes in treatment, in consultation with the patient's physician. You will review lab results and provide appropriate education for nutritional interventions on such topics as fluid and sodium, potassium, and CKD-Mineral and Bone Disorder management. You will also conduct patient assessments and care planning activities, as well as participating in monthly interdisciplinary patient care conferences.
Teamwork. You will collaborate with the Medical Director, physician/advanced practice provider, facility administrator, social worker, nurse, and patient care technician to maintain positive relationships with area hospitals, skilled nursing facilities, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes, with a focus on improving patient health and achieving target goals for patient outcomes. You will also help maintain a productive quality assurance program and participate in monthly Quality Assessment and Performance Improvement (QAPI) activities.
The Dialysis RegisteredDietitian position is for our Alvin Clinic, located at 100 E House St, Alvin, TX 77511, and for our Texas City Clinic, located at 6518 Memorial Dr, Texas City, TX 77591.
The position is full-time with benefits, offering an initial schedule of 30 hours a week, with potential for growth to 40 hours a week.
What we're looking for
• Current RegisteredDietitian (RD/RDN) certification showing proof of registration with Commission on Dietetic Registration (includes Baccalaureate or Advanced degree).
• Minimum of one-year experience in clinical nutrition as a RegisteredDietitian.
• Current Dietitian (LD/LDN) license in applicable state is required unless employed in the state of AZ, CA, CO, HI, IN, MI, NJ or VA. State certification (Certified Dietitian/Nutritionist) required in CT and NY. License must be maintained as current and in good standing.
• Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
• Must have basic computer skills, including Microsoft Office (Word, Excel, Outlook); proficiency in all company clinical applications required within 90 days of hire.
• Must meet any practice requirement(s) for the applicable state.

Requirements
The ideal Outpatient RegisteredDietitian candidate will possess:
2 years of post-internship clinical experience required.
Master's degree or advanced certification: CDCES, CSSD, CSOWM, etc. preferred.
Must enjoy a fast-paced environment, provide patient-centered care and counseling, demonstrate flexibility and adaptability to change, exhibit high level of motivation and drive to stay current with nutrition trends and research, work well in a team environment, have strong communication skills: interpersonal, written and verbal skills.
